Report on Medical Textiles

The Scope of Medical Textiles in India

Technical textiles are one of the faster growing sectors of the global textile

industry. Textile materials manufactured primarily for their technical performance and

functional properties rather than their aesthetic and decorative values are commonly

known as Technical textiles. In India, technical textile market is estimated to be worth

Rs. 20,128 Crore in 2005 and the market is expected to reach Rs. 42,006 Crore by

2007-081. The market size and potential of various sectors in technical textiles and

their rankings are shown in Table-1. Textiles used for medical purposes, termed as

Medical textiles are found to be in 6th position among technical textiles.

Indian Medical Industry

The Indian healthcare sector growth is expected to be more than 17 % per

annum in view of the rising per capita spending on healthcare products, greater

exposure to international products in this area, importance and awareness of

healthcare insurance and medical tourism4. Healthcare industry is fragmented into

corporate hospitals, several small hospitals and nursing homes.

Healthcare industry is a $ 17 B / year business in India5. In 2003, a CII and

Mckinsey study projected that India's healthcare market could touch Rs.3,200 million

by 20126.

The share of Meditech, a part of healthcare sector in Indian technical textiles

market is found to be 5%. Among the wide range of medical textile products,

healthcare and hygiene products including sanitary napkins (35%), surgical dressings

(30%), sutures (20%) and diapers and orthopedic implants were contributing

considerably to the meditech market in India7.

Some demand sectors in India for medical textiles consists of the following8.

While IV filters are demanded at very high volume running to several millions

perhaps all the other devices are at lower demand. Typically in India about 35000

open heart surgeries are done and hence that will be the volume requirement for filters

for cardiac surgery. About 60% of the need is met by imports.

Potential of Indian Healthcare Market

� Population of 1.02 B(2001 census) demand high quality health services at a

competitive price5.

� Medical tourism could be $1B / year business by 20125. According to Confederation

of Indian Industries, India has a potential to attract 1 million health tourists per annum,

which could contribute $ 5 billion to the country’s economy.

� Privatisation of healthcare and health insurance is granting better acces to middle and

high income groups (currently only 10-15% of population is covered under a health care

coverage)5. Provisions on health insurance will make a difference to the quality of life,

thereby promoting demand for the sector.

� Private hospitals, importing more high-tech medical equipment and supplies, demand

for high quality single use medical textiles such as face masks, head covers, shoe covers,

drape and gowns as high as 40%5.

� The heart hospitals are growing exponentially increasing the number of bypass

surgeries and angioplasties in India. In the year 2002 itself, about 100,000 bypass

surgeries and 150,000 angioplasties were performed. This higher number also increases

the demand for high quality medical textiles in considerable amount5.

� The purchasing power of India’s middle class people (about 200 million) is found to

be Rs. 5000 to Rs. 10000 on an average per person every year primarily on preventive

care and health checkups. The children in these families are potential consumers for all

kinds of baby diapers, baby wipes etc. The currently low penetration of these products

provides an untrapped market for new entrants to India11.

� The consumers for disposable nonwoven products in the age group of 18 to 35 is

nearly 500 million. This group is well educated and has high enough disposable income to

afford nonwoven disposable products12.

Categories of medical textiles

The wide range of textile products used in the medical industry are classified in to

four major segments namely Non-Implantable materials, Implantable materials,

Extracorporeal devices and Healthcare & hygiene products.

1. Non-implantable materials

These materials are used for external applications on the body and may or may not

contact with skin. They are employed as covering, absorbent, protective and supports for

injured or diseased part.

� Primary wound dressings are placed next to the wound surface.

� Absorbent dressings are similar to wound pads used in surgery, manufactured from

well-bleached, carded and cleaned cotton fabrics.

� Bandages are narrow cotton or linen, plain weave cloth of low texture, either woven

or knitted,

used to hold dressings in place over wounds. Some of the types of bandages are simple, light

support, compression, orthopedic and four-layer bandages. The market share of bandages are

shown below2.

