Rebate applies to purchases made between 5/19/15 to 8/14/15.

Please follow these instructions completely and accurately in order to qualify for the rebate(s).

To receive a rebate by mail from Torklift International you MUST complete the following steps:

1.) Submit a guess on the Mystery Product between 4/29/15 – 5/18/15

2.) Pre-order or purchase a qualifying Torklift GlowStep Revolution between 5/19/15 and 8/14/15

3.) All documents must be submitted at the same time.

4.) This offer is good only in the United States and Canada.

5.) Rebate(s) are only issued in U.S. funds.

6.) Qualifying complete GlowStep Revolution part numbers include: A8002, A8003, A8004, A8005, A8006.

7.) Limit of one $10 GlowStep Revolution rebate per household or address. Retail consumer offer only. Can’t be combined with any other offer.

• Fully completed mail-in rebate form. (see form below)

• Warranty registration form.

• A copy of your original, dated sales receipt(s) showing the Torklift International GlowStep Revolution purchase.

• Original UPC (see example below) cut off the box(es)* from the original

GlowStep Revolution package. Do not send the shipping label — this will not be accepted.

• All items above must be included with appropriate postage postmarked by 8/15/15 in order for rebate to be honored.

The UPC code is located on the side panel of the Torklift International GlowStep Revolution packaging and will have a part number, barcode and Torklift logo. Please cut out and send the entire label as shown below. Do not send the shipping label – that will not be accepted.
