Chair's Welcome

Have you read Simon Steven's Five Year Forward View for the NHS? It’s a concise, well written and quite inspiring read for all who love the NHS, as it gives some hope that it just might survive if we can engage communities in prevention, get even more efficient, and get £8 billion out of the treasury! I hope it puts pressure on the politicians to respond in their election manifestos

C&H is getting some recognition for being an efficient health economy because of our clinical pathways and joint work, and we are doing more work on the data to support what it is we do, that means we are in the almost unique position of being able to invest in services and have our main providers in balance and hitting their quality markers most of the time. It does make C&H a much better place to be a GP than most.

The clinical senate with the Homerton was enjoyable, and had a good turnout of GPs and consultants. Its always nice to have time to talk, and we debated how far we could take integration locally. Read Simon Stevens views on possible models.

Please do read the pack for the CCF (to be sent out shortly) and discuss in your practice as this is your chance to have input into all our commissioning intentions for next year, including those with the GP Confederation.

Finally its great to be top in England for diabetes annual reviews, fantastic work!

Clinical Senate

On Wednesday 8th October, the CCG held it's annual Clinical Senate at Homerton Hospital. It was an evening of debate, planning and a chance to meet Martin Kuper, the new medical director at Homerton and fellow consultant colleagues.

Thanks to all who came, we had an enjoyable time meeting up with our hospital colleagues. Some are keen to develop more integration, and think about forming an Accountable Care Organisation across all organisations. We did all agree that even better joint walking is crucial.

We have re-circulated all duty doc practice numbers to Homerton clinicians, and hope they will all now know what a care plan is!

Our 5 Year Outlook

Please download a copy of our final 5 Year Plan, submitted to NHS England, which outlines our clinical ambitions and plans to improve services. These have been developed in partnership through our Programme Boards, and we think set out an exciting range of initiatives, which should make a real difference for our patients.

Please look at this in your practice as it is the background to our CCF on the 6th November, to go over our commissioning intentions.

Clinical Lead Vacancies

The Children’s Programme Board would welcome interest from GPs for two new clinical CCG posts:1)      GP lead for Disabled Children’s Services2)      GP lead for Children’s Long Term Conditions (asthma, epilepsy and diabetes, initially)

The Children’s Board is changing …… with close joint working across health, social care and education and the implementation of the Special Educational Needs and Disabilities reforms, there has never been a better time to contribute to meaningful change for children’s health services in Hackney and the City. 

We need you to liaise with providers – including fellow GPs, the acute services, community health services, the local authority, public health and the voluntary sector - and lead for the CCG on redesigning and improving services and pathways for both these children, with disabilities or chronic conditions, and their families.

NHS City and Hackney CCG Super Patient Participation Groups 

City and Hackney CCG are pleased to announce that Healthwatch Hackney have won the contract to start delivering the Super Patient Participation Groups (SPPGs) which bring together the GP practice based participation groups in City and Hackney. 

Healthwatch Hackney will be delivering the service in partnership with Age UK in Hackney, and we expect the service to start in October. We are sure that their understanding of the NHS as well as their knowledge and experience of working with local communities will enable Healthwatch Hackney and Age UK to develop these participation groups into local hubs for involvement. Children’s Board report to the Clinical Executive Committee

The Children’s Programme Board (CPB) briefed the CEC on recent work and upcoming plans, including the launch of the vulnerable children’s contract had been delayed due to ongoing discussions with the GP Confederation.  The contract will be started as soon as possible.  Further work is underway to issue new guidance around children’s asthma, diabetes and epilepsy pathways and the CPB will continue to look at developing new children’s community health services and nurse specialist services as evidence supports them.  A transition service to...read more

Direct Access Colonoscopy Service (DACS)

In a meeting yesterday to review the progress of the Direct Access Colonoscopy Service (DACS), it was surprising to hear that 50% of all referrals to DACS so far have been rejected by the Homerton, and cancelled on Choose and Book. The reasons for this in most cases is for simple admin reasons. This is obviously creating a lot of extra work for GPs, who will then re-refer, and inconvenience to patients, and I am writing to try to improve this process.

The main reasons for the rejected referrals:- incorrect referral form used (standard letters and old InHealth forms are not accepted, as the DACS referral form also acts as a checklist to ensure proper preparation for the procedure) - see below for correct form- box not ticked to indicate that bowel prep has been prescribed (procedure would not be done if patient arrives in the department not having taken bowel prep)- box not ticked to indicate that patient information leaflet has been given (which also outlines when to take the bowel prep and important dietary changes beforehand)- outside age range (only ages 40-70 accepted) - consider referral to clinic if outside of this range

The correct forms can be found on the referral forms webpage under Gastroenterology, (one emis version that can be installed to self-populate). Please save to your local intranet, and remove any old referral forms.

We hope that the prescription of the bowel prep and the printing of the info leaflet will not prove too laborious and time-consuming in the challenges of a 10 minute consultation! It is appropriate that GPs do the prescribing (rather than this being sent out from the Homerton, who will not have seen or assessed the patient), and in setting up the service, the tariff was agreed on the basis of DACS not requiring much admin support at the Homerton end. We would welcome any feedback on the DACS service.

Hackney Best in England for Diabetes Checks

A huge pat on the back to us all! Our first 'Best in the whole country result'.

Results of the 2012/13 National Diabetes Audit which has just been published, showed that 78% of City and Hackney patients received all of the key checks recommended by the National Institute for Health and Care Excellence (NICE): blood glucose; blood pressure; cholesterol; eye screening; foot and leg health; kidney function; weight and smoking status, compared to just 59% nationally.

We are also doing well in out Long Term Conditions outcomes, but haven't had all the comparative data as yet.

Dementia Diagnosis Rates - Second Best in London!

