1) Please REMOVE the following Site under condition 10, part G of our Radioactive Material License:

34 Dunnam Road Bridgeport West Virginia 26330

A sketched map of the Close-Out Survey of our Bridgeport, West Virginia facility has been provided detailing surrounding roads and buildings, showing surveys taken in mR/hr (Attachment # 1 ).

A current leak test certificates along with forms 117 showing Transfer of Radioactive Material for Sealed Source SIN C-7 have been provided. (Attachments # 2).

In addition all radiation signs I postings have been removed from the Bridgeport, West Virginia site.

The original and one copy of the amendment request with pertinent information have been submitted for review as required by the NRC Radiation Management Section.
