Ian Needham – Latrobe Health Assembly

Ian Needham provided the following update on Latrobe Health Assembly and the Latrobe Health Authority: The Latrobe Health Assembly have met 4 times and currently has 41 members (26 community

representatives, 9 direct agency invites, 6 foundation representatives). The board meets on a monthly basis and will soon be incorporated Currently in the process of setting up working groups within the assembly The 4 key areas for focus at the moment have been identified as:

o Education, Employment, Wellbeing and Pride of placeo Sports, Exercise and Nutritiono Early Childhood and Familieso Chronic Illness and Wellness

Ian has officially been appointed as the Executive Officer of Latrobe Health Assembly, commencing next Monday

The roles of Administration Officer, Project Coordinator and Engagement and Communication Coordination will be advertised shortly

Looking at moving to a stand-alone office in Morwell as the Latrobe Health Assembly are currently sharing

the Berry Street building A Latrobe data and activity mapping register will take place to find and collate all relevant, current data that

is available around Latrobe City including current activity mapping The Latrobe Health Assembly will look at how to add to the work that is already being done. If you have any

The care book was discussed at the last meeting with positive reviews The original care book was developed in Rural areas for rural communities This is a great resource for both consumers, carers and healthcare providers to have autonomy and control Localising the book for Gippsland was discussed and members agreed this would be a good project

year We are looking to expand into more GP clinics, including Baw Baw clinics The S2S usage report indicates a decrease of 7.75% as aged care referrals are now going through My

eCare planning project This is not being expanded at this stage due to the MAC transition although continually encourage people

Jeannette Douglas attended the last Complex care meeting and provided an update on Health Pathway which had launched with 40 services

Since the last meeting, 9 more services have been added – Bowel cancer screening, Urgent mental health referrals, Q fever, Carer stress, In home support for older persons, Access to allied psychological services (ATAPS), hospital in the home, Residential in reach and Hospital admission risk program

Community nursing pathway will go live shortly

The next Allied Health Symposium will be held on Friday 1st June 2018. Further details will be released at a later date.

Mental Health First Aid training – Monday 19th and Monday 26th June. This course is for people who work with farmers or socialise with farmers, that will teach participants how to identify signs and symptoms of mental health issues, as well as how to offer initial support to people that think may be feeling overwhelmed, depressed or anxious, or developing a mental illness.

to 25 years who may have emerging or developing psychosis

Diana Mueller (Dept of Education and Training) Lots of discussions around the NDIS implementation The respectful relationships project is partnering with schools. Inner and Outer Gippsland have both

completed their inductions

Richard Adams (WGHG) The Regional Development Coordinator position will be readvertised for the Sectorial Development team The HACC training program which previously was provided from Chisolm will be reactivated with another

Liz Meggetto (CWGPCP) Providing refresher S2S training Care planning training Trauma Informed Practice information session has been held with a further 2 planned Continuing to work with organisations to ensure they are become more health literate. Quality improvement

plans have been completed with LRH, LCHS, WGHG, Wellways, Baw Baw Shire Council and Vision Australia.
