HOM and theCommunity

Inform/Educate

My “personal” connection

HOM DIGITAL EXPERIENCE1. Mobile is changing the landscape of our

interaction with patients and families2. HOM’s app is unique to our service model3. The app will extend our reach to

great-grandchildren4. It puts the ability to donate

in palm of their hands5. It is a limitless “Swiss

Army Knife”

WHAT DOES THIS MEAN?

THE SITUATION

10 years ago 80% of our patients lived at home;

today less than 60% continue to live in their own homes.

THE PROBLEM

Increasingly, family members are distributed across the

country, and the world. Sons and daughters do not “see”

the care given at the bedside.

THE SOLUTION

A mobile app that notifies family when a visit occurs.

THE SOLUTION

IN TOUCH AND INFORMED ABOUT A LOVED ONE’S CARE

When a loved one is ill, families want to be informed about care, and they want to stay connected. Regular updates provide peace of mind to family members – whether they are near or far.

Hospice of Michigan developed a mobile app called HOM Cares that sends updates to the primary caregiver and to the network of family and friends that the primary caregiver sets up.
