The annual visit by specialist doctors & other medical-support staff (from England) to BKL Walawalkar Hospital at Dervan has now become a regular feature in the Hospital’s calendar of events. It is a result of the high quality & dedication in the work being carried out at the hospital, that these renowned specialist doctors & medical-support staff commit themselves to return every year and offer their services to the patients without any expectation of remuneration. This sustained effort has led us on to the path of realizing a part of the vision of Shree Sahajanand Saraswati Swami Samarth Maharaj, that of “Health for All”.

Walking the path with DivineIt gives me great pleasure to place this copy of “Derwan Newsletter” in your Hands. This year, is a special year for our hospital, as the U.K. Medical team which visits our hospital every year and offers their medical services voluntarily for the patients over here, completes 10 years of their mission. In February 2014, the hospital also completes 19 years of service and enters its 20th year. A small hospital with 60 Beds has grown into a multispecialty hospital with 300 Beds and would add 200 more beds very shortly.Although the Hospital has thus expanded all-round and comprehensively, yet the noble principles cherished by Parampujya Digambardas Maharaj while establishing this Trust, and the ethics and values for which, ShreeKaka Maharaj has created this Hospital and brought it to this level by his continuous untired and relentless efforts in the past 19 years, are still being taken care of and protected with the same vigour, warmth and affection while running this Hospital day after day. Our main objective in running this Hospital is our firm belief that cordial and wholehearted service to virtually helpless patients, is the real and true worship to God and our this sincere belief has only prompted us to supply all services in this well equipped ultramodern Hospital at very subsidized and moderate rates. We provide radiotherapy treatment to poor cancer patients absolutely free. The Hospital conducts many social projects and activities with dedication, enthusiasm and honesty. The Hospital vans go to various villages every day for medical check-up of people of those villages. The Hospital also conducts regular camps at various places for dental diagnosis of children. The teenage girls are given proper medical advices, similarly the pregnant ladies and small children are provided with nutritious food free of any cost. The Hospital also undertakes programmes like REACH and MAHER which are useful to the society at large.The purpose of establishing this hospital is not only to treat the patients who come to Walawalkar Hospital, but also to go to the doorsteps of the rural people for providing them health facilities whatever best we can, and for making them health conscious through education. Our journey over the last two decades has been in this direction and will continue to be so.When we look behind through the advancement of the trust activities over the span of last 25 years, we realize that the blessings of our Divine ShreeDigambardas Maharaj are throughout behind us, and we are fully confident that they will remain so as long as we continue to walk on the path shown by him.

A progressive society addresses its need for betterment in different ways. In such a society, one would find institutions for improving the quality of life of the under-privileged through social services in remote areas of Konkan and also institution to make its citizen a better human beings.
However very rarely one would find these objectives being achieved through the same institutions, the reason being their lack of connect as commonly perceived.
However, at Dervan, ShreeVithalrao Joshi Charities Trust have been successfully running programms for last 40 years to achieve these objectives of the betterment of the life for the under-privileged people through social services and imparting humanitarian values to the people. This success has been achieved based on the thought espoused by ShreeVithalrao Joshi, the founder of the trust. He has expressed his views clearly in an article in Marathi titled “For the benefit of the mankind”. He says humanitarian values need to be imbibed by everybody in the society, if society is to achieve peace and prosperity. He further elaborates that since the government or ruling power usually won’t be in a position to impart humanitarian values to the large population, the same has to be done by a dedicated social organization. However with a natural human tendency for material advancement any insistence on the imparting of humanitarian values won’t find resonance till the recipients are well placed in their daily life. Hence, an organization is needed which
would address these interdependent objectives with the same dexterity. Exactly such organizations were built by ShreeVithalrao Joshi. ShreeVithalrao Joshi Charities Trust has been active in the Dervan area for the last 40 years. Their work so far has been related to running programs for under-privileged on the following points…

