Welcome to OSSTEM Meeting 2012 Lisbon Chairman

Dear colleagues and friends,

It’s an immense pleasure to invite all of you to the 2nd OSSTEM European Meeting 2012 which will take place in my home country Portugal.

It is a great honor for me to be entrusted to chair this conference providing a multifaceted program. Covering various aspects of dental treatment techniques, planning or complication management, the OSSTEM Lisbon Meeting 2012 offers a lot of opportunities to benefit from.With the purpose to introduce cutting edge knowledge and the latest developments by international experts, the conference aims to be an excellent platform for clinicians and all those interested in these fields. All lectures will be held by well recognized specialists in prosthodontics, periodontology, oral surgery as well as in regenerative surgery. The design of the two day conference furthermore offers a live surgery and various workshops with hands-on practice.

On this occasion, I would like to express my heartfelt happiness to meet colleagues from the global dentistry community at the city of Lisbon. This magnificent place, full of history, culture and incredible modern architecture will make it worth your while. But these are only a few of many reasons to come. The history of Convento do Beato dates back to the 15th century, when Queen D. Isabel was given authorisation to construct a hospice in the chapel of S. Bento for the congregation of the “frades azuis", the "b|ue monks" as they were known. Before she was able to fulfil her wish, Queen D. lsabel died, leaving 8000 gold crowns in her will for building the hospice.In the 16th century, Frei Antonio da Conceicao set the construction of the monastery in motion. The stony goes that with just seven "tostoes" (small coins like farthings) which he had received in alms, he managed to get the building of the magnificent monastery underway. The reputation of this mi-raculous building increased and in 1602 the people made him into a saint (he was only recognised as such by the church in the 18th century), and they began to refer to him as Beato Antonio (Antonio the Beatified) and his magnificent creation as the Convento do Beato.

Topic:“25-years experience from Dental Implant treatment; when should we prefer and when should we avoid implant treatment?“

Abstract:My implant background: My team started with dental implants 1982. I joined the team 1987. I will present a timeline that describes what kind of cases we treated during those 25-30 years of practice. We will then look at four to five different clinical cases. The first we meet when the patient is 15 years old. We will follow her treatment till the age of 42 (last year). The implants we will study have been in function for about 22-23 years. The next case is a rather fresh case where we for different reasons recommended toothsupported solution but the referring dentist didn‘t care and proceeded with an implant supported solution. The result was a total disaster. We will learn what we saw in advance and what to do to avoid such disasters. The third case is a girl with an extremely rare dental disease that made her almost totally edentolous in the age of 18. The story how she was treated and how she looks today is very interesting (she won a beauty contest!) The fourth case is a female professional basketballplayer that lacks two front teeth and how we discuss different alternatives in her treatment. What is the general learning from these cases: If the patient suffers from severe dental diseases you can use „heavy tools“ and do risky treatment with little risk for total failures. But on the other hand, the patient that „suffers“ from relatively small dental problems (that could be solved in many alter-native ways) you have to be much more careful. The risk of failure could (controversy to what many dentists think) be much higher in these so called „easy cases“

Topic:“Easy and safe crestal approach sinus bone graft using the CAS-KIT“

Abstract:Sinus bone graft can be performed using either the lateral window approach or the crestal approach. Crestal approach technique is preferred for general dentists because of its relatively easy surgical skills and less complications. The key to success in crestal approach sinus bone graft technique can be summarized as follows;1. Sinus floor fracture without membrane perforation2. Separate sinus membrane widely for extended surface contact of bone graft material Although conventional Osteotome sinus lift technique was a popular technique and relatively easy to perform, it has some limitations in achieving these goals. Many specially designed instruments, which make it easy to perform sinus lift procedures, have been introduced. Basing on unique ideas and concept of design, most of them show better performance in sinus lift procedures. Recently, Osstem (HiOssen) introduced the CAS kit (Crestal Approach Sinus kit). It has many advan-tages in performing crestal approach sinus bone graft. The specially designed CAS drill, and the Stopper system in the CAS kit make it easy and fast to fracture the sinus floor without the membrane tearing during the sinus lift procedure. Hydraulic lift system in the CAS kit makes it easy and safe to detach the sinus membrane widely and adequately as we want.I want to present the characteristics and advantages of the CAS kit with clinical cases.

Topic:“Treatment planning for implant fitting in aesthetic areas using a new 3D navigation system“

Abstract:Digital dentistry is becoming a new powerful instrument to improve diagnosis and treatment outcomes.Navigations systems allow the dentist to interact with the radiologist in evaluating, studying and treatment planning cases.Knowing in advance the anatomy of the patient greatly reduces the chances of making surgical mis-takes and allows the operators to custom plan what before was, most of the times, done by chance. New navigation systems translate the DICOM data into 3D models that can be worked out with the use of computers.Surgeons and prosthodontists can plan their final restoration before the day of surgery and the accu-racy of such treatments is becoming more reliable.The addition of these technologies makes surgery simpler, most of times flapless and the postopera-tive time a pleasant one for the patients.Beside allowing patients to find their ‚fixed‘ teeth in the same day, navigators are becoming a power-ful instruments to improve esthetics, if the Periodontist or the surgeon have created an ideal surgical situation, the fact of using a controlled flapless approach reduces surgical time, trauma and is the most respectful surgery for the soft tissue. Therefore digital surgery is becoming quite important when esthetic is in demand. The presentation will highlight the impact of such procedures in maintai-ning and developing an ‚invisible‘ esthetic restoration.

