Event DatesThe 2010 US Open Wheelchair Tennis Competition will take place Thursday, September 9, through Sunday, September 12, at the USTA Billie Jean King National Tennis Center in Flushing, NY.

Entry Deadline for Singles and DoublesThe Entry Form (see last page) must be signed and received by the USTA Wheelchair Tennis Department in White Plains, NY, by 5 pm Eastern Daylight Time (EDT), Thursday, July 29. The entry deadline is the same for Singles and Doubles events, and there are no Doubles-only entries.

Registration and Mandatory Sign-InAll Singles players must personally sign in and register at the Player Registration Office of the USTA Billie Jean King National Tennis Center between 8 am and 4 pm on Tuesday, September 7, or Wednesday, September 8. Sign-in is mandatory for all players and is not permitted via telephone.

Only those players who personally sign in by 4 pm on Wednesday, September 8, will be accepted into the 2010 US Open Wheelchair Tennis Competition.

Singles DrawThe eight Men, eight Women, four Quad Draw will be made Wednesday, September 8, after mandatory sign-in. Singles events will begin with first round matches on Thursday, September 9.

Doubles DrawThe Doubles Draw will consist of four Men’s teams, four Women’s teams and two Quad teams. Entry for Doubles events will be by this Entry Form due Thursday, July 29 (see Entry Deadline above), and by personal sign-in at the Player Registration Office of the USTA Billie Jean King National Tennis Center prior to 4 pm on Wednesday, September 8. There are no Doubles-only entries. The Doubles events will begin with first round matches on Friday, September 10.

Wild CardsPlayers wishing to apply for a Wild Card into the 2010 US Open Wheelchair Tennis Competition must submit both the Entry Form and a Wild Card Request Form (both due by 5 pm EDT, Thursday, July 29). Wild Card Request Forms are available via the Players Only Web page (USOpen.org/playersonly). Only those players submitting both applications will receive consideration for a Wild Card.

Credential Application FormAll players, guests, coaches and other individuals requesting a credential for the 2010 US Open Wheelchair Tennis Competition must submit a Credential Application Form which is available via the Players Only Web page (USOpen.org/playersonly). Please note this application must be printed, completed and received via regular or overnight mail no later than Thursday, July 29. Credential Application Forms are NOT accepted via e-mail. The issuance of credentials is subject to the prior approval of the USTA, and such credentials can be withheld or revoked at any time for any reason at the sole discretion of the USTA and/or its representatives. Credentials may not be picked up until Tuesday, September 7.

Additional CredentialsPlayers competing in the US Open Wheelchair Tennis Competition receive three (3) guest credentials (all subject to application approval) that allow access to the Player Lounge and tournament shuttle bus service.*

One (1) of these three (3) credentials may be assigned to a coach with access to the appropriate Locker Room (coaches of female players are assigned to the Indoor Training Center Locker Rooms).

Everyone must wear his/her credential at all times and abide by all credential terms and conditions issued by the USTA and/or its representatives. Credentials are not transferable, and their appropriate use will be strictly enforced.

The USTA reserves the right to alter the access allowed for any credential at any time with or without notice.

Player MeetingAll players must attend the pre-tournament player meeting at 4 pm on Wednesday, September 8, at the USTA Billie Jean King National Tennis Center, with the exact location to be announced during registration.

Rules and RegulationsThe Rules, Regulations and Code of Conduct adopted and published by the ITF will govern all play. All matches will be the best of three tie-break sets.

Surface and Official Tennis BallsThe US Open Wheelchair Tennis Competition will be played on DecoTurf II. The Wilson US Open tennis ball (Extra Duty for Men, Regular Duty for Women and Quads) is the Official Ball of the US Open. Ball change occurs after the first seven games, then after every nine games. Six balls are in play at all times.

Hotel Accommodations/Per DiemPlayers, coaches and player guests are required to make their own hotel reservations for the US Open Wheelchair Tennis Competition.Wheelchair participants will receive a per diem allowance of $230 per day through the day of the player’s complete elimination from

the tournament, plus three days. All per diem payments will be included in prize money checks issued by the Purser’s Office. Players are responsible for settling their own hotel bills.

