Pure Air Control Services, Inc. (PACS) is pleased to submit the Environmental Remediation Report for the Duct Cleaning, PURE-Decon Room Sanitization and PURE-Liner of two (2) units at the University located at 2901 Lake Blvd., Cleveland, OH 44101. This work was completed on September 18th, 2014.

Other Comments:
Today's remediation process at the University consisted of:
• The environmental removal of mold contaminated insulation in the supply ductwork & supply side of AHU # 2
• AHU # 2 & supply ductwork associated with this AHU, were mechanically brushed & HEPA vacuumed
• Section of the return ductwork was also cleaned, but not treated
• All work areas were cleaned & AHU was brought back on line before leaving the job site.
• All material was double bagged, disinfected & properly disposed of
• Lockout/tagout was in place throughout the remediation process and removed at the end of the shift
• All work areas were cleaned & AHU was brought back on line before leaving the job site.
We performed topical cleaning of the contained area located in / at __________, All surfaces were HEPA vacuumed then wet wiped
with multicide (or other antimicrobial)
