BPD with noticeable improvements specifically in impulsivity, affect instability, feelings of emptiness and psychosis when used short term.

o The potential effects of concurrent medications on observed symptom improvement cannot be ignored.

• Paliperidone has an effect on a broad range of BPD symptoms/characteristics. • Specific effects of paliperidone were observed on impulsivity, anger outbursts, cognitive

Authors’ Conclusions

• Quetiapine produced a significant reduction in impulsivity and affective symptoms. • Quetiapine decreased motor, attentional and cognitive symptoms of impulsivity. • Quetiapine had a, “mood stabilizing effect,” as seen by fewer reports of mood shifts

between anxiety, anger, elation, depression, and normal mood. • Patients had improvement in good choices and less interference on neuropsychological

testing, a validated measure of impulsivity. • Quetiapine should be considered in patients with affective and impulsivity issues.

9. Conclusions

A. There is some weak evidence for use of atypical antipsychotics in the short-term (8-12 weeks) management of BPD.

B. Atypical antipsychotics appear effective in reducing specific behavioral symptoms of BPD that lead to problems with personal relationships, legal trouble and most often hospitalization in short-term use.

C. The risk of side effects versus the benefit of symptom management is critical to determine whether atypical antipsychotics should be used in the short term for BPD.

D. More research using specific BPD symptom assessments along with standardized outcome measures is needed.

E. More research is needed in order to evaluate the benefits and risks associated with the long-term use of atypical antipsychotics in BPD.