TransportationShuttle Bus Service:* Full shuttle bus service between the Grand Hyatt, Waldorf-Astoria, Le Parker Meridien and InterContinental (The Barclay) hotels and the USTA Billie Jean King National Tennis Center will begin Monday, August 30. Shuttle bus service will begin at 8 am and will operate up to 45 minutes following the last match. All final transportation information, including schedules and exact shuttle bus stop locations, will be available via the Players Only Web page (USOpen.org/playersonly), the 2010 US Open Player Handbook and the 2010 US Open Transportation Brochure.

Car Transportation Limited car transportation service for Wheelchair competitors will begin Monday, September 6, with airport pick-ups only. Full car transportation service will begin Tuesday, September 7. Car transportation is available for players only, with priority given to those players with matches that day and seeded Singles players still competing in the tournament. The car transportation rule will remain “one player, one car” whenever possible, and requires the player to be one of the passengers in the car. Car transportation will only be available between the USTA Billie Jean King National Tennis Center and the player’s hotel/local residence, provided the location is within a 15-mile radius of the USTA Billie Jean King National Tennis Center (see 2010 US Open Player Handbook or 2010 US Open Transportation Brochure for map defining the 15-mile radius). Please call the Transportation Office at 718.760.6300 between the hours of 8 am and 11 pm for reservations and information. Car transportation reservations require at least two (2) hours notice, and service is provided based on vehicle availability. All final transportation information, including schedules and exact shuttle bus stop locations, will be available via the Players Only Web page (USOpen.org/playersonly), the 2010 US Open Player Handbook and the 2010 US Open Transportation Brochure.

Meal Allowance CreditEach player will receive an automated daily meal allowance of $25 per day on his/her credential beginning Tuesday, September 7, through the night of complete elimination (Singles and Doubles) from the tournament.

*Please note: Shuttle buses are not wheelchair accessible.

LEGAL CONSENTIn consideration of the acceptance of my entry, I acknowledge the risks associated with participating in the 2010 US Open Tennis Championships Wheelchair Competition and/or any event staged in connection therewith (collectively, the “2010 US Open”), accept those risks voluntarily and hereby for myself and on behalf of my heirs and legal representatives release and discharge United States Tennis Association Incorporated (“USTA”), USTA National Tennis Center Incorporated, the City of New York, and each of their respective officers, directors, employees, volunteers and representatives (collectively, the “Released Parties”), of and from any and all claims and demands of every kind, nature and character for any loss, injury, damage, which I may suffer or sustain, by negligence or otherwise, to the fullest extent permitted by law in connection with any aspect of the 2010 US Open, including but not limited to any action and/or decision taken regarding the conduct of any matches.

In consideration of the acceptance of my entry, I hereby agree to abide by all applicable rules, regulations and codes of the USTA, Grand Slam Committee, ITF and the 2010 US Open, including but not limited to the following: (1) Every player must be dressed in a professional manner for all matches. Clean and customary tennis attire must be worn. The Referee will have the authority to order the player to change his/her attire immediately if, in his opinion, it is not acceptable. Failure of a player to comply with such order may result in immediate default, and the player will be fined according to the Grand Slam Code of Conduct; (2) At all times while on-site at the 2010 US Open (both on-court and off-court including press conferences), no commercial identification shall be displayed by player other than as specifically permitted by the 2010 Official Grand Slam Rule Book; and (3) Players are strictly prohibited from taking motion pictures, still pictures, sound recordings and any other reproductions of any description in the locker rooms. I am aware of and accept that: (1) The Referee will have the authority to take any action he deems necessary in the interests of the 2010 US Open, including but not limited to defaulting a player, without any resulting liability for any of the Released Parties; (2) My coach (to the extent he/she receives a 2010 US Open Coach Credential) will be required to be available for and participate in televised interviews as requested by the USTA or its designee immediately prior to, during and immediately following my matches; (3) I am strictly prohibited from requesting a 2010 US Open Credential and/or player ticket for any person(s) other than my coach, trainer, family members and bona fide guests whose attendance is directly related to my participation in the 2010 US Open. 2010 US Open Player Guest Credentials and player tickets, if available, may only be used for legitimate player guests (no resale or transfer by player guest), and player may not receive any direct financial or other benefit as consideration. The USTA will hold me responsible for any sale, purchase or transfer of my 2010 US Open Coach/Trainer/Player Guest Credentials and/or player tickets under the Grand Slam Code of Conduct and may also revoke my coach/trainer/player guest credential and/or player ticket privileges in the future; and (4) The USTA will hold me responsible under the Grand Slam Code of Conduct for the behavior of anyone receiving a 2010 US Open Credential at my request. In addition, I am aware of and accept that: (1) I am strictly prohibited, directly or indirectly through others, from wagering anything of value in connection with the 2010 US Open; (2) I am strictly prohibited, directly or indirectly through others, from offering or receiving anything of value to or from any person with the intent to influence any player’s efforts in the 2010 US Open; (3) I will strictly adhere to the Uniform Tennis Anti-Corruption Program; and (4) Any violation of these regulations will subject me to severe financial penalties as well as a maximum penalty of permanent suspension from play in all Grand Slams.

