To make REGISTRATION as easy as possible, please follow these simple instructions:

1. Use a separate form for each registrant. If payment is through a purchase order or institutional check for more than one registrant, please include a separate registration form for each individual and attach a copy of the purchase order to each form; purchase orders must list each attendee’s name. ATTN: SCHOOL DISTRICT BUSINESS OFFICES: PLEASE DO NOT INCLUDE HOTEL PAYEMENT OR DEPOSITS IN PURCHASE ORDER TO THE SPONSORING GROUP.

2. Send registration payment by check, money order, or purchase order with the form. Do not send cash. Please note that purchase orders must be paid upon receipt of an invoice. NO PERSONAL CHECKS WILL BE ACCEPTED!!

3. Submit request to substitute names on registration forms in writing to the address above. Substitutions on registrations (including purchase orders) cannot be accepted after February 19, 2018.

4. Requests for cancellations and fee refunds must be submitted in writing to the address above by February 19, 2018. No cancellations/refunds will be granted after that date. Refunds will be processed six weeks after the conference, and a $100 cancellation fee will apply.

5. Your completed registration must be postmarked by February 16, 2018. Registration postmarked after February 19, 2018 will be processed at the on-site registration rate.

6. All lodging reservations need to be made directly with the motels themselves. Here is a list of motels within walking distance of the conference. Please ask for the government rate:
