1.1 Purpose of Survey

Adams Environmental Ltd were instructed by the Client, Mr Brian Stamp, North East London NHS Foundation Trust, to carry out a survey at Thorpe Coombe Hospital, Forest Road, Walthamstow, London, E17 3HP. Buildings for inspection being Wood House, Family Consultation Block (CAMHS), Larkswood Centre, Psychotherapy Clinic and Thorpe Coombe House. The purpose of the re-inspection survey was to collate, confirm and update existing record information relating specifically to known and/or suspected ACMs.

Previous survey and reporting of ACMs has been carried out at Thorpe Coombe Hospital, by Adams Environmental Ltd, as detailed in the Survey Particulars (see 1.2 below).

This Asbestos Re-inspection Survey Report includes information on the location, extent, condition and type of asbestos material identified or suspected, together with comment and recommendations for management of identified asbestos materials, based on the requirements and occupation of the site.

This Report & Register supersedes and replaces the previous Report(s). It is recommended that all previous records are kept and appropriately archived by the Client.

1.3 Methodology

1.3.1 Site Re-Inspection The Asbestos Register details the locations of positively identified and suspected Asbestos containing

materials (ACMs) and any specific areas of ‘no access’. Within it, data is recorded for type and extent of building component, type of asbestos fibres, with comments on condition, surface treatment and recommendations.

Inspection consisted of re-visiting all specific locations where asbestos occurrences have previously been identified / suspected and recorded in the existing site Asbestos Register information. Comments on condition, surface treatment, and recommendations for remedial action or management, are updated where appropriate, including information where asbestos removal / remedial works have been carried out. The updated Register forms Appendix I of this Report.

1.3.2 Sample Analysis of ACMs Analysis results of sampled materials have been carried forward from the previous survey inspection. No

further sampling has been carried out as part of this re-inspection.

Original sample analysis results, taken as part of the original site inspection, are reproduced at Appendix II of this Report.

Details of sampling and analysis techniques are as documented in the original survey report.

1.3.3 Plans Plans with this Report form Appendix III. These are based on original supplied plans, suitably updated.

Plans are colour-highlighted and annotated to indicate asbestos occurrences.

1.3.4 Photographs Photographs providing an illustration of the asbestos occurrences present at the site are included within

Appendix IV.

1.4 Extent of Survey

Inspection has been made specifically to locations where ACMs have been previously identified as being present, as recorded in the previous survey documentation. Where materials were recorded as ‘non-asbestos’ or where locations were recorded as ‘inspected no asbestos identified’, these statements have been reproduced directly and have not been subject to any further re-inspection / sampling, etc.

The following areas of the site/building were not accessed for the reasons stated:

o Larkswood Centre Ground Floor – No key access to understairs store. o Larkswood Centre 3rd Floor – No key access to walk in cupboard within Office 3.10. o Rodway & Bryson Wards – Buildings found to be demolished at time of December 2017 re-inspection. o Thorpe Coombe House – Ground floor roofspace not accessed (hatch located over server equipment)

1.5 Survey Re-Inspection Parameters

Within an individual location requiring re-inspection, parts of the building structure that could be accessed due to its design were done so, i.e. via ceiling voids, service risers, ducts etc. Investigation into the sub-surface structure of the building fabric was not made, other than as described herein.

Re-inspection was not continued where damage was likely to be caused to existing building decorations (e.g. opening up of screwed wood riser cover panels that were heavily over-painted or filled to joints).

Similarly, re-inspection did not proceed where the Surveyor deemed that the opening / manoeuvring / replacing of any access cover, etc. could not be safely performed by the Surveyor.

Where previously identified / suspected asbestos materials would be, or were likely to be disturbed by re-inspection, (i.e. opening up of riser covers where unsealed asbestos materials were known to be present behind, etc.) specific re-inspection was not continued in those areas.

The opening up / dismantling of live (and suspected live) electrical equipment, etc. was not within the scope of works.

