Enclosed are two (2) copies of your approved Well Construction Permit for the captioned well(s). As part of the Commission's approval, the following special conditions were added and are part of your permit under Standard Permit Condition 10:

Special Conditions

1. No permanent monitor tube is required. 2. The long-term continuous test shall be at least 8 hours. 3. No step-drawdown test is required. 4. Cement grout of the annular space shall seal at least 70% of the distance between

the ground surface and the top of the aquifer. 5. The well should not be used for drinking water unless it is properly tested and

treated. 6. If potable water is used to supply both domestic and irrigation purposes in a single

system, the permitee shall eliminate cross-connections and backflow connections by physically separating potable and non-potable systems by an air gap or an approved backflow preventer, and by clearly labeling all non-potable spigots with warning signs to prevent inadvertent consumption of non-potable water.

Additionally, the Commission authorized the Chairperson to approve and issue a pump installation permit supported by information provided from aquifer pumping test results, required in Well Construction Standard Condition 6e, subject to the Standard Pump ~stallation Conditions which will be issued to you when we receive your aquifer pump test results and completed Well Construction Report (Part 1).

OvELL CONSTRUCTION PERMIT 0 Kaupo Ranch Well #1, Well No. 3806-02

In accordance with Department of Land and Natural Resources, Commission on Water Resource Management's Administrative Rules, Section 13-168, entitled 'Water Use, Wells, and Stream Diversion Works·, this document permits the construction and testing of Kaupo Ranch Well #1 (Well No. 380~2) at Kaupo, Hana, Maul, TMK 1-7-2:1, subject to the Hawaii Well Construction & Pump Installation Standards (1123197) which include but are not limited to the following conditions:

1. The Chairperson of the Commission on Water Resource Management (Commission), P.O. Box 621, Honolulu, HI 96809, shall be notified, in writing, at least two (2) weeks before any work authorized by this permit commences.

2. The well construction permit shall be for construction and testing of the well only. A minimum one-inch diameter monitor tube shall be permanently installed, in a manner acceptable to the Chairperson, to accurately record water levels. The permittee shan coordinate with the Chairperson and conduct a pumping test in accordance with the attached Aquifer Pump Testing Procedure (attached). The permittee shaD submit to the Chairperson the test results as a basis for supporting an application to install a permanent pump and withdraw water for use. No permanent pump may be installed until a pump installation permit is approved and issued by the Chairperson.

3. In basal ground water, the depth of the wen may not exceed one-fourth (114) of the theoretical thickness (41 times initial head) of the basal ground water unless otherwise authorized by the Chairperson.

4. The permittee shan incorporate mitigation measures to prevent construction debris from entering the aquatic environment, to schedule work to avoid periods of high rainfall, and to revegetate any cleared areas as soon as possible.

5. In the event that subsurface cultural remains such as artifacts, burials or concentrations of shells or charcoal are encountered during construction, the permittee shan stop work and contact the Department's Historic Preservation Division (587-0045) immediately.

6. The proposed weI construction shall not adversely affect existing or future legal uses of water in the area, including any surface water or estabrlShed instream flow standards. This permit or the authorization to construct the well shan not constitute a determination of correlative water rights.

7. The following shan be submitted to the Chairperson within sixty (60) days after completion of work: a. Wen completion report. (attached - Part I, Wen Construction Report). b. Elevation (referenced to mean sea level, msO survey by a Hawaii-licensed surveyor. c. As-built sectional drawing of the well. d. Plot plan and map showing the exact location of the well. e. Complete pumping test records, including time, pumping rate, drawdown, chloride content, and other data.

8. The permittee shaH comply with all applicable laws, rules, and ordinances, and non-compliance may be grounds for revocation of this permit.

9. The wen construction permit application is incorporated into this permit by reference and is subject to the HawaD Well Construction & Pump Installation Standards (1123197).