The latest dementia figures have been published by HSCIC. The September figures show that nationally, 53.3% of the estimated number of people with dementia have a dementia diagnosis.  

Looking specifically at CCG figures for City and Hackney, our diagnosis rate is 66.5% with an estimated prevalence of 1,258. This identifies a gap of 3 people who would benefit from access to support by way of a dementia diagnosis. 

This shows that we are surpassing the target we set for 2014-15 which is 60.1%. 

Mental Health Programme Board

Primary Care Mental Health Transformation

Primary care mental health services in City and Hackney are currently being re-designed through a joint project that brings together clinical commissioners, GP Practices as providers, service users and East London NHS Foundation Trust. 

At present, primary care mental health services in City and Hackney play a key role in supporting people with a serious mental illness step down from secondary care mental health services to primary care. The services also support people, who are already in primary care but need to step up to a more enhanced level of care.  The current service is jointly provided through the SMI contract held by the GP Confederation and through practice based support from the secondary care mental health provider, East London NHS Foundation Trust.  

Following a clinically led detailed analysis of demand, which included a deep dive analysis of actual patients in selected GP practices it is clear that the service could become more inclusive and improve the transfer of patients from secondary care. To support this, the new service will have:•    Increased resources including higher numbers of liaison workers and support workers•    A stronger focus on outcomes and recovery•    Increased access to new community mental health resources such as the Mental Health Network•    Better navigation  to community resources through Liaison Workers•    Peer support•    Improvements in mental health training for practices•    Improved information, recording and monitoring

A lunchtime workshop will be held this December for GP practices to communicate the proposed service changes.  Date to be confirmed

Community Midwifery Changes

We know that the implementation of the new community midwifery service has caused disruption and upset to some practices.The purpose of the changes is to ensure all women can be seen close to home in the community and by their named midwife whenever possible. The overarching aim is to improve women's experience of their antenatal care.

However, as GPs we recognise that no longer having integrated working between GPs and midwives in some practices is a significant loss. The maternity programme board will be closely monitoring the service and then carrying out a formal review in the spring.

We welcome any GP feedback and will be asking GPs to input into the formal review. In the meantime, if there are any concerns about the running of the new service then please raise them via 'Duty of Candour' or directly to Dawn ([email protected]) or Kirsten ([email protected]).

Transforming Wound Care: exciting new contracts

Lymphoedema ServiceAs of 1 October 2014, City and Hackney has commissioned a new Lymphoedema Service. Previously known as “East London Wound Healing Centre”, Accelerate CIC was incorporated as a social enterprise provider of chronic wound and lymphoedema services in August 2011. Accelerate has been serving City and Hackney complex patients since 1995, however, we are pleased to bring their expertise closer to our patients through locally based clinics and home visits. This is in collaboration with the Homerton and is an exciting opportunity for staff and service development. 

Dressings Optimisation SchemeCity and Hackney CCG has commissioned Accelerate CIC to set up and manage a new dressings scheme, which encompasses a centralised dressings store. The aim of the DOS is to deliver reliable and timely access to wound management products whilst meeting clinical need and improving resource management. 

This service is a fundamental system change from the FP10 prescription route. It will be available to all C&H registered patients accessing wound care services through GP surgeries, District Nursing, Venous Leg Ulcer Clinics, Tissue Viability Service, and Nursing Homes within City and Hackney.   

Accelerate will be in contact with all providers to set up stakeholder groups for both new services and communicate set-up requirements for the DOS to launch.

Be Clear on Cancer Campaign- prostate cancer pilot

As you may be aware, Public Health England is launching a local Be Clear on Cancer pilot in London to raise awareness of the increased risk of prostate cancer amongst black men aged 45+.  The pilot will run from 20 October to 23 November 2014 in six London Boroughs of Hackney, Haringey, Newham; Lambeth, Lewisham and Southwark with the following key message:..read full article

£300k Community Fund Launched to Boost Hackney’s Health & Happiness 

Hackney Council is launching a new grants scheme to provide community and voluntary groups with funding for innovative health projects.  It would be great if GPs could promote the fund to any relevant groups that they work with.   The aim of the Healthier Hackney Fund is to find and support new ways to tackle some of the borough’s health challenges such as smoking, mental health, substance misuse and sexual health. Money from the £300,000 fund will be awarded to community groups and organisations that can demonstrate that their project will make an impact. It has three grant streams: ·    Healthy Activities. A grant of up to £30,000 to run practical activities that will prevent people from starting smoking or improve mental health. ·    Healthy Ideas. A grant of up to £8,000 to develop and pilot a new approach to tackling the entrenched problems related to substance misuse or sexual health. ·    Healthy Neighbourhoods. A £1,000 kickstart grant for neighbourhood projects that will get residents helping each other to be healthy. As well as funding, successful bidders will receive ongoing support from the Council’s public health staff to help maximise the impact of projects with their specialist skills, knowledge and connections. Cllr Jonathan McShane, Cabinet Member for Health, said: “Councils are learning to work smarter and achieve more with the same money. We know that local partners and external organisations sometimes have better ways of doing things, so the Healthier Hackney Fund was set up to make the most of those ideas and the energy and connections these groups have. “It is a huge opportunity to unite everyone in the borough behind the common cause of improving health and wellbeing and making Hackney a happier, healthier place to live.“  For more information visit http://news.hackney.gov.uk/healthier-hackney-fund.

CCG Charity Bake Off

On Friday 26th September, CCG staff took part in a bake off to raise money for Macmillan Cancer Research. After the judging, staff from St Leonard's Hospital were invited along to purchase the many entries.

See the picture of the winning Victoria Sponge cake, baked by our General Manger, Teresa.

The CCG managed to raise £170.88 towards this great charity. Special thanks to Eeeva, Laurie and Curtis for organising.