India is a developing country widely spread and with diversity of climate, culture, beliefs and languages. 70 percent of its population lives in rural areas and 25 percent live below the poverty line...
Foresight, vision, sincere and honest implementation programmes of NGOs have been altering the condition of their territory. Remarkable one such hospital is BKL Walawalkar Hospital is situated on the Mumbai - Goa highway along the West coast of India, in a village called Dervan, about 282 Km from Mumbai.
The region is not only financially backward but dominated by age old beliefs, traditions, rituals, and poor education levels, no faith in power of medicines and ignorance or neglect of hygiene. BKL Walawalkar Hospital is established in early 1996 with the objective of improving the Healthcare Service, providing good medical facilities at all times and affordable to all and achieving social upliftment.
With the advent of the BKL Walawalkar Hospital on the scene, a new hope was offered to people who were falling prey to snake-bites, scorpion-bites, accidents, infectious diseases, Tuberculosis, complications related to child-birth process etc.
It is a multispecialty hospital with state of the art machinery under one roof. All departments have state-of-the-art-machinery, Electric generator set, Computerized system and Internet facility. This infrastructure makes the hospital at par with any modern urban hospital. Underprivileged are given substantial concession in their hospital bills.
Since the year 2003, the hospital is associated with Tata Memorial Centre - Rural Outreach Project. It has screened the population of Ratnagiri and Sindhudurg districts for oral, cervical and breast Cancer. House-hold survey and screening was done to create awareness regarding Cancer and its early detection and prevention. Cases where Cancer was detected were bought to the hospital and treated free of cost under this project. Top of the line radiation facilities like Cobalt 60, Brachytherapy, high
density radiation, CT simulator for planning of radiation are provided free of cost to patients undergoing treatment at hospital.
Thousands of lives were saved due to early detection and free Oncosurgery, Radiotherapy and Chemotherapy. Thus the hospital has been transforming the life of the underprivileged of the region and nearing the ultimate objectives “Health for all, Education for all and Self-respect for all”. People in the region have apathy towards health; it leads to always patients being admitted to the hospital in critical condition. Considering this apathy towards modern medicine and lack of awareness about disease, hospital started various community based Awareness Programmes.
Walawalkar Hospital devoted itself for this cause. It started giving eye openers on improving health/hygiene and overall basic health practices encountered in life. A key part of the efforts has been the programs focusing on women. This is because the concept of mother of future generations was lost somewhere in the society. Family feuds, thrashings at the hands of husband, delivering malnourished children one after the other while chasing the crazy idea of having a “son” for the family and finally doing all what it takes everyday to look after the welfare of these children; all this reduced the status of the women in their own household. Recognizing the key role played by women in the stability & prosperity of the society, the hospital decided to support women right from their birth to maturity, i.e. till becoming a mother. The hospital took this vision forward by introducing programs empowering the women in the area e.g. starting of self-help-groups (SHGs) for women; even then some areas of possible intervention remain.
The preventive programmes are made exhaustive to cover all aspects of health for women in the society. As a woman is the driving force of society and if she is healthy and educated then she will give birth to a healthy future generation.

Towards The Health Revolution

A Life cycle approach to these programmes start from new born children, adolescent girls, pregnant ladies, and mothers. All are targeted through specifically designed programmes. Through these programmes, the BKL Walawalkar Hospital ends up in providing the women a relevant education & and good health at various stages of her life.
The hospital has devised and started courses in Nursing for the girls after secondary and higher secondary education. They are like ANM/GNM/BSc. & MSc.. The qualified girls from the institute have reached to remote places where the medical service is scarce.
An important stage in the life cycle approach to woman’s health programme is pregnancy. Being a rural area, lack of knowledge and indifference towards modern medicines makes many a pregnant lady prefer to deliver at home and finally land up in to complications and increase in infant mortality rate.
To overcome this challenge and develop an emotional bond between modern medical facilities at Hospital and the pregnant lady, hospital arranges a baby shower Ceremony every month. This ceremony is attached to the sentiments of rural women. The participant expecting mothers also get bonus of free antenatal (before delivery) check up, ambulance services to the hospital and delivery facilities free of cost.
Children are the real human resource of the nation. But malnutrition among the children of is widespread and needs to be dealt with on priority. It is observed that when they are being breast-fed they are healthy but when the weaning starts, these working mothers go to work on fields leaving their children alone at home. With nobody at home to pay attention to kids, they are not fed properly and end up being malnourished.
Keeping this fact in mind hospital decided to keep a track of all children from this vulnerable age group that is the age of 6 months to 12 months. This was indeed a challenge as the parents were ignorant to pay attention to the child health. Thus an idea was floated to celebrate the birthday of these children. The part of immunization is also introduced at birthday celebration.
Walawalkar Hospital has surveyed thousands of malnourished children and it was found that 80% of them have Dental “carries”. These “Carries teeth” makes the teeth non- functional, cause pain and affect chewing. So such children prefer to eat less and ultimately suffer from Malnutrition. So obviously Hospital with its holistic
approach decided to alleviate the sufferings. Hospital launched a Dental Check-up and Treatment Project at School level for the children of this region with special emphasis on oral health education.
A mobile team from the Hospital visits schools and children are examined by Dentist, while being given demonstration about how to maintain their Dental Hygiene, also to encourage good Dental hygiene a child with “no carries” is given a prize. Regular dental check-ups at their schools ensure oral health and this would also reduce tobacco habits as hygienic practices about dental/oral care would be taught to them. This would help in achieving the dream of oral cancer free region.
Children with severe “carried teeth” and those who cannot eat properly because of “carries” are brought to the hospital and treated for the same. So far thousands of children have been treated free of cost so that may be after 15-20 years this land will be free of Cancer and Malnutrition.
Doctors from Newcastle UK noticed gigantic and honest health care work done by the hospital for the ignorant and financially backward people, A team of more than 30 medicos and paramedics from Newcastle visits Walawalkar Hospital for 1 week. Impressed by the infrastructure, cleanliness and expertise available here these doctors have shown interest in volunteering for work at Dervan. Then onwards for the last 10 years this team pays a visit stay for a week and render their special service.
The efforts & results at Dervan are a glowing example of what a transparent organization with vision of a healthy nation and spiritual base coupled with a genuine desire to deliver most modern health services at doorsteps of deprived can do. Such an endeavor cannot stay ignored for a long time and therefore it has attracted towards it, several people from different walks of life and professions, all working towards transforming lives of the people.
There is a verse which says that ”Those who are denied of even basic needs like food, clothes and shelter cannot think of health”. The Hospital recognized this. Hence for the holistic betterment of the people the SVJC trust developed these social initiatives rather than running hotels, motels and tourist attractions. After all this, as we look back, we realize that we are on our way of success as far as health goals are concerned.
We are confident that the region of Dervan will soon see a major health revolution being brought about by the dedicated team of Walawalkar Hospital and constant support from our doctor friends in UK.