10. The permit may be revoked if work is not started within six (6) months after the date of approval or if work is suspended or abandoned for six (6) months, unless otherwise specified. The work proposed in the well construction permit application shall be completed within two (2) years from the date of permit approval, unless otherwise specified. The permit may be extended by the Chairperson upon a showing of good cause and gOod-faith performance. A request to extend the permit shaD be submitted to the Chairperson no later than three (3) months prior to the date the permit expires. If the commencement date is not met, the Commission may revoke the permit after giving the permittee notice of the proposed action and an opportunity to be heard.

11. If the well is not to be used it must be properly capped. If the well is to be abandoned then the permittee must apply for a well abandonment permit in accordance with §13-168-12(f) prior to any well sealing or plugging work.

12. Special conditions in the attached cover transmittal letter are incorporated herein by reference.

BACKGROUND The application for this well was accepted as complete on November 25, 1996.

ISSUES/ANAL YSIS Published: December 1996

DoH Review: The Safe Drinking Water Branch recommends that potable water wells such as this one not requiring Department of Health approval be tested for bacteriological and chemical presence and be routinely monitored thereafter. In similar situations, that Branch has also specified that potable and irrigation applications using the same source be equipped to prevent backflow from non-potable to potable systems, and labelled to prevent inadvertent consumption of non-potable water.

Staff review: The proposed well would tap fresh basal ground water, mauka of a known spring serving the same owner but subject to inundation during storm, conditions. Proposed use is approximately 72,000 gallons per day for stockwater use and some domestic use by field employees. There are no other wells within one mile. No adverse impacts are expected.

Water discharged from the well during the step-drawdown and long-term test shall be carried away from the well to a distance sufficient to preclude circulation of the discharge water downward to the ground water table. Acceptable measurement tolerances for pump tests are as follows: a. discharge from the well shall not fluctuate beyond ± 10%. b. depth to water measurements in the pumped well shall be accurate to 0.01 feet. c. time shall be accurate within ± 1 %. Raw and graphed data shall be provided to the Commission in the approved Table 1 and Graph 1 format.

Step-Drawdown Test (To determine efficiency of the well and give a preliminary view on the yield quantity and water quality of the well) 1. At least two days prior to the initiation of the step-drawdown test, the water level in the

pumped well shall be measured every 12 hours, to obtain the pre-test trend in water

levels. The step-drawdown test consists of continuously pumping the well for four hours at four different rates; a. the change from one pumping rate to the next must tbe sufficient to induce an

observable change in water level in the well from the rrevious pumpage rate. b. the four different rates may represent the full range 0 pump capacity (if the

yield can sustain this), but this is not necessary. Each pumping rate should run for one hour, then institute the new rate as rapidly as possible. Pumping should be continuous through the entire step-drawdown test. Begin pumping at the lowest rate and conclude with the highest rate. Measure chloride concentration and temperature of the discharge water at least five times: a. at the very beginning of the test, and b. at the end of each pumping rate during the step-drawdown test. Following the termination of the step-drawdown test, measure the water level in the pumped well a sufficient number of times to establish that the water level fully recovers to the pre-test level.

Lon{l-Terrn Continuous Test (To determine hydraulic properties of the aquifer, to explore for and IdentifY nearby aquifer boundaries such as streams or dikes, and to observe the trend in chloride concentration of the discharge water) 1. Commence the long-term test only when the water level in the pumped well is at pre-test

levels (generally about four hours following a step-drawdown test); measure water level immediately before commencing long-term test.

The pump rate for the long-term test should be sufficient to create an observable drawdown. Run the test 24 hours per day for at least seven days, except that if the water level remains the same for a period of 24 hours, the test can be terminated. Measure the chloride concentration and temperature of the discharge water at the beginning of the test and every six hours thereafter. Measure depth to water with sufficient frequency that each logarithmic cycle in time on the data plots (Graph 1) contains at least ten data points spread through the cycle. (Example: measure at t=O, 1, 1.5, 2, 2.5, 3, 4, 5, 6, 7, and 8 minutes for the ffrst ten minutes, and at all succeeding decimal multiples of these - t=10, 15, 20, 25, etc.) Following the termination of the long-term continuous test, measure the water level in the pumped well a sufficient number of times to establish that the water level fully recovers to the pre-test level.