I am aware of and accept that: (1) I may be required to undergo drug testing imposed upon the 2010 US Open by authorities outside its control and/or the governing bodies of the sport; (2) I confirm that I have read and understood Article VI, Section 7 of the Wheelchair Tennis Handbook 2010; and (3) I confirm that in accordance with Article X, Section 29 (K) of the same that I have adequate travel and medical insurance. I am aware of and accept that: (1) The US Open is subject to extension and/or cancellation (in whole or in part) due to force majeure or other events as determined by the USTA in its sole discretion; and (2) The USTA reserves the right, in its sole discretion, to reduce and/or reallocate prize money in the event the US Open is cancelled in whole or in part, for whatever reason.

In consideration of the acceptance of my entry: (1) I hereby agree that I will be respectful of US Open sponsors, television partners, business partners, and all attendees; and (2) I hereby grant to the USTA and its assignees the right to use or authorize the use of my name, voice, likeness, signature and biographical material without additional compensation for the purpose of publicizing, promoting and advertising the USTA and its affiliates, and the US Open and for the televising, broadcasting and filming of the same; and hereby grant and assign to the USTA and its assignees in perpetuity throughout the world the right to make, use, show and reproduce in any way now known or hereafter devised, for commercial and non-commercial purposes without additional compensation, still pictures, motion pictures, sound recordings, posters, live, taped or filmed television taken of me during the 2010 US Open; provided, however, the use of my identification (e.g. name, photograph, likeness, biography, voice, etc.) shall not be identified as or represented to be an endorsement by me of any product, service or company. I further acknowledge that the player guest boxes in Arthur Ashe Stadium may be equipped with microphones and I will advise my guests accordingly.

In further consideration of the acceptance of my entry, and to assist the USTA in its mission to promote and develop the growth of tennis, I hereby agree to: (1) Cooperate with the media in reasonable pre-match interviews, post-match interviews, press conferences, interviews on-court and finals ceremonies; (2) Participate, if requested, in Arthur Ashe Kids’ Day on Saturday, August 28, 2010, or another reasonable promotional activity (not to exceed 60 minutes) related to the 2010 US Open; and (3) Participate, if requested, in the 2011 US Open Draw Ceremony should I win the 2010 US Open. My participation in any of the promotional activities outlined herein shall not be identified as or represented to be an endorsement by me of any product, service or company.

I understand that: (1) Entry by my agent or through any authorized entity on my behalf in no way diminishes the application of the above rules; and (2) I will not be allowed to collect my credential or participate in any competition at the 2010 US Open until this Entry Form has been duly signed by me. This agreement shall be construed in accordance with the laws of the State of New York, USA, without regard to its principles of conflicts of laws. Any dispute arising under this agreement shall be litigated in a federal or state court of competent jurisdiction located within the City of New York, State of New York, USA.