Re-inspection was curtailed where site equipment, fittings, heavy furniture or any other item prevented or restricted access. Within a location requiring re-inspection, the removal of stock, stored items or other material that prevented or restricted access was not carried out. Where the Client was able to remove such items to facilitate access at the allocated time of inspection, survey was performed.

Equipment, machinery, ducting, etc., was not moved, opened up or examined. Floor finishes (e.g. linoleum and carpets) were not taken up.

Internal building manhole covers believed to be part of the drainage system were not accessed. Other floor duct covers within the building requiring re-inspection were accessed provided that they were free to move and did not require specialist keys or lifting equipment.

Ladders were supplied in order to access heights not exceeding 3.0m for visual re-inspection. Re-inspection of building finishes, surfaces, ceiling voids etc. in excess of 3.0m above clear floor level was not carried out unless means of safe access was provided by the Client at the allocated time of site re-inspection.

NOTE: In the absence of any confirmed record of identified asbestos material to a particular location, it should

not be assumed that asbestos materials will not be encountered in or adjacent that location as part of refurbishment or building maintenance works. Such works should be carried out with caution, given the knowledge of asbestos uses that have been identified on the site.

Particular difficulties are associated with areas where ad-hoc alteration and refurbishment have previously been carried out, and where asbestos may be hidden behind cladding materials. Asbestos is also frequently concealed within the fabric of buildings within sealed voids, as shuttering etc. It is therefore possible that further asbestos containing materials may be found, particularly during electrical rewiring, heating installations and other refurbishments or demolition works.

Adams Environmental Ltd do not accept responsibility for any damage to the building fabric arising from survey inspections, or for any asbestos materials found at a later date which are not specifically detailed in the supplied Report & Register.

The following details the general asbestos occurrences that have been identified during the inspection. Where materials have not been positively identified but are suspected to be present, these are noted. Asbestos products are listed from high friable potential material type to low friable potential material type. 2.1. Lagging (Thermal Insulation)

Typical historical composition and uses

The term ‘lagging’ covers a wide range of thermal insulation materials, including sectional and composite pipe insulation, insulation to boilers and pressure vessels, slabs, blankets, quilts, asbestos paper, felts, plastered applications, together with ‘loose fill’ insulation types. Variable asbestos contents can be encountered. Examples include: 70% asbestos fibre in calcium silicate slabs, 20% in magnesium carbonate insulation, 10-15% in magnesia, calcium silicate and diatomite. Blankets, mattresses, loose fills, papers and felts can approach 100% by volume. All types of asbestos fibres have been used in lagging applications. Typical uses include insulation of boilers, pressure vessels, calorifiers and pipework associated with heating systems. ‘Loose fill’ asbestos was sometimes used as loft insulation, packing around cables between floors. Bagged ‘loose fill’ asbestos material can be found as sound proofing under floors and in walls. The likelihood of fibre release from lagging depends on its composition, friability, state of repair and surface finish. It is particularly susceptible to damage or disturbance during maintenance, or because of leaks from pipes and boilers. ‘Loose fill’ and unsealed materials may readily become airborne if disturbed. Asbestos Licensed Contractor required if any works on material required. 14 day notification to local authority or HSE prior to any major works on material. Locations and uses: CAMHS Family Consultation Block - Basement – Pipe insulation and debris to boiler room duct. Duct running off stairwell in basement area. Insulation debris to floor of duct. (Access point to duct now

sealed). Recommendation: Manage in situ.

