Followinganundergraduatedegreeinphysics,MarkinitiallytrainedinLondonasamedicalphysicist,specialisinginMRI,nuclearmedicine,andradiotherapy.HeundertookresearchintofunctionalMRIandnovelMRsequences,establishingabodyofworkwithinthefieldofparallelimaging.Withanever-growingfascinationforradiology,Markdecidedtoretrainasamedicaldoctor,qualifyingwithdistinctionfrommedicalschool,beforecompletingjuniordoctorjobsinCambridge,atwhichtimehisresearchwasininterventionalneuroradiologyonstentingtreatmentsforbenignintracranialhypertension.
MarkthenmovedtoOxfordafterbeingawardedanNIHRfundedAcademicClinicalFellowship(ACF)inClinicalRadiology.Hiscurrentresearchinterestsareinthefieldofinterventionalradiology;investigatinginnovativeMRIsequencestoquantifyperfusionfollowingangioplasty,theroleofpercutaneousmicrowaveablationinthetreatmentoflung,liverandrenalmalignancies,andtheutilisationofprostaticarteryembolistaioninthetreatmentofbenignprostatichyperplasia.Markhaspresentedhisworkinternationallyandpublishedinpeer-reviewedjournals.

AndrewReillyisaConsultantClinicalScientistintheradiotherapyphysicsdepartmentattheClatterbridgeCancerCentre.Hisprimaryroleistosupporttheclinicaluseanddevelopmentofradiotherapyimagingtechnologiesandworktowardsimprovedsystemsintegration.Hehasaparticularinterestinbridgingthegapbetweendifferentimagingdisciplinesandoptimisingimagingacrosstheradiotherapyprocess.HeisfounderoftheIQWorksproject,leadstheRadiotherapyImagingUserGroupandprovidedphysicssupportunderthenationalNRIGmentoringprogrammeforIGRTimplementation.AndrewservedasChairmanoftheBIRRadiation,PhysicsandDosimetryCommitteeuntil2009,wasamemberofBIRCouncilfrom2010-2013andrepresentstheBIRontheDHMedicalPhysicsExpertworkinggroup.

JamesisaClinicalScientistspecialisinginMedicalPhysicsandNuclearMedicine.JamesstudiedforaPhDinSpacePhysicsatImperialCollegeLondonbeforetransferringtothehealthcarefield.AftercompletingtheNHStrainingschemeforClinicalScientistsatStGeorge’s,theRoyalMarsdenandtheRoyalSurreyCountyHospitals,JamesremainedinGuildfordtotakeapositionintheNuclearMedicinePhysicssection.HisbackgroundinresearchandinterestinteachingledtoaVisitingResearcherpositionatthenearbyUniversityofSurrey.CollaborativeprojectswiththeUniversityeventuallyresultedinasuccessfulapplicationtotheNIHRforapart-timepost-doctoralfellowship.ThisenabledJamestopursueacademicresearchwhilstmaintaininghisclinicalcompetencies.HavingnowcompletedhisFellowship,Jamescontinuestotrytobalanceafull-timeclinicaljobwithongoingresearchprojectsinthefieldsofnovelradiationdetectorsanddosimetryforradionuclidetherapy.

Gettingstartedinresearchcanatfirstseemonerous.Thispresentationaimstobreakdownthemythsandbarrierstogettinginvolvedinresearch.
Routesintoresearchanddevelopmentwillbeexplored,withemphasisbeingmadeonseizingtheinitiativeinone’sownplaceofwork,whetherornotcurrentlyemployedinaresearchposition.Fundingopportunitieswillbediscussedandmanagementofthedifferentphasesoftheprojectlifecyclewillbeconsidered.Successfulresearchreliesoncontributionsfromthevariousmembersoftheprojectteam,eachofwhomhasaparticularroletoplay.Allroleswithintheteamareimportant,anddifferentskillsandpersonalstrengthsarerequiredforeach.Therefore,itislikelythatanindividualinterestedinresearchwillbeabletofindanactivitysuitedtotheirownaptitude.Thevariousroleswillbeconsideredandtheskillsrequiredforeachdiscussed.

Casereportsandcaseseriesareacommonroutetopublication,particularlyforthoseintheearlierstagesoftheircareer.However,opportunitiesforpublishingsuchreportsinthepeer-reviewed,indexedliteraturearediminishing.Inthistalk,IwillcoversomeofthelessonsIhavelearnedfrompublishingvariouscasereportsandcaseseries,andpointoutsomeoftheimportantfactorstoconsiderbeforedraftingthereport.Similarly,auditsandserviceevaluationscanbeafruitfulroutetoseeingyournameinprint.Tomaximizethechanceofsuccessfulpublication,somebasicprinciplesshouldbefollowed;Iwillhighlighttheseandprovidesomeexamplesfromthepublishedliterature.Bytheendofthetalk,delegateswillhaveanunderstandingofthebenefitsofattemptingtopublishcasereportsandaudits,thedownsidesoftheseasaroutetopublicationandtheprinciplesinvolvedinmakingacasereportorauditanappealingone,helpingincreasethechanceofsuccessfulpublication.

FollowingtheWalportreportin2005,theinceptionoftheNationalInstituteforHealthResearchClinicalResearchtrainingprogrammehasgivenstructuretoclinicalacademictrainingintheUK.Theacademicfoundationprogrammeprovidesdoctorsatthebeginningoftheirtrainingwithanopportunitytospendaperiodoftimeresearching,learningaboutmedicaleducation,ormanagement,whilstundertakingtheirclinicalfoundationprogramme.Theacademicclinicalfellowship(ACF),isacompetitiveawardthatpermitsdoctorsfromST1tospend25%oftheirtrainingtimeinresearch,learningtheskillstoenablethemtoinitiate,designandundertakeresearchwithinaclinicaldiscipline;ACFpostsarerun-throughandleadtoanationaltrainingnumbersuffixA.Academicclinicallectureships(ACLs)areopentodoctorsalreadyholdingaPhDorequivalentfrom

ST3level;theyaredesignedtoallow50%oftimededicatedtoresearchandtheremainderspentinclinicalpractice.ThedurationofanACLisforamaximumof4yearsanditisexpectedthatACLswillcompletetheirspecialtytrainingduringthisperiod.Alongsideclinicaltraining,ACLswillbeabletofurtherdeveloptheiracademicskillsandwillbeencouragedtoapplyforfundingtosupportfurtherpostdoctoraloreducationalisttraining.Asanacademicclinicalfellow,Iwilldiscussmyacademiccareertodate,talkingabouttheprosandcons,obstaclesandmistakesIhavemade.Iwilladdressprotocolwriting,ethicsandfundingapplications.

Researchanddevelopmentshouldberegardedasanessentialcomponentoftheroutinedutiesofaclinicalscientist.However,thedeliveryoftheclinicalservicemusttakepriority,andsignificantresourcesareoftenrequiredforsubstantialresearchprojects.Thispresentationwillprovidesomedetailontheoptionsavailableforclinicalscientistswhowishtopursuedoctoralorpost-doctoralresearchprojects.AswellasthenewHigherSpecialistScientificTrainingschemeintheNHS,optionsforindividualfundingfrombodiessuchastheNIHRwillalsobeconsidered.Thepresentationwillalsohighlightsomeofthechallengesofbalancingclinicalandresearchcommitmentsthatfaceacademically-mindedNHSscientists.
