VAULT ACCESSEmployees can access the Vault via the H&S Intranet page (if on the Beca network).

Employees can access the Vault via a separate URL if they are not on the Beca network we will provide this later

For any Vault issues please lodge a call via the Helpdesk

5VAULT MAIN PAGENote: Access to the Vault system is available to all Section Managers and H&S Reps using single sign on so if you are signed into the Beca network you can access the Vault directly. Employees can choose the required option which will take them to a report form for completion.

SMs, H&S Reps and Corporate Services - If you have actions to complete after a report has been lodged you would select this option.6REPORTING

REPORTINGREPORT HAZARD

A hazard is a situation that poses a level of threat to life, health, property, or environment.A hazard could be identified at office or work site during an inspection or site walk through.Hazards identified during site walk through are required to be registered in VAULT as Hazards.Hazards that are removed immediately could be closed after review and Risk Assessment carried out when the hazard identified are integral part of work which could not be eliminated.

Click on the Body Area/Injury and select the appropriate area. Click on Add to Injury List to confirm selection. Incident Files save files related to injury investigation (preliminary) report

Once you have completed the injury details your report is finished so click on Submit at the bottom of the page.SUBMIT YOUR REPORT

You will receive an incident number once you have successfully submitted an incident.UNSUCCESSFUL you get an error message (as below) then you need to scroll back up the form to see if any areas are highlighted by a red underline if so fill in the missing information and then select Submit again.

Once you have completed the injury details your report is finished so click on Submit at the bottom of the page. SUCCESSFULWHAT NEXTPerson reported / Person involved & H&S Safety Hub Coordinator receives an email on the incident