Karl Otto Braun (KOB) is the world's leading manufacturer of elastic bandages and

fabrics for the medical market with annual production over 60 million square meters of fabric

exported to more than 80 countries. Its special features are Cohesive bandages which stick to

themselves but not to hair or skin, used for non-slipping support and relief bandaging on

joints, conical and round parts of the body, etc. Brand names of bandages manufactured by

KOB are given in Table-4.

� A Plaster cast consists of gauze impregnated with plaster of paris. The modern plaster

fabric is made from spun bonded nonwovens of cotton, viscose, polyester or glass

fibre.

� Protective eye pads are used to lit over the eye in outpatient clinics and industrial

medical departments.

� Adhesive tapes is narrow, plain weave fabric coated with adhesive paste. It is used

with other pads to conform them to the injury.

2. Implantable materials

The implantable materials are used when natural way to transplant defective body

parts is not always possible.

� Cardiovascular implants are used to replace arteries or veins of the natural

cardiovascular system that are blocked or weakened.

a) Vascular grafts

� Made by warp knitting technique

� Types – Polyester grafts, Dacron grafts, Polytetrafluoro ethylene grafts

� Grafts are inserted to bypass the blockages and restore circulation

� W.L Gore & Associates dominating the market with a share of about 60%

followed by

Edward Life Sciences and Boston Scientific

� Market would grow at an annual growth rate of about 2% - 3%2.

� The demand is estimated to increase from Rs. 3.19 Crore (2003-04) to Rs.3.59

Crore (2007- 2008).

b) Heart valves

� Mechanical valves and tissue valves are the major types

� Indian exports of heart valves is in the range of Rs.0.15 Crore to Rs.0.72 Crore

(2001-03)2.

� Imports are mainly from Canada, Switzerland, Singapore, etc (Rs.11.54 to

Rs.23.86 Crore).

� Market size is estimated to grow at an average annual growth rate of 7%. Future

market would increase from Rs. 56 Crore in 2003-04 to Rs. 73 Crore by

2007-082.

c) Implants for Ligaments

Braided composite structure made of carbon and polyester is suitable for the

replacement of knee ligaments.

� Soft tissue implants

a) Artificial tendon (Mesh)

� Existing market size was assessed at about Rs. 20 Crore in 2001-022.

� Market size for artificial tendon (mesh) is estimated to grow at an average annual

growth rate of 11 %. Accordingly, the market size for artificial tendon (mesh)

would increase from Rs. 25 Crore in 2003-04 to Rs. 37 Crore by 2007-082.

� Orthopaedic implants

a) Artificial joints

� Inor Orthopaedics and Johnson & Johnson are major players

� Imports have been from Germany, France, Switzerland, USA, etc and were in the

range of Rs. 0.59 Crore to Rs. 4.07 Crore (2001-03)2. Indian exports of artificial

joints were in the range of Rs. 1.03 Crore to Rs. 1.90 Crore (2001-03)2.

� Market size for artificial joints is estimated to grow at an annual rate of 12 % and

accordingly, the market size of artificial joints is estimated to increase from Rs.

9.00 Crore in 2003-04 to Rs. 14.21 Crore by 2007-082.

� Sutures

� A surgical suture is a thread ranging in diameter from about 0.01 to 1 mm, either

braided or monofilament, and is used to tie blood vessels, to close wounds and to

join tissues.

� Under ideal conditions sutures should possess some attributes such as

absorbability, breaking strength, capillarity, elasticity, fluid absorption, knot-pull

tensile strength, knot strength, memory, plasticity, pliability, straight-pull tensile

strength and suture pullout value.

� Based on their nature sutures are categorised in to absorbable (degradable) and

non-absorbable (non-degradable) sutures. The various materials used for non-

absorbable sutures are nylon, polypropylene, silk, polyester and

polytetrafluoroethylene (PTFE).

� Indian market for sutures is assessed at around Rs.280 Crore with absorbable

sutures having 60% share amounting to Rs. 168 Crore while 40 % of the share is

accounted for by non- absorbable sutures2.

� Market share of non-absorbable sutures using silk accounts for 50%, nylon 10%

and the remaining 40% by other raw materials2.

