Loss of Containment– Flanged BoltedConnections

A bolted joint is one of many criticalcomponents of a pressurisedsystem. Dependent upon the contents,pressure, location and purpose of thesystem, leakage or failure of a singlebolted joint, can have potentiallycatastrophic consequences. To meetthis challenge, every operator ofpressurised systems must have in placea system to positively and pro-activelymanage the integrity of bolted joints.It is expected that such a system will be part of an asset and/or processintegrity management system, and bebuilt around the principle of continuousimprovement, based on lessonslearned. The duty holder should ensure thatthe integrity of bolted pipe jointsover the whole life of the plant isaddressed within the managementsystem. This should be achieved as a matter of management policy, byspecifically identifying management ofbolted pipe joints within the engineeringdesign, construction, maintenance andoperation standards and procedures. Control of the Training andCompetence Assurance ofall personnel working onBolted Flanged Connectionsis a critical factor inachieving and maintainingLeak Free Performance.

Within alignment tolerances, existingcontrols are in place, eg procedures,vendor information, competent personnel;safe to proceed.

Additional controls need to be identified,consider the use of specialist contractors,limit personnel in the area during testingand initial operation, improve access,re-check after completion.

Engineering review of piping designand configuration, consider the use of a specialist contractor or vendor.

A procedure will be developed andimplemented to ensure that individualjoint status is displayed visually onsite, by use of a multi-part taggingsystem. This will include a focus ontemporary joints and joints requiringreinstatement after testing orcomponent replacement. Includingcontractor activity.

To ensure the optimum level ofresources and verification is carried outon each joint, a criticality assessmentwill be carried out, which will considerthe service, pressure and localconditions of each connection.