malignancy at East Surrey Hospital, U.K informed the delegates on “Medical and Surgical Management of BPH”. Dr. Abhay Desai presented a short, crisp and entertaining talk on “Surgeon’s Hand”. Dr. Kalpana Patil, Paediatric Urologist, at Evelina Children’s Hospital provided on overview on “Undescended Testis”. Dr. Sudhanshu Chitale, Urological surgeon, London described well “Urological Emergencies”. The session concluded with an entertaining lecture on “Surgical Management of Diabetic Foot” by Dr. Pramod Bapat, Professor of General Surgery.
The conference agenda blended researchers, practitioners, and advocates giving the audience a broad overview of the theme of CME from a wide range of perspectives.
With the help of 19th international C.M.E. at B.K.L. Walawalkar Hospital & Research Centre succeded in its efforts to form a strategic partnership with the general practitioners and specialist doctors of rural area of Konkan so as to provide international quality health care to maximum number of rural people and thus ultimately helping them to lead a productive and fruitful life.
The programme includes presentations, panel discussions and interactive sessions in the CME and the same was highly appreciated by the audience. All the delegated were actively participated with the resource speakers and exchanged their themes and innovative ideas.
The first day of the 10th annual U.K. Camp witnessed the grand opening ceremony of the camp and inauguration of 19th Maharashtra Medical Council accredited C.M.E. titled “Medicine Today & Tomorrow”, on 25th of January 2015 at the B. K. L. Walawalkar Rural Medical College Lecture Hall, Dervan.
The dignitaries present to grace the function were ShreeVikas Walawalkar - Managing Trustee, ShreeVithalrao Joshi Charities Trust, Dr. Sudhir Joshi, Dr. Suvarna Patil - Medical Director, Dr. Shripad Banavali, Dr. Sunil Nadkarni, Dr. Sanjay Deshpande, Dr. Lance Cope , Professor Stephen Attwood, David Wales and all the members of the U.K. team.
The event brought together more than 100 professionals and key officials from wide range of medical fraternity. To ensure that the goals of the meeting were effectively met, the plenary session consisted of 8 speakers from India and Europe, each with expertise in their respective fields.

Laparoscopic Hernia RepairThough hernia repairs are routinely performed laparoscopically elsewhere, we at BKL Walawalkar Hospital were not very keen, as the usual repair required expensive consummables. Professor Attwood and Dr. Duncan Light demonstrated a relatively simple, yet inexpensive method of laparoscopic hernia repairs, using the conventional prolene hernia mesh and endocorporeal suturing with readily available Vircyl sutures.
The relative ease and excellent post operative outcome has really spurred me to perform more of such repairs in the future.

