In order to address the presence of volatile organic compounds (VOCs), primarily Tetrachloroethene (PCE) and Trichloroethene (TCE) in the subsurface, Mullins Rubber Products has:
Implemented the pilot-scale Soil Vapor Extraction (SVE) system to treat VOCs in unsaturatedsoils in January 2016;
Based on successful implementation of the pilot-scale SVE system and the results of the May 2016Supplemental Source Area Investigation to evaluate other potential source areas, expanded theSVE system to include two additional recovery wells in January 2017; and,
Installed an emulsified zero valent iron (eZVI) treatment zone along the western propertyboundary in January 2017 to address impacted groundwater before it leaves the property.
Summary of Activities
This report details the operations and maintenance (O&M) activities performed at the Mullins Rubber Products facility at 2949 Valley Pike in Dayton, Ohio on December 10, 2019. This visit marks the final quarterly O&M visit to the site in 2019.
The O&M tasks performed in Q4 2019 include the following:
Collection of one blower influent vapor sample from the SVE treatment system. The SVE systemlayout is presented in Figure 1. Laboratory results of the vapor sampling are summarized inTable 1. Complete laboratory analytical reports are included in Attachment A.

Recording of system measurements at all SVE monitoring points, SVE wells, and the SVE blower as summarized in Attachment B. The parameters recorded at these locations included:
— Air flow
— Temperature
— Vacuum
Inspection of the SVE blower and routine maintenance as needed (e.g., change oil, add grease for lubrication, empty condensate collection tank).
Collection of water levels from onsite monitoring wells MW-EPA-08, MW-01R, MW-02, MW-EPA-07, and MW-PW; as well as offsite monitoring wells MW-EPA-14 and MW-04. A summary of the water level data collected from the accessible monitoring wells during December 2019 is included in Table 2.
Collection of groundwater samples from monitoring wells MW-EPA-08, MW-EPA-14, and MW-04. Laboratory results are summarized in Table 3 and the complete laboratory analytical report is included in Attachment A. Groundwater parameters collected during field sampling are included in Attachment C.
SVE System Performance
Prior to the October 2018 monthly O&M visit, the two granular activated carbon (GAC) vessels were disconnected from the SVE treatment system due to carbon saturation. With the carbon vessels no longer connected to the SVE treatment system, TRC has been monitoring the blower influent vapor streams as of October 2018. Table 1 shows the complete analytical laboratory results of VOC concentrations measured during O&M visits, as well as calculated values reflecting the daily amounts of VOCs collected from the subsurface and the amount discharged into the atmosphere.
The amount of VOCs discharged daily to the atmosphere are required to be below 10 pounds per day per the de Minimis source exemption specified in Ohio Administration Code (OAC) 3745-15-05. Currently, calculated discharge rates to the atmosphere continue to be below the discharge limit, as observed in December 2019 to be at 0.0036 pounds per day. TRC will continue to monitor vapor discharge levels on a quarterly basis throughout 2020. If emission levels approach 25% of the OAC exemption limit at any time in the future, the SVE system will be shut down to replace carbon and put the GAC vessels back on line.
Table 1 also includes the cumulative total mass of VOCs recovered during the lifespan of the SVE system. To date, the SVE treatment system has recovered 964.95 pounds of VOCs. The complete analytical laboratory report from the most recent vapor sampling event in December can be found in Attachment A.

Figure 2 presents the vapor influent sample PCE and TCE concentrations over the course of the SVE treatment system’s lifespan. Start-up dates for the SVE Pilot Test including the original three (3) recovery wells and the full scale SVE system expanded to five (5) recovery wells are indicated in the figure to demonstrate the effect of these events on system performance. Figure 2 generally shows a steady decrease in recovery of PCE and TCE recovery over time.
Field measurements of the SVE treatment system are recorded quarterly to gauge the effectiveness and efficiency of the overall system. These measurements are recorded on O&M log forms that are kept on record by TRC. Attachment B summarizes the quarterly SVE O&M measurements. Based on these quarterly data, vacuum continues to propagate through each SVE monitoring point in the subsurface. These vacuum measurements are reflected in the radii of influence shown in Figure 1.
Groundwater Conditions
Static water elevations collected from the six onsite wells and two offsite wells and their resulting groundwater elevations are summarized in Table 2. Groundwater elevation data was used to develop an updated groundwater flow map as reflected in Figure 3. Groundwater flow is westerly from the source area though the eZVI groundwater treatment zone depicted in Figure 4. The historic water level data has shown that groundwater elevation remains relatively consistent throughout the year, corresponding with a westerly flow direction.
Groundwater samples were collected from monitoring wells MW-04, MW-EPA-14, and MW-EPA-08 during the O&M visit on December 10, 2019. Groundwater sampling results from these three wells are shown in Table 3 and the associated Mann-Kendall trend analyses graphs. Groundwater collection parameters for each well are shown in Attachment C.
Results of the most recent groundwater sampling event show that PCE levels in MW-EPA-08 and MW-EPA-14 have increased since these monitoring wells were last sampled in September 2019. This increase is not consistent with recent historic decreases in PCE levels in these monitoring wells as noted in Table 3 and the attached Mann-Kendall graphs. Data from the next scheduled sampling event in March 2020 will be evaluated to assess if this increase in PCE levels is an anomaly or indication of a new upward trend. TCE and cis-1,2 Dichloroethane levels above laboratory quantitation limits were also reported for the first time in MW-EPA-14. These are likely to be breakdown products of PCE de-chlorination. PCE levels in monitoring well MW-04 remain consistent with historical data.
Projected Work for the Next Reporting Periods TRC is scheduled to be onsite again in March 2020 to perform the next quarterly (Q1 2020) O&M
site visit. TRC will coordinate with Mullins Rubber at least 2 weeks prior to visiting the site to perform the Q1 2020 O&M activities. TRC will continue to monitor SVE emission levels via

