    Purpose This page is a site map type of page. It allows the user to access every page in this web site.

    Logo This logo, located in the upper right of the page just below the banner, will be replaced with W-2c Managerfor all pages within theManage Employee W-2c Forms section. The logo will not change on the Home page.

    The horizontal tabs align with the W-2c Managerlogo.

    Create Form

    Purpose Allows the user to look up the latest W-2 or W-2c forms from which to create a W-2c for the employee.

    Note: Working items will not be displayed.

    Tabs Create Form selected.

    Mass Change, Edit Forms, All Forms, Print, Amendment Files, History unselected.

    Embedded Text Select a tax year and enter a Social Security number, and then click Find to display a form (W-2 or W-2c) for which youwant to create a W-2c. To refine your search, enter a FEIN and/or a name.

    (Last Name) Embedded help text under each text field for Name:

    (First Name MI)

    Error Messages You must enter an authentic US Social Security Numberin the format ######### or ###-##-####.

    Social Security number was entered in an incorrect format(upon clicking Find).

    You must enter an authentic Federal EmployerIdentification number in the format ######### or ##-#######.

    There is no available FACS data for the information youhave entered. To create a blank W-2c for this employee,you must verify the information and try again.

    Create a Blank W-2c (Additional FACS Info Step 2)

    Purpose Allows the user to choose the FACS information needed to create a blank W-2c.

    Note: The page is not available when only one set of FACS information exists.

    Page Title Create a Blank W-2c

    Embedded Text Multiple sets of FACS company information exist for your entries on the previous page. Click to select the appropriate setand then click Next.

    Error Messages You must make a selection and then click Next. A radio button was not selected (upon clicking Next.)

    Working Table The FACS Information table displays the groupings of FACS information available based on the information entered onthe Create a Blank W-2c page.

    The FACS Information table displays the following column headings:

    No heading supports radio buttons center position

    FACS Code left position

    FACS Branch left position

    No columns are sortable.

    The position of the table shall be flush left.

    The table shall scroll within the page when it is larger than the viewable area. The column headings and the page itself willremain static.

    Navigation Goes back to the Create a Blank W-2c entry page.

    Saves the information entered, and generates a blank W-2c based on the data as storedwith PSS.

    Does not save any information, and returns to the Create a Form tab.

    Purpose Allows the user to create and/or edit a W-2c form for an employee.
