In compliance with the provisions of RA 67L3, otherwise known as the "Code of Conduct and

Ethical Standards for Public Officials and Employees" and Civil Seruice Commission (CSC)

Circular No. 3, s. 2015, all PSHS System - OED officials and employees are directed to file /submit their Statement of Assets, Liabilities and Net wofth (SALN) for the year 2019 on orbefore June 30, 2020.

Relative thereto, a Review and Compliance Committee and Secretariat is hereby created to be

composed of the following personnel:

Establish procedures for the review of the submitted Statement of Assets, Liabilitiesand Net worth (SALN), as to its completeness;

2. In the event that an official or employee of this office has not filed his / her SALN on

time, said Committee shall have the authority to take the necessary action in

accordance with the Civil Seruice Commission laws, rules and regulations; and

3. Ensure that the SALN Filers must attach and submit required documents.

Moreover, the secretariat shall render seruices to the Review and Compliance Committee and

handle the safekeeping and systematics filling of the SALN documents.

This Order shall take effect immediately and shall remain in force until revoked or amendedby the appropriate authority.

ln the exigency of public service and in compliance with government auditing rules, the followingpersonnel shallcompose the SALN Review and Compliance Committee for the Campus:

The SALN Review and Compliance Committee shall perform the fotlowing functions:1. Review SALN and determine whether these were properly accomplished

2. Submit a list of PSHS-CARC employees who: (i) filed their SALNs with complete data; (ii) filed

their SALNs but with incomplete data; and (iii) did not file their SALNs to the Head of Office,

copy furnished the CSC, on or before May 15 of every year;

3. Transmit all original copies of the SAl.-ils received, on or before June ll0 of every year to the

concerned offices;

4. Resolve issues pertaining to compliance with the submission and filing of SALNs by all PSHS-

CARC officials and employees;

5. Recommend the procedure to be adopted by PSHS-CARC in providing public access to the

SALN of its officials and employees through process and established best practices; and

6. Perform such other functions as may be necessary to ensure compliance by PSHS-CARC with

all SALN related concerns.

This order shall take effect immediately and will remain enforced until January 13, 2020 unless

earlier revised or revoked.

In the interest of service, the PSHS-CLC Statement of Assets, Liabilities and Net Worth (SALN) Review and Compliance Committee is hereby established and shall composed the following personnel with their corresponding designation:

The Committee shall have the following functions and responsibilities:

Collate and evaluate SALN Forms to determine whether said statements have been properly accomplished;

Be delegated the ministerial duty of the Head of Office to issue Compliance Order as stated in Section 3 of CSC Resolution No. 1300174 dated January 24, 2013 which reads:

"Section 3. Ministerial Duty of the Head of Office to Issue Compliance Order. Within five (5) days from receipt of the aforementioned list and recommendation, it shall be the ministerial duty of the Head of Office to issue an order requiring those who have incomplete data in their SALN to correct/supply the desired information and those who did not file/submit their SALNs to comply within a non-extendible period of thirty (30) days from receipt of the said Order.

Prepare Summary List of Filers (see attached) and issue a Certification (see attached) that all SALN Forms to be submitted are reviewed and found compliant with the guidelines in the filing out and and submission of the said forms; and

4. Submit SALN Forms to the appropriate offices (Office of the President, Office of the Deputy Ombudsman, and Civil Service Commission) on or before June 30 of every year.

This order shall take effect immediately and shall remain valid until revoked.