Reduction MammoplastyWe performed this surgery on a young girl, with gigantomastia( enlargement of both breasts) which had affected her so badly that she had dropped out of school, was socially very embarrassed and had backache due to the sheer weight. The surgery was tricky, as we had to ensure that not too much breast tissue was excised, as it would render her unable to breast feed and at the same time, leave enough of overlying skin to prevent necrosis.
The post surgery period was uneventful and she recovered well.
Post burns contracture releaseThough this surgery is performed routinely at every UK camp, it merits a special mention, because it is carried out not for cosmesis, but to improve function of movement.We performed quite a few contracture releases, which also included a young girl who has been attending the UK camp regularly and has undergone several similar surgeries for multiple burns contractures.
Cleft Lip and Palate surgeriesA majority of the plastic surgery work comprised cleft lip and palate repairs. A notable example was a girl who has undergone multiple stage surgeries over the last few years consecutively. Thankfully, she has emerged more beautiful with each passing procedure.

Community Outreach Activities
The social activities like Baby Shower ceremony, common birthday of children, antenatal mother and post natal mother function are close to the heart of villagers. A unique initiatives were taken by B. K. L. Walawalkar Hospital to start all these social, community outreach activities in the hospital. This helped in the advocacy, communication and social mobilization efforts of the institution in order to achieve its objective of community based social and health care services.
Activities
Rural Empowerment And Community Health (REACH)The Rural Empowerment and Community Health (REACH) project intend to make women economically and socially empowered by giving them opportunity to earn their livehood. The key part of the initiatives is to form self-Help-Groups( SHGs) which are given vocational training such as preparing processed food, handbags, jewelry etc. SHGs are micro credit group run by women at village level managing their finances for home based needs by selling these products. Economic empowerment usually is the first step towards social empowerment for these rural women.
The visit to Self Help Group at village Ambatkhol was arranged to see efforts made through REACH project. During visit women welcome U.K. team in traditional way and traditional dance. They showed them their home based business of rice mill and explained functioning of SHGs.

Common Birthdays and Postnatal Mothers WorkshopsThe important objective of this function is to ensure follow-up of all children born in Walawalkar Hospital till they reach 2 years of age. This birthday ceremony was arranged by Community health department in Walawalkar Hospital for children of 6 month to 2 years. 25 children along with their mothers were participated in ceremony. It was arranged with view to guide mothers regarding weaning, diet and monitoring growth and development of toddlers. U.K. Team members along with Dr. Ranee Limaye, Padiatrician interacted with mothers gave blessings to toddlers. All members of UK team enjoyed the function.
UK Medicos in Baby Showers Ceremony
Dohale Jevan (Baby Shower Ceremony)Dohal Jevan is an ancient Hindu tradition which is beneficial for inculcating good sanskar (Values) in the baby while it is in the mother’s womb. It also fulfills the good desires of mothers thus making them happy and healthy.
UK team participated in Baby Shower Ceremony conducted by community health department of B.K.L. Walawalkar Hospital. 30 expectant mothers (pregnant women) registered in hospital for delivery from different villages came for ceremony. Dr. Kalpana Patil and Dr. Varsha Deshpande, Dawn Orr, Caitlin Lambert, Williams Marc Harry, Dr. Ailsa Dunphy and Rebecca Vallins actively participated and guided them regarding pregnancy and ante-natal care. All of them have enjoyed traditional dances arranged specially for pregnant women.

This is my 4th trip to hospital however I have been involved with the project from year 2006. Each year we sending much needed equipment to the hospital and I will continue to be involved throughout the coming years.I am also involved with the UK Charity in the Chairman, raising funds to help get staff and equipments each year.
The early years I was involved with the developments of anesthesia staff with qualification to assist effectively in theatres. 2015 visit has proved that all the input has come to fruition this quality, commitment, enthusiasm of staff equals any of the committed staff in the UK. I am so proud to have been able to contribute towards their developments within the hospital.
Each year I visit and also get involved with Health Safety Role Management, stock control and storage. Whilst there is still a long way to go towards perfection just as in UK, the management under Mrs. Patil and Mr. Godbole we continually striving towards their goals.

This is my 8th annual visit to BKL Walawalkar Hospital. The hospital has completely transformed over this time, the expansion and modernization is fantastic particularly over the past year. The improvement is reflected in the overall health of the local population. The expansion in teaching, health promotion and a holistic approach to healthcare is extraordinary. The motivation, dedication, teamwork and common efforts of all the staff is inspirational themselves under the guidance of Shree Kaka Maharaj.