� Indian exports of sutures are mainly to countries like Tunisia, Bangladesh,

Indonesia, Netherlands, Philippines, etc. Exports have been in the range of

Rs. 11.15 Crore to Rs.17.34 Crore during the period 1999-2002. The imports of

sutures have been from Denmark, Germany, Korea, USA, etc and were in the

range of Rs. 3.39 Crore to Rs. 6.46 Crore 2.

� Annual expenditure of hospitals on sutures has been increasing in the range of

10-20 % per annum. Accordingly, the demand for sutures is expected to grow

from Rs.280 Crore in 2003-04 to Rs. 441 Crore in 2007-20082.

3. Extracorporeal devices

Extra corporal devices are mechanical organs that are used for blood purification and

include the artificial kidney, the artificial liver and the mechanical lung.

� Artificial kidney

� Its function is to circulate the blood through a membrane, which may be either a

flat sheet of hollow hair sized cellulose fibres or hollow polyester fibre which

retains the unwanted waste material from patient’s blood.

� Market size have been growing at an annual rate of 5 to 10 % and accordingly, the

market size of artificial kidneys is estimated to increase from Rs. 13.00 Crore in

2003-04 to Rs. 17.00 Crore by 2007-082.

� Artificial liver

� Made with hollow viscose. It separates and disposes patient's plasma and will

supply fresh.

� Artificial heart

� It is an 8 - ounce plastic pump lined with dacron velour to reduce damage to

blood, about the size of a human heart.

� Silastic backing makes the fabric impervious to emerging gas that is not desirable

in the blood.

� Mechanical lung

� Made with hollow polypropylene fibre or a hollow silicone membrane.

� Used to remove carbon-di-oxide from patient's blood and supply fresh oxygen to

the heart.

4. Healthcare and hygiene products

� Healthcare textiles comprise surgical clothing (gowns, caps, masks, uniforms,

etc), surgical covers (drapes, covers, etc), beddings (sheets, blankets, pillow cases

etc) and disposables.

� Disposable health care textiles would increase from Rs.11 Crore in 2003-04 to

Rs.120 Crore in 2007-20082.

� The most important consumer disposable markets are hygiene and medical

disposable products. J&J, K-C and P&G dominate the hygiene market. The

disposable surplus is increasing rapidly and the disposables market in India is

expected to grow at 10-15%14.

� Sanitary Napkins

� The market penetration of feminine hygiene is only 15% and sales are mainly in the

urban areas. With more than 250 million eligible customers of hygiene products,

India will be a huge market when penetration reaches 60%14.

� A typical potential estimate based on per capita consumption of 50 units per annum

by the eligible population of nearly 300 million users (age group 15 to 40) will give a

theoretical total market size of approximately 15 billion pieces. This could result in

market sale value of $1.5 billion at 10 cents per piece12.

� Market size and Potential is Rs. 336 Crore (2001-2002), 430 Crore (2003-04) and

699 Crore (2007-08)2.

� The market size of sanitary napkins in terms of volume is expected to increase from

1228 mn numbers with a value of Rs. 420 Crore (2003-04) to 1997.44 mn numbers

with a value of Rs. 699 Crore (2007-08)2.

� The sales of healthcare and hygiene products by Procter and Gamble is Rs. 55

Crore ore for the year 2002-2003. Its feminine hygiene brand ‘Whisper Ultra’

continues to grow year after year and has recorded a 30% growth.

� Incontinence (Adult) Diapers

� The problems of incontinence associated with old age are less of a concern in India

so the adult diaper market at present and in the immediate future is not expected to

be big14.

� Market size and Potential is Rs. 20 Crore (2001-2002), Rs. 54 Crore (2003-2004)

and Rs. 135 Crore (2007-2008)2.

� Baby Diapers

� If 25% of infants (out of some 24 million babies born in India every year) in the

period between birth and 24 months, use atleast 28 diapers a week, the number of

baby diapers is estimated to be 8.7 million pieces per year14.

� Among the disposable markets, the diaper market is still in its infancy in India and

they are mostly imported or repackaged in the country. Pampers and huggies are

already present in the market, but the pricing is at a level where it cannot stimulate

demand.

