BrightStar begins every case with an assessment by a Registered Nurse.This is our first step in discovering the right care relationship with you or your loved one.

This assessment can be completed at home or, in a hospital, skilled nursing facility or assisted living community before you go back home.

• Based on this meeting, our RN will develop a customized senior home care plan to be followed by the BrightStar caregiver. • The family’s involvement and approval of any in home care plan is an integral part of this process.