After ten years of my experience these could be the only words to express my gratitude towards magnificent work done in Dervan for the poor and needy people by Walawalkar Hospital. This was my second experience with the UK team and I must say, I continue to be impressed with their enthusiasm! This UK camp was the 10th one, but one could easily be fooled to think it was their first! The 30 plus strong team included many familiar faces, but this time, we were visited by Professor Stephen Attwood and Dr Duncan Light as part of the general surgery team for the first time.
We performed several surgeries laparoscopically and I managed to learn quite a few new techniques. I was particularly impressed by their easy demeanour, enthusiasm to teach, willingness to manage with the barest of resources and above all, a great amount of enthusiasm. They taught me a lot in terms of not only laparoscopic surgery, but also in terms of patient safety, protocol adherence, operative field and time optimization and so on.
I truly appreciate the time and efforts spent on me by Professor Attwood and Dr Duncan Light in training me in Laparoscopic hernia repairs, endosuturing as well as discussion on several potential research topics. I thank them for their patience, as well as Dr Sanjay Deshpande, Shelley Quantrill, Eleanor Freeman, Davy and others for organizing such a wonderful camp.

Every year U.K. team is regularly visiting the Walawalkar Hospital to serve the needy patients of Konkan region. My experience of working with U.K. team was colourful. U.K. team is visiting since last 10 years. I observed following points while working with them.
• All the team was very enthusiastic and co-operative. Very dedicated, punctual and hard working.

• All members were having the spirit of team work. This camp was great success and had multi-factual benefits.

MICU Sisters: We received very important information about Ventilators in Casualty, how to take care of serious patients and how to handle these patients. Also they taught us the different exercises of patients, the method of inspection of glassgo coma scale and information about E.C.G. in a very simple language.
This information is very important in our daily routine duties and certainly it will boost our confidence in any emergency treatment as well.
Sonali Kambale, Sister, Female Surgery Ward: Intimacy for other people is the best value one may find in U.K. doctors. The respect to any person in front of him is another important value that we must adopt from them. When all these U.K. doctors were on pre-operative ward rounds, when they heard about the poor financial condition of the patient Mrs. Asha Shelar whose Cystocopy surgery was planned, the doctor from pain management specialty immediately decided to pay for the charges. Dr. Suvarna Patil madam had already helped patient by giving concession through hospital and this help by unknown doctor made patient very gratified.
The working by communicating each other is very noticeable.
I personally feel, if we accept the good things from these people such as discipline, the value of time, respect for others, understanding and helping nature, surely it will help us in future life.
Staff Nurses: The staff nurses who were present at the time of surgeries like Hernia, Appendices, Cholecystectomy told that all those surgeries were carried out with Laparoscope only. It is very important responsibility to take care of this Laparoscope and they explained the method in a simple manner. They gave information about emergency medicine and the care to be taken at the time of trolley preparation in a ward. Also they practically taught us what care should be taken to keep the trolley disinfected.
Surely all these valuable information will be important for us in future during our nursing duties. This became possible because of UK Team only.

At the same time we kept on learning something new from those doctors and nurses. Throughout the camp they taught us many things related to patient care. There is a lot to learn from them. Half of the illness will decrease just by giving psychological care. They consider each and every patient present in the hospital and take update concerned to illness on daily basis. We could feel the importance of nursing-patient care. This was really great training for us in many aspects.

They take care of patients very well. Their way of speaking with us and patients is very polite. Their face keeps always smiling so that anyone who comes in front of them becomes very happy, patients too.
They also taught us the different lectures of communication, CPR, Vital signs, Waste material management. We all are used to enjoy their lectures and attend their lectures daily. While teaching to us they also enjoy the lectures with us.
At last on 31.01.2015 they had dressed our cultural costume (Saree), all were looking pretty in sarees. It was a precious day we all doctors, nursing staff, students were gathered for their sendoff party. It was memorable moment for us and the UK people.

Mrs. Supriya Chandramani Sawant:My name is Mrs. Supriya Chandramani Sawant, from Rampur, Tal – Chiplun, Dist – Ratnagiri. I am 38 years old, having husband and two children. I belong to very poor family. Two years back I was extensively burned while cooking in my house. I was treated at J. J. Hospital, Mumbai. But after the treatment Post-burn scars and scar contractures over the abdomen and both elbow remains same. Due to this I am unable to do my routine work at my home. Mr. Nandadeep Krishnaji Naik: I am Mr. Nandadeep Krishnaji Naik, 62years old from Kolvan, tal- Kankavali, Dist – Sindhudurg. I was having 5 large (5cm x 5cm) multiple swellings over scalp of head from my birth itself. The size was increasing gradually since one year. It was very difficult to survive in society because everybody was asking about the same and I was under mental stress. My grandfather was also having the same swellings over the head. I have taken the treatment at many places and requested to many doctors for surgical intervention but they were not ready to take risk.

Since last 5 - 6 months I was suffering from hip joint pain while walking. I took medicines at many places but did not have the relief. At the same time I fall and was bed ridden from last 4 months.
