SECTION B SETTING THE LEARNING AND PERFORMANCE OBJECTIVES
The learning and performance objectives shall be agreed upon between the trainee and his/her supervisor.
NB: It is a requirement that this section is completed before the training can be processed. Indicate the specific areas where the learning from the training will be applied. This should include setting of Performance targets based on the learning objectives. The agreed targets should form the basis for evaluation in the following sections of this report.

SECTION C ACHIEVEMENT OF THE OBJECTIVES
To be completed by the trainee within seven days after the training.
Indicate the degree to which the objectives in section B above were met. (Explain any gaps between the stated learning objectives and the actual achievement of the course objectives)

SECTION D EVALUATION BY IMMEDIATE MANAGER/SUPERVISOR
Within 3 6 months after setting the performance objectives, the manager/supervisor should meet with the staff to discuss his/her performance before completing this section. The report should then be forwarded to the Human Resource & Administration Director.
Indicate the achievement of the agreed performance objectives. Any improvements in the performance of the system including tangible benefit, which can be attributed to the training, should be mentioned.