� Market size is Rs 70 Crore Pa (2001-02) at estimated growth rate of around 5-10%

� (average:7%)2. The value of market is expected to be Rs. 105.05 Crore in 2007-082.

� Wipes

� Total market size comprising domestic, medical, cosmetic and baby wipes is Rs.450

Crore 2.

� The consumer wipes market is negligible at present. Premoistened baby wipes have

been recently introduced and have become popular in urban settings. The market is

expected to grow 10-15%14.

� Medical Gauzes

� Gauze is an open weave absorbent fabric used for the treatment of burns and scalds.

� Market potential spun lace based guazes will be Rs.39.5 Crore (approx. Rs.40 Crore)

by 2007-08.

� Surgical gowns

� Surgical gowns are garments worn during medical procedures as uniform of the

surgical team to prevent contamination by splattering of body fluids such as

blood, respiratory secretions, vomit, urine or feces during medical procedures.

� Surgical gowns are made of fluid-resistant materials to reduce the transfer of body

fluids.

� Isolation gowns are usually intended to protect the wearer from the transfer of

micro organisms and only small amounts of body fluids.

� Wound dressing

Wound dressings act as Physical barrier for wounds and are found to have some

distinguished properties like fluid control, odour management, microbial control and wound

healing acceleration. Market Size of woundcare products is Rs. 158 Crore (2001-02)2.

Among which,

� Wound contact layer/absorbent pad/base material-85%

� Anticipated growth- more than 5-10%p.a (Avg. 7.5%)

� Cotton and guaze were the important products of traditional woundcare

segment of India with swabs, dressings and tapes making up the rest.

� Advanced wound care products like hydrocolloid, hydrogel, alginate, foam,

film and tulle dressings are high cost-low benefit products and hence

contribute only small % to the Indian wound care market12.

� Organised companies in wound care market are 3M, beirsdorf, Casil Health

products, Johnson & Johnson, Elder Hartmann and Smith & Nephew. Large

whole salers are involved in the supplying of international brands like

Coloplast, Convatec, Hollister, Lohmann & Rauscher and Molnlycke.

However the number and values of unorganized players in this sector are very

significant.

Classification of wound dressings

Different types of wounds and the different stages of a healing wound require different

dressings or combinations of dressings. Table-10 shows suitable dressings for particular

wound types.

Proposed standard test methods for medical textiles*

Given below are the tests for most commonly used non-implantable, health care hygiene care products basically used in the hospitals in India. We have not included Hygiene Products like Sanitary Napkins & Diapers due to their proprietary designs and limited usage in rural areas. These tests are based either on 1. Indian pharmacopoeia (IP) 2. Drug & Cosmetic rules (D&C) 3. British Pharmacopoeia (BP) 4. US Pharmacopoeia (USP) and Bureau of Indian standards (BIS). Sterility test can be conducted at any public testing laboratory like Geochem (Mulund), SGS (Chennai), ITA (Mumbai) or M.K. Rangnekar lab, Bombay College of Pharmacy, Kalina, Mumbai or Sree Chitra Tirunal Institute for Medical Sciences and Technology, Thiruvananthapuram, Kerala. Test for skin sensitivity can be done at KEM Hospital or Jay Research Foundation, Daman or Sree Chitra Tirunal Institute for Medical Sciences and Technology, Thiruvananthapuram, Kerala. Physical tests and other tests on fabrics like Tensile strength; Adhesion to steel & stretch can be conducted on Tensile testing m/c at laboratories like Indian Institute of Packaging or Textile Research Association with accreditation to ISO 17025 standard. Test for Biocompatibility can be conducted at National Institute for Toxicology, Pune or Sree Chitra Tirunal Institute for Medical Sciences and Technology, Thiruvananthapuram, Kerala. Biocompatibility is referred to reaction of textile with blood & tissue of body. An implantable device has more potential for reaction than an external device and is therefore subject to tighter regulations. Microbiological test for determination of barrier properties of Mask & Gown can be conducted at BTRA or Rnd Bio, Coimbatore 641 015.