vapor sampling of the blower influent stream. Groundwater samples will also be collected from monitoring wells MW-04, MW-EPA-14, and MW-EPA-08 during the upcoming O&M visit.

Air - GC/MS VOA Methods TO 15 LL, TO-14A, TO-15: EPA methods TO-14A and TO-15 specify the use of humidified "zero air" as the blank reagent for canister cleaning, instrument calibration and sample analysis. Ultra-high purity humidified nitrogen from a cryogenic reservoir is used in
place of "zero air" by Eurofins TestAmerica Knoxville.
No additional analytical or quality issues were noted, other than those described above or in the Definitions/Glossary page.

With the exceptions noted as flags or footnotes, standard analytical protocols were followed in the analysis of the samples and no problems were encountered or anomalies observed. In addition all laboratory quality control samples were within established control limits, with any exceptions noted below. Each sample was analyzed to achieve the lowest possible reporting limit within the constraints of
the method. In some cases, due to interference or analytes present at high concentrations, samples were diluted. For diluted samples, the reporting limits are adjusted relative to the dilution required.
Calculations are performed before rounding to avoid round-off errors in calculated results.
All holding times were met and proper preservation noted for the methods performed on these samples, unless otherwise detailed in the individual sections below.
Eurofins TestAmerica utilizes USEPA approved methods, where applicable, in all analytical work. The samples presented in this report were analyzed for the parameter(s) listed on the analytical methods summary page in accordance with the method(s) indicated and were
analyzed in accordance with Ohio Voluntary Action Program protocols, where applicable. The following requested analytes, parameter groups or methods analyzed and contained in this report are not certified by the laboratory: The RSK_175_CO2 Carbon Dioxide analysis was performed at the Eurofins TestAmerica, Burlington laboratory, RSK_175 Dissolved Gases, 9060A TOC, and 4500_Cl_G Residual Chlorine.
A summary of QC data for these analyses is included at the back of the report.
Eurofins TestAmerica, Canton attests to the validity of the laboratory data generated by Eurofins TestAmerica facilities reported herein. All analyses performed by Eurofins TestAmerica facilities were done using established laboratory SOPs that incorporate QA/QC procedures described in the applicable methods. Eurofins TestAmerica's operations groups have reviewed the data for compliance with the
laboratory QA/QC plan, and data have been found to be compliant with laboratory protocols unless otherwise noted below.
All solid sample results are reported on an "as received" basis unless otherwise indicated by the presence of a % solids value in the
method header.
This laboratory report is confidential and is intended for the sole use of Eurofins TestAmerica and its client.
RECEIPT
The samples were received on 12/13/2019 8:00 AM; the samples arrived in good condition, properly preserved and, where required, on ice. The temperature of the cooler at receipt was 1.5º C.
VOLATILE ORGANIC COMPOUNDS (GCMS)
Samples MW-04_20191210 (240-123784-1), MW-EPA-14_20191210 (240-123784-2), MW-EPA-08_20191210 (240-123784-3),
MW-EPA-07_20191210 (240-123784-4) and TB-01_20191210 (240-123784-5) were analyzed for volatile organic compounds (GCMS) in accordance with EPA SW-846 Method 8260B. The samples were analyzed on 12/23/2019 and 12/24/2019.
Trichloroethene failed the recovery criteria high for the MS of sample 240-123690-12 in batch 240-416879. Refer to the QC report for details.
