“70 male who applied for Medicaid [and] food benefits. He was incarcerated for 26 years and released in 2015. Client worked with Kitty… at the Alpena Area Senior Citizens Council. Client was interviewed by phone call lasting 15 minutes. Another phone call was made the same day to provide Bridge card replacement number, but client preferred to have Kitty help him.”

“Client has a lease which includes electric and water/sewer, pays for propane heat... With client’s permission, called Karen at Northern Financial Enterprises…to verify heat expense since was not stated on the lease.”

“Client received prorated amount of FAP in July of $28 and August 2016 forward received $146.”