Federal and state regulations define a public water system as a system that serves 25 or more individuals at least 60 days per year or has at least 15 service connections. All public water system owners and operators are required to comply with Hawaii Administrative Rules, Title 11, Chapter 20, Rules Relating to Potable Water Systems.

The application indicates that this will be a new source of potable water. Section 11-20-29 of Chapter 20 requires that all new sources of potable water serving a public water system be approved by the Director of Health prior to its use. Such an approval is based primarily upon the submission of a satisfactory engineering report which addresses the requirements set in Section 11-20-29.

The permit application indicates that the source will be used for potable water. If the well doesn't provide water to a public water system, I recommend the private owner perform bacteriological and chemical analyses prior to useage as a drinking water source and thereafter perform routine analyses to monitor the water quality.

Attached is a Geologist's Report by Stephen P. Bowles, written in February of 1996, referring to a possible location of a well on the ranch to be bored at elevation 600', referred to as Kumunui Mauka. It is expected that a good, dependable source of clean, potable water could be developed at this location, but it will be a fairly costly well to develop.

PUNAHOA SPRING

Attached is a Geologist's Report by Stephen P. Bowles, written on February 16, 1996, referring to the spring on the coast known as the Punahoa Spring. This spring has supplied the Ranch with water for their stock watering system for many years. The Ranch is presently moving the main pump system across the road to a mauka location, where it will not be subject to storm damage.

This spring is subject to inundation and contamination by sea water during storm periods, with possible damage to the submersible pump presently used in this spring to supply feed water to the main pump.

Drilling a shallow well of approximately 85' depth at a location next to the relocated main pump system should allow access to the water strata that is presently accessed at the Punahoa Spring. This would be a 6" diameter cased well. The water quality from this proposed well should be at least as good as the water from the spring, and there is an excellent chance that it will be better quality water than the spring. Complete protection from sea water contamination during stormy periods is also assured with this well.

The Punahoa Spring pump sump is subject to salt water contamination during high surf periods. As an alternative to this pump site,·a wen is proposed at an elevation of 80 to 85 feet above sea level just mauka of the Kaupo to Hana road up slope from the present pump at Punahoa.

A six inch cased well, drilled to a depth of 5 to 10 feet below sea level, should provide an adequate back up to the· proposed mauka well at 500 feet. Care must be exercised when drilling below sea level to find the best combination of yield to salinity. It is expected that the water level will stand several feet above sea level and that at small pumping rates of 20 to 30 gpm (gallons per minute) the water will be fresh and can serve as a drinking water stand by source.

The well would be constructed in the lavas of the Kula volcanic series. Deep surface weathering may reduce permeability so it may be necessary to cut back further into the ridge when sitting the well. If at all pOSSible, the well should enter fresh rock above sea level.

The proposed well is to be located at an elevation of about 600 feet as indicated on the map to be attached with the application. A recently constructed (1995) water tank is located adjacent to the well site. Surface lava flows are capped with a' a clinkers of the Hana volcanic series. It is expected that the dense Hana lava will be penetrated at a depth of 30 to 50 feet. A great unconformity is evidenced by gravel, rubble and the weathered surface of the older, thin bedded lavas of the Kula volcanic series (see cross-section) within the Kula lavas.

Groundwater levels at the site are unknown but most likely will be associated with a basal fresh water lens with the water level standing between elevation 5 and 10 feet. The well should penetrate to an elevation of -20 to -30 feet and should be test pumped at a rate of about 50 gpm before casing is permanently installed. Final setting of the casing depths and hole completion should be based on the results of this test.