2.2. Asbestos Cement (AC) Products Typical historical composition and uses

Asbestos Cement (AC) was widely used in many building types. Uses included profiled sheets, forming roofing, wall cladding and weather-boarding. Semi and fully compressed flat sheets and partition boards, forming partitions, shuttering, decorative panels, bath panels, soffits, wall and ceiling linings, portable buildings, fire surrounds and composite panels for fire protection. Tiles and slates, used as cladding, decking, promenade tiles and roofing. Pre-formed moulded products including cisterns and tanks, drains, sewer pipes, rainwater goods, flue pipes, roofing components, cable troughs, ventilators and ducts, window boxes. AC products generally contain 10-15% of asbestos fibre bound in a matrix of Portland Cement or autoclaved calcium silicate. All types of asbestos have been used in these materials, the most common being Chrysotile (white asbestos), though Crocidolite (blue) and /or Amosite (brown) can be found. Fibres are firmly bonded and only likely to be released if the material is mechanically damaged or aged, deteriorated or decomposed. Abrasion, handsawing or use of power tools is likely to result in significant fibre release. Locations and uses: Wood House (former Children’s Centre) - External - Front entrance canopy soffit panels, cladding

panels to front & rear end elevations of ‘domed’ roof building. Wood House (former Children’s Centre) - External – Roof tiles to rear extension building. CAMHS Family Consultation Block – Basement – Shuttering and upstand within sealed duct.

Recommendation: AC materials as detailed above; manage in situ.

2.3. Textured Coatings and Paints

Typical historical composition and uses

Used as decorative applied coatings to walls and ceilings. Asbestos was used in small quantities within ‘dry mix’ textured coatings or paints such as ‘Artex’. Approximately 1% to 5% asbestos was used in these materials and fibres would have been released when the ‘dry mix’ materials were prepared. The material is not homogeneous, i.e. asbestos fibres may not be uniformly distributed within the applied coating. Use of such material was extensive through 1960’s to mid 1980’s. In-situ, fibres are well contained in the matrix but release is likely if the existing coating is sanded down or scraped off dry. Locations and uses: Wood House (former Children’s Centre) – Ground Floor – ‘Artex’ type decorative coating to

original ceilings and walls above dropped plaster ceilings. Recommendation: Manage in situ.

2.4. Compressed Asbestos Fibre (CAF) Gaskets Typical historical composition and uses

CAF materials contain asbestos fibres bonded with specially selected polymers suitable for pressures up to 2000 psi and temperatures up to 600 0C. Gaskets have asbestos fibre content approaching 100% Chrysotile (white asbestos). Such gaskets are required where two metal surfaces are joined, to allow easy future separation. CAF materials are used to seal pipework flanges, boilers and other industrial plant. The gaskets may be pre-shaped or cut from large CAF sheets. Fibres can be released when the material is cut, abraded or torn during gasket removal or gasket shaping from sheet materials. Locations and uses: Thorpe Coombe House – Basement – Gaskets to pipe flanges.

3.1 Management of Asbestos Materials

Regulation 4 of The Control of Asbestos Regulations (2012), ‘The management of asbestos in non-domestic premises’ places a duty on those who have repair and maintenance responsibilities for premises, because of a contract or tenancy, to manage the risk of asbestos in those premises. Where there is no specific contract or tenancy the person who is deemed to be in control of building maintenance & repair is regarded as the ‘duty holder’. The ‘duty holder’ is required to:

1. Take reasonable steps to find materials in premises likely to contain asbestos and check their

condition 2. Presume that materials contain asbestos unless there is strong evidence to suppose they do not 3. Make a written record of the location and condition of asbestos and presumed asbestos-containing

materials and keep such record(s) up to date 4. Assess the risk of the likelihood of anyone being exposed to these materials 5. Prepare a plan to manage the identified risk and put it into effect to ensure that:

a) Any material known or presumed to contain asbestos is kept in a good state of repair. b) Any material that contains or is presumed to contain asbestos is, because of the risks

associated with its location or condition, repaired or if necessary removed. c) Information on the location and condition of the material is given to anyone potentially at risk. d) The condition of identified and presumed asbestos materials is monitored. e) The management plan, including the arrangements made to put it in place, is monitored and

regularly reviewed.

