The following are applications for extra-provincial and public vehicle operating licenses filed under the Motor Vehicle Transport Act, 1987, and the Public Vehicles Act. On December 30, 2010, amendments to the following rules and forms came into force under the Securities Act:

• National Instrument 51-102 Standards of Disclosure for Oil and Gas Activities (NI 51-101);

• Form 51-101F1 Statement of Reserves Data and Other Oil and Gas Information;

• Form 51-101F2 Report on Reserves Data by Independent Qualified Reserves Evaluator or Auditor;

• Form51-101F3Report of Management and Directors on Oil and Gas Disclosure; and

• NationalInstrument41-101General Prospectus Requirements, including its Form 41-101F1 Information Required in a Prospectus.

Additionally, a new form, Form 51-101F4 Notice of Filing of 51-101F1 was created.

NI 51-101 sets out the annual filing requirements for reporting issuers who are involved in oil and gas activities, notably in respect of their estimates of reserves and resources. In addition, NI 51-101 sets out the general disclosure standards for reporting issuers who are reporting on their oil and gas activities. The disclosure standards apply to any disclosure made by a reporting issuer throughout the year.

The amendments fell into the following broad categories: amendments for clarification, amendments to codify existing staff guidance and practice, and added requirements to enhance reliability of certain disclosure of reserves and resources other than reserves.

Personal Health information Protection act, 2004

The Minister of Health and Long-Term Care on behalf of the Government of Ontario invites public comments on a regulation proposed to be made under the Personal Health Information Protection Act, 2004 (“PHIPA”).

PHIPA came into force on November 1, 2004. Ontario Regulation 329/04, “General,” made under PHIPA also came into force on November 1, 2004. The Government is proposing a regulation to amend Regulation 329/04. The Act requires that the Minister publish a notice of the proposed regulation and allow 60 days for public comment, after which the Minister reports to the Lieutenant Governor in Council, who may then finalize the regulation with or without changes.

Content of Proposed Regulation

The proposed regulation would make the following amendments to the existing regulation under PHIPA, Regulation 329/04:

Amend paragraph 6 of s. 13(1) of O.Reg. 329/04 to prescribe Cancer Care Ontario in respect of the Ontario Cancer Screening Registry, instead of the •Colorectal Cancer Screening Registry, to encompass all of Cancer Care Ontario’s cancer screening programs.Amend paragraph 7 of s. 13(1) of O.Reg. 329/04 to replace the former name of the prescribed registry that is being operated by the Children’s Hospital •of Eastern Ontario with the new name: Better Outcomes Registry and Network.

La LPRPS et son règlement d’application 329/04, intitulé Dispositions générales, sont entrés en vigueur le 1er novembre 2004. Le projet de règlement du gouvernement modifierait le Règlement de l’Ontario 329/04. La loi oblige le ministre à publier un avis de projet de règlement et à prévoir une période de consultation publique de 60 jours, après quoi il doit faire rapport au lieutenant-général en conseil, qui peut alors sanctionner le règlement avec ou sans modifications.

Contenu du projet de règlement

Le projet de règlement prévoit de modifier comme suit l’actuel Règlement de l’Ontario 329/04 pris en application de la LPRPS :

Modifier la disposition 6 du paragraphe 13(1) du Règlement de l’Ontario 329/04 afin de prescrire que tous les programmes de dépistage du cancer •d’Action Cancer Ontario doivent être regroupés au sein du Registre ontarien de dépistage du cancer, plutôt que du Registre de dépistage du cancer colorectal.Modifier la disposition 7 du paragraphe 13(1) du Règlement de l’Ontario 329/04 afin de remplacer l’ancien nom du registre prescrit qui est exploité par •le Centre hospitalier pour enfants de l’est de l’Ontario par le nouveau nom : Registre et réseau des meilleurs résultats.

1. Regulation 1012 of the Revised Regulations of Ontario, 1990 is amended by adding the following section:

1.0.1 For the purposes of the definition of “qualifying trust” in subsection 1 (1) of the Act, a trust is a qualifying trust if it qualifies as an employee life and health trust under subsection 144.1 (2) of the Income Tax Act (Canada), if the portion before paragraph (a) of that subsection were read without reference to the words “throughout the taxation year”.

2. This Regulation is deemed to have come into force on December 1, 2010.

1. Regulation 737 of the Revised Regulations of Ontario, 1990 is amended by adding the following section:

KEEMAN

21.1 (1) A Local Services Board is established under the name “The KeeMan Local Services Board”.(2) The boundaries of the Board area are those described in the Schedule. (3) The Board shall be composed of five members.(4) The Board may exercise the powers set out in paragraph 2 of the Schedule to the Act.

SCHEDULE

In the geographic Townships of Gundy, Broderick, Gidley, Pellatt, Ewat, Forgie, Boys and Glass, and in unsurveyed territory in the Territorial District of Kenora, being composed of those parts of the said geographic townships and unsurveyed territory designated as Part 1 on a plan known as Plan of the Local Services Board of KeeMan filed on April 21, 2010 with the Office of the Surveyor General of Ontario in the Ministry of Natural Resources.

2. This Regulation comes into force on the day it is filed.

The Ontario Gazette is published every Saturday. Advertisements/notices must be received no later than 3 pm on Thursday, 9 days before publication of the issue in which they should appear. For weeks including Easter Monday, November 11th or a statutory holiday allow an extra day. 2) For a first insertion electronically submitted the basic rate is $75 up to ¼ page.

3) For subsequent insertions of the same notice ordered at the same time the rate is $40 each.
