1 Job assigned in a vessel that requires the vessel to be entered (definition of confined space).
2 Vessel is taken out of service
3 Hydrocarbon free – washed, purged, steam – depending on the service
4 LOTO
5 Install safety blinds – Safe Work Permit
6 Open Manways – Safe work permit
7 Issue Confined Space work permit – prior to breaking plane of entry.• Copy of the confined space permit• Atmospheric Testing. – if does not meet criteria – may required additional washing or other
steps. • Different services require different tests (need a table of tests required)• JSA on the area• Safety blinds MUST be verified prior to breaking the plane• Install ventilation for system (cannot be running during testing) • Certified confined space attendant – wallet card – certification by company (get outline for

8 Begin monitoring Confined space entry and area by Confined Space attendanta. Entry Log – sign in everyone who enters vessel (when enters, when ends)b. Continuous monitoring of atmosphere (O2 toxic gas – LEL (any hydrocarbon)c. Air horn and vest to identify persond. (Duties of Confined Space Entry attendant – need outline)e. Any atmospheric “alarm” – the CSE attendant will
i. sound horn and ii. evacuate vesseliii. ensure on entry log that everyone is out. iv. Pertains to all entries to the common vesselv. Entry personnel must not enter at one manway and exit at another – to
ensure complete tracking of personnel. vi. All openings must be monitored by a Confined Space Attendant.
f. May only be relieved by another certified Confined Space Attendant – relief is noted in log.