The information relating to identified /suspected asbestos occurrences supplied within this Survey Re-inspection Report will be of assistance to the Duty Holder in complying with the requirements of The Control of Asbestos Regulations 2012. As stipulated in HSG 264, ‘Asbestos: The Survey Guide’, the Asbestos Register contains a Materials Assessment (MA) rating for each identified asbestos occurrence. (Type of asbestos product, asbestos fibre content, condition of the material and presence of a sealant are the assessed factors). This gives initial guidance as to the potential for fibre release should the material become disturbed. Priority Assessments for each identified ACM are also provided in the Register. These have been generated from Client Duty Holder supplied information and are based on ‘human risk’ factors related to the likelihood of person(s) actually disturbing the material and include the location, extent and accessibility of the material, the use of the location and the type, frequency, volume and duration of occupancy for both normal use and maintenance activities. Consideration of these factors allows formal Priority Risk Assessments for each identified asbestos occurrence to be made. HSE document HSG227 gives comprehensive guidance for management requirements of the Duty Holder under the Asbestos Regulations and includes specific recommendations for Priority & Risk Assessments.

Register Glossary

Survey Report No / Issue Date

The Report Number issued by Adams Environmental Ltd is unique to the site. The date of authorisation records the date that the Report was authorised issued by

Adams Environmental Ltd to the Client. The Revision Issue number indicates the re-issue following any update.

Site Identification

The site name, and where appropriate, the relevant building and floor level are indicated.

Location (Area/Room)

The reference code / name of each surveyed building area as found on site or as used on any supplied plans. Where none is present a suitable reference relevant to building and floor level is given at time of site inspection to allow cross-reference between Register and plans.

Sample N° The reference given to the sample when it was taken from the parent material on site, as detailed in Appendix II: Materials Report. The suffix (A) indicates the sample has been taken from this location and analysed. The suffix (M) indicates that the result is mastered from similar analysed material.

Building Component (Product Type)

The most appropriate description of the material as a building component. This may reflect the position of the material rather than its purpose, e.g. an asbestos panel fixed to the rear of a riser access hatch may be termed ‘Door Panel’ as opposed to ‘Fire Protection’.

Asbestos Content

The type of asbestos fibre identified by sampling and analysis. Further details are given within Appendix II: Materials Report. Chrysotile, commonly known as White asbestos Amosite, commonly known as Brown asbestos Crocidolite, commonly known as Blue asbestos Where no asbestos has been detected in the sampled material, this is indicated.

Extent An approximate extent of the material is given in either square or linear metres. The symbol @ is used to denote the extent of each instance of a material where it has been used discretely and severally. These measurements are for guidance only.

Condition Good: No visible damage. Satisfactory: Asbestos is in generally sound condition with no / little exposure noted. Fair: In average condition with minor areas of damage / surface exposure. Poor: The material is in damaged or deteriorated condition and/or in debris form.

Surface Treatment

An indication of the exposure of the surface of the material, relevant to the Product Type. Sealants may be in liquid (e.g. paint encapsulant) or rigid form (e.g. overclad with board). Composite: Materials containing asbestos; reinforced plastics, resins, vinyl tiles, etc Enclosed: The asbestos material is sealed by a protection greater than paint application

alone. Sealed: The asbestos material is sealed by paint or other similar encapsulant. Partially Sealed: Sealant is present but does not completely cover the material or is

deteriorating. Unsealed: The material has not been sealed, and the surface is exposed.

Material Assessment

The numerical score given for each identified asbestos occurrence is derived from the application of a material assessment algorithm. The Materials Assessment (MA) is generated by scoring Type, Condition, Surface Treatment and Asbestos Fibre Type for each asbestos occurrence. Scores (0, 1, 2 or 3) are given for each parameter and then totalled to give a final score out of 12. This algorithm is based on parameters described in HSG 264 and Adams Environmental Ltd’s documented in-house procedures.

(The use of the Materials Assessment by the Client as the basis for risk assessment is described further in Part 3 of this Report).

Accessibility Direct access: The material can be directly accessed within the location, i.e. an AIB panel fitted to the rear face of a fire door / asbestos lagging to pipework attached to a boiler.

