Request for Capstone Project - Form Completion Guidelines

Item Description1. Project title: Enter the name of the proposed project. The name should convey the purpose.

It is strongly suggested that it contains an action word such as improve, maximize, minimize, develop, implement, reduce, etc.

2. Start date: The start date should be the date that the student is going to start with the trainings and/or the first date of the student works at project site.

3. Target completion date:

It should be close to the last date of the academic semester. The final presentation is expected to occur after the target completion date.

4. Project description:

Details the objectives that the organization wants to attain, includes data on current status. A general description of the magnitude of the project, the reasons why the organization wants to undertake the project and the general approach the organization suggests should be employed. The problem description should not be written as a solution, since the solution is going to be found by the students through this project.

5. Project scope: The project “is” should be the listing and descriptions of the boundaries of the project. For example, the processes, materials, products, information, etc. The project “is not” are the processes, materials, products and/or information that immediately precede or follow the ones included in the project.

6. Project objectives and goals:

List and describe project objectives and goals, and provide the metrics and targets that are expected in order to claim the project was successful.The goals and objectives need to be SMART (Specific, Measurable, Achievable, Relevant and Time-Based).

7. Strategic Business Goals Impacted:

Describe how the project impact business goals, vision or mission. It is expected that the project will assign resources if the project is aligned with business strategic plans.

8. Key Risks and/or Dependencies:

List anticipated risk or dependencies that could hamper the progress of the project.

9. Key Resources and Team Members:

List names and titles of key organization personnel that will be supporting the students’ activities.

10. Estimated hours per week:

If the project were assigned to a new-hire and it is expected to be completed in 16 weeks, how many hours per week would you expect he/she will dedicate to the project? This time will not be later divided into the number of team members since they will be working as a team.

11. Check requests that apply:

If there are special requests for the students to comply please check the appropriate box. Requests such as doping, vaccines, tests, etc. should be specified here. A cost estimate of tests and/or vaccines should also be provided here if is expected for the student to cover the cost.
