Job Title: Generic Support Worker

Location: Various across Lincolnshire

Hours of work: Full Time Post 35 hours.

Responsible to: Wellbeing Operations Manager

Job Purpose/Role:

The Short Term Intervention service will be available for 7 days a week 52 weeks of the year from 9am until 6pm. The delivery of the short term generic support service will be available to an individual for a maximum average of 6 weeks.

Main tasks:

Receive referral and support plan from the Wellbeing Service Coordination Centre. Visit service user in their own home at appointed time as arranged by the Wellbeing

Coordination Centre. Provide Information and advice including signposting to existing services that meet the

service user’s needs. To assist in completing all relevant paperwork and application forms for benefits and grants. Maintain accurate electronic service user case reports for each service user. Keep up to date with new legislations and practices in the relevant fields of work e.g through

attendance at training courses and staff meetings. Work with relevant local agencies in developing both the service and supporting service

users. To work as part of a team of case workers. Adhere to all policies and procedures of the organisation at all times. Develop and maintain strong relationships with supporting organisations in your area. Ensure the service user has a clear understanding of the exit strategy and keep to time scales

set. Accompany service users to groups and social activities when specified on the support plan. Assist with the smooth running of the organisation by following administration procedures

and maintaining accurate manual and computer records on all allocated cases, ensuring that files are up to date and easily understandable by other members of staff or outside agencies.

Carry out all other duties as deemed reasonably appropriate by your line manager.

Generic Support Worker

Characteristics Essential DesirableKnowledge & Skills Understanding and

appreciation of the needs of vulnerable individuals.

Awareness of the function of adult social care within the County of Lincolnshire.

Good knowledge of welfare benefits system and/or grants and funding routes for vulnerable people.

A knowledge of the work of statutory and voluntary groups in your area that support vulnerable people.

Knowledge of IT systems, particularly with Word Processing, Spread Sheets, Data Bases and use of systems to monitor and manage progress.

Effective time management.

Education, Qualifications & Specialist Training

Good standard of general education.

Qualification to NVQ 2 (or equivalent) in Health & Social Care or equivalent health care qualification.

Evidence of ongoing training and development.

Experience Experience in following detailed assessment plan.Providing one to one support to vulnerable people.Efficient completion of relevant claim forms.Maintaining accurate records.Good organiser and time keeperWilling to accept instruction and responsibility.Self-motivated.Flexible, cooperative, approach to work.Commitment to providing a client focused service.Understanding the importance of and commitment to confidentiality.

Personal Characteristics Positive attitude and good sense of humour.