Improvement works required - Given when an asbestos material in other than satisfactory condition is found and given its location, requires remedial works to be carried out to place it in satisfactory condition. Where asbestos materials are to remain in-situ following identification, they should be maintained in / placed into a sound, sealed condition, undamaged, not releasing dust and should not be disturbed. This may be achieved by carrying out appropriate repair, encapsulation, protection works, etc. or by placing appropriate restrictions on the access / use of the location where the material is present.

Monitor material condition - Given when an asbestos material has been identified in

satisfactory condition at the time of site inspection. Where such materials are to remain in-situ, monitoring to confirm that satisfactory conditions are being maintained is required. This would normally involve site re-inspection by a competent person and updating / recording of results, etc. The timescale of re-inspections will be determined by the likelihood of the material condition changing, given factors surrounding it. These will include the physical location of the asbestos material and the likelihood of its being disturbed.

Programme further investigation - Given when asbestos materials are suspected to be

present but, within the scope and parameters of the inspection carried out, have not been positively identified, or for locations where access could not be gained at the time of site inspection.

Further to the material assessment recorded for positively identified asbestos occurrences (as detailed above), the priority assessment algorithm is based on parameters described in HSE Guidance ‘HSG 227 – A Comprehensive Guide to Managing Asbestos in Premises (2002)’. Information fields, as described below, are collated. This requires scrutiny from the Client to confirm accurate data. Four fields are each scored out of 4 parameters (0, 1, 2, or 3), (averaged when appropriate) to derive a total score out of 12, to then be used with the material assessment score to generate a Risk Assessment.

Likelihood of Disturbance Location The size of location within which the material exists, i.e. external, internal; large room, small room,

confined space, etc. Material Accessibility

The ease with which the material, given its location, can be accessed and therefore disturbed, i.e. unlikely to be disturbed, occasionally likely to be disturbed, easily disturbed, routinely disturbed. For example: Within a ceiling void – unlikely to be disturbed. Panelling to a corridor fire door – routinely disturbed.

Material Extent

The volume of the material within the recorded location.

Normal Occupant Activity Main Type of Activity

A rating of the likelihood of the main activity that is being carried out in the location where the asbestos material is present having some incidental affect on the asbestos. For example: Asbestos present in a disused store room – rare disturbance activity. Asbestos present to a fire door in a heavily trafficked corridor – high disturbance activity.

Secondary Activities

A rating of the likelihood of secondary activities that are carried out in the location where the asbestos material is present having some incidental affect on the asbestos.

Human Exposure PotentialNumber of Occupants

The maximum number of occupants considered likely to occupy the location where the asbestos material is present at any one time.

Frequency of Use

The frequency of use of the location in which the asbestos is present, i.e. daily, weekly, monthly, etc.

Time in Area The maximum period of continuous time likely to be spent in the location by the occupants, i.e. less than 1 hour, between 1 & 6 hours, etc.

Maintenance Activity Frequency of Maintenance

The frequency of maintenance or other similar likely disturbance activities that may affect the asbestos material. For example: Unlikely to be disturbed, likely to be disturbed less than once per year, more than once per year, more than once a month.

Maintenance Activity

A rating of the likelihood of the maintenance activity that is carried out in the location where the asbestos material is present having some direct affect on the asbestos. For example: Minor disturbance likely – i.e. a possibility of contact when gaining access. Low disturbance – i.e. changing lightbulbs only to inset lighting in an asbestos panelled ceiling. High disturbance – i.e. needing to remove asbestos panels to gain access to drainage pipe rodding positions.

Risk Assessment Score

The risk assessment score is derived from the Materials Assessment & Priority Assessment ratings that have been collated and combined. Scores of 20 -24 are regarded as having a VERY HIGH risk rating. Scores of 15-19 are regarded as having HIGH risk rating. Scores of 11-14 are regarded as MEDIUM risk rating, scores 6-10 LOW risk rating and scores of 5 or less VERY LOW risk rating.
