The following information is required:

Last 4 of SSN OR Full SSN Full first and last

name Date of birth

Setting up user ID and Password Complete the

“Required Fields” section.

Remember to write down your user id and password.

The second part is optional.

Once you have completed and select “Submit,” you will be logged in.

A VSP benefits page will appear.

Savings Statement and Previous Visit

If services are performed by a VSP network doctor, you will be able to view and print your “VSP Savings Statement,” by clicking the “Previous Visit & Savings” tab.