Abstract:Placement of endosseous implants in the atrophic jaw is often restricted because of a lack of sup-porting alveolar bone. To overcome this anatomic limitation, sinus lifting, bone block transplantation and GBR has become a common surgical procedure in oral implant treatment. This presentation will highlight through in depth clinical case documentation, the new and evolving role of instrumentation for access to the Sinus for Bone Augmentation and a new method for the GBR. Since the initial ap-plication of sinus lift augmentation, GBR and implant placement , many new approaches have been develloped, articles have been published describing predictable techniques with reliable long-term results. A knowledge of useful and clinically manageable techniques that provide access but minimize large surgical flaps are keys to success. We will see how to achieve successful results by creating bone in different regions of the jaw with osseointegration maintaining occlusal function under load.

Abstract:Dental implants are established alternatives for replacing missing teeth. In case of alveolar bone resorption, implant placement may be prevented unless the volume of hard tissues is increased before or during implantation.Autologous bone graft is still regarded as the “gold standard” in alveolar recostruction for lateral and vertical bone augmentation. Intraoral sites generally allow for shorter procedures and are associated with few complications and less postoperative discomfort; it’s also important to evaluate the biologic qualities of this trans-planted bone, due to the embryologic origin, morphology, cytological constituents and biochemical composition of the extracellular matrix.Aim of this presentation is to analyze the different techniques as Guided Bone Regeneration, onlay bone graft and sinus lift with lateral and crestal (Cas-Kit) approach to perform an esthetic and func-tional implant restoration of the posterior edentolous jaws.

Abstract:Predictable soft and hard tissue regeneration, so crucial to the clinical success of each case is highly dependent on the appropriate selection of material as well as the technique and approach. With ab-undance of newer materials on the market, the clinician is given the task to marry the right material and technique, often without much scientific guidance. Replacing a missing tooth by surgical placement of an endosseous implant, and immaculate restorati-on of the lost tooth structure seems to a predictable and mundane routine dental procedure. As the general population enjoys more and more dental implant restorations, failing and ailing implants are readily being encountered on a daily basis. Can we realistically promise our patients that the implant will have a lifespan similar or better to their life? Does “implant for life” have true evidence, or is it a simple marketing catch-phrase on the part of dentists and implant manufacturers?This presentation will review the existing evidence on the science of osseointegration, and clinical studies that validate long-term health of dental implant restorations. What factors, if any, do play a role in the breakdown of dental implant support? What should the home care, and professional main-tenance protocol be? Do the materials, techniques, and implant systems that a clinician chooses have any bearing on these factors? The presenter will highlight various innovations in soft and hard tissue handling techniques in recent years. Dr. Kim will also review the essence of implant maintenance pro-tocols, diagnosis and recognition of peri-implant diseases, together with a systemic review as to how peri-implant break down can be prevented.

Topic:“A Novel approach of sinus membrane perforation and oroantral perforation and its management“

Abstract:Maxillary sinus floor elevation with autogenous or synthetic grafting material has been proved to be a reliable treatment method that enables the insertion of endosseous implants in patients with a severely resorbed maxilla.The lateral window technique involves a top hingedoor in the lateral maxil-lary sinus wall. This technique has been shown to be highly successful and predictable with 10 years more longevity. Despite its predictability and the high success rates achieved with this augmentation technique, complications do occur. Perforation of the maxillary sinus membrane is the most common complication that occurs during sinus floor elevation procedures. And furthermore this schneideri-an membrane perforation during sinus surgery is highly related with postoperative infection, vital bone formation failure at all. Because of difficulty of management appropriately during or after sinus augmentation,the graft material may be displaced into the sinus, leading to infection, which may cause maxillary sinusitis and threaten implant survival. And also, oroantral perforation after tooth extraction jeopardizes implant installation situation. So I want to introduce a novel management of sinus membrane perforation according to a perforation dimension and suggest a simultaneous or stage approach for making a bone in fistula defect area from my 13 years clinical results using Osstem implants.

Abstract:Immediate placement of dental implants after teeth extraction is one technique more common today with applicability in partial and total edentulous cases.Successful surgical and prosthetic rehabilitation of patients with severe bone resorption in the maxil-la is often compromised by the inability to obtain sufficient anchorage for the prosthesis; maxillary tuberosity and zygomatic bone are considerable anatomic structures. The author will present several surgical protocols with zygomatic implants using intra, extra or combined maxillary sinus techniques to approach these clinical cases. The radiological information available justify the choice of this ana-tomic structures combined with the possibility of 3D software bone reconstruction, surgical guides and stereolithography models.In Portugal, 30,1% of the elderly population are total edentulous, the purpose of this presentati-on is to present fixed rehabilitation alternatives for severe resorbed maxilla using prosthetic infra-structures in titanium or ceramic combined with final teeth in acrylic or ceramic. The final part of the presentation will show the rehabilitation of cancer cases and the development of new protocols, new materials and new approach methods.
