Also check “yes” if you:

o smoke (or are around smokers at work or at home)

o weigh too much

o get less than 150 minutes of moderate exercise a week

o have family members who had a stroke or heart attack before age 55

o are a man over 45

o are a woman past the change of life

o are often tense, stressed, rushed or get angry quickly

o don’t know if you have high blood pressure or high cholesterol

o eat high-fat foods often (like red meats, bacon, sausage, butter, egg yolks, fried foods, ice cream, whole milk, cheese, chips, gravy, sauces)

If you checked 1 or 2 of these, you have some increased risk for heart disease. If you checked 3 or more, you most likely are at a much higher risk for heart disease.

Make plans now to improve your health before serious heart damage occurs.

Check the step(s) you will take:

o Stop smoking and stay away from smokers

o Prevent or control high blood pressure

o Eat less fat and cholesterol

o Keep your weight in a healthy range

o Exercise regularly to strengthen your heart

o Manage stress so it does not damage you

o Manage your blood sugar, if you have diabetes, to reduce buildup of fat and cholesterol in your arteries