A minimum inside diameter of casing should be 6 inches with 8 inches preferred to allow for future expansion of the pump capacity. About 20 feet of perforated casing should extend below sea level and all casing should be steel. A cement basket should be placed at sea level and back filled with several feet of beach sand followed by any initial cement plug. Once the initial plug is cured, the balance of the annular space should be sealed with neat cement grout placed in 100 foot increments which should be cured over night following each 100 foot placement.

The well bore should be drilled to the original depth with a 12 inch bore. A small bit can be used to drill open hole after casing, should such open hold be needed.

Attached is a Geologist's Report by Stephen P. Bowles, written in February of 1996, referring to a possible location of a well on the ranch to be bored at elevation 600', referred to as Kumunui Mauka. It is expected that a good, dependable source of clean, potable water could be developed at this location, but it will be a fairly costly well to develop.

PUNAHOA SPRING

Attached is a Geologist's Report by Stephen P. Bowles, written on February 16, 1996, referring to the spring on the coast known as the Punahoa Spring. This spring has supplied the Ranch with water for their stock watering system for many years. The Ranch is presently moving the main pump system across the road to a mauka location, where it will not be subject to storm damage.

This spring is subject to inundation and contamination by sea water during storm periods, with possible damage to the submersible pump presently used in this spring to supply feed water to the main pump.

Drilling a shallow well of approximately 85' depth at a location next to the relocated main oumo system should allow access to the water strata that is

presently accessed at the Punahoa Spring. This would be a 6" diameter cased welL The water quality from this proposed well should be at least as good as the water from the spring, and there is an excellent chance that it will be better quality water than the spring. Complete protection from sea water contamination during stormy periods is also assured with this well.

The Punahoa Spring pump sump is subject to salt water contamination during high surf periods. As an alternative to this pump site,·a wen is proposed at an elevation of 80 to 85 feet above sea level just mauka of the Kaupo to Hana road up slope from the present pump at Punahoa.

A six inch cased well, drilled to a depth of 5 to 10 feet below sea level, should provide an adequate back up to the· proposed mauka well at 500 feet. Care must be exercised when drilling below sea level to find the best combination of yield to salinity. It is expected that the water level will stand several feet above sea level and that at small pumping rates of 20 to 30 gpm (gallons per minute) the water will be fresh and can serve as a drinking water stand by source.

The well would be constructed in the lavas of the Kula volcanic series. Deep surface weathering may reduce permeability so it may be necessary to cut back further into the ridge when sitting the well. If at all possible, the well should enter fresh rock above sea level.

The proposed well is to be located at an elevation of about 600 feet as indicated on the map to be attached with the application. A recently constructed (1995) water tank is located adjacent to the well site. Surface lava flows are capped with a'a clinkers of the Hana volcanic series. It is expected that the dense Hana lava will be penetrated at a depth of 30 to 50 feet. A great unconformity is evidenced by gravel, rubble and the weathered surface of the older, thin bedded lavas of the Kula volcanic series (see cross-section) within the Kula lavas.

Groundwater levels at the site are unknown but most likely will be associated with a basal fresh water lens with the water level standing between elevation 5 and 10 feet. The well should penetrate to an elevation of -20 to -30 feet and should be test pumped at a rate of about 50 gpm before casing is permanently installed. Final setting of the casing depths and hole completion should be based on the results of this test.

A minimum inside diameter of casing should be 6 inches with 8 inches preferred to allow for future expansion of the pump capacity. About 20 feet of perforated casing should extend below sea level and all casing should be steel. A cement basket should be placed at sea level and back filled with several feet of beach sand followed by any initial cement plug. Once the initial plug is cured, the balance of the annular space should be sealed with neat cement grout placed in 100 foot increments which should be cured over night following each 100 foot placement.

The well bore should be drilled to the original depth with a 12 inch bore. A small bit can be used to drill open hole after casing, should such open hold be needed.
