CC-ABHIisauniquecollaborationofhealthcare,science,industry,not-for-profit,andgovernmentpartnersstrivingtohelpolderadultsagesafelyinthesettingoftheirchoice.Wearecollaboratingwithcliniciansandindustrypartnerstodevelop,testanddisseminateinnovativesolutionsthatmeettheneedsoftheagingpopulation,inOntario,Canadaandaroundtheglobe.

ReportfromourManagingDirector,RonRiesenbachThefocusofourfirstyearhasbeentointegrateournetworkofcollaboratorsandbuildafoundationforfuturegrowthandsuccess.
Ithasbeenanexcitingfirstyearwithmanyaccomplishmentstospeakofandsharewithyou.Amongmanyimportantmilestonesachievedthisyear,wecompletedacomprehensive5-yearBusinessPlan,developedouryear1,andyear2OperatingPlans,negotiatedandsignedkeyfundingagreements,andlaunchedtheprocessestodevelopfoundationalagreementswithnetworkpartners.Wealsodevelopedkeyperformanceindicatorsandbenchmarksagainstwhichwearemeasuringoursuccess.
Nowintooursecondyear,andwithahighly-skilledmanagementteaminplace,CC-ABHIispoisedtoaccelerateitsactivitiesandrealizeitsgloballeadershipmission.
Fosteringandmaintainingmulti-sectoral,multi-jurisdictionalpartnershipsiscriticalforCC-ABHIinestablishingitselfasanationalhubandgloballeaderinthedevelopment,validation,commercialization,andadoptionofbrainhealthandseniors’caresolutions.CollaborationenablesCC-ABHItoleveragetheuniqueskillsandexpertiseofadiversegroupoforganizationstoadvancepromisinginnovativesolutionsfromprototype,tobedside,tomarket,andthispastyearwehaveengagedwithournetworkpartnerstoplanhowtoleverourcombinedstrategies.Asweenterintoyear2ofoperation,weareexcitedtoformandfacilitatenewcollaborationswithlocal,national,andinternationalpartners.

adultsinlong-termcaresettingsthroughanoperationalmodelthatadvancesevidence-basedbestpracticesandfacilitateslearningamongorganizations.
Aconsortiumof14ofthehighestperformingeldercareorganizationsandtheirassociateduniversitiesinNorthAmerica,theSQLIprovidesatestinggroundforinnovativeproductsandservices.Membersprovidetheiruniqueexpertisetoenableevaluation,dissemination,andadoptionofnewcarepractices,whiledesigningandbeta-testingemergingtechnologiesthatsupportseniors’well-beinginreal-worldcaresettings.SomeoftheinauguralinnovationsCC-ABHIis
pursuingincludeanonlinecognitiveassessment,consumer-directedcognitiveneuro-rehabilitativestrategies,facialrecognitionsoftware,remotewellnessmonitoring,health-coachingsoftware,mobilemedicationmonitoringandtele-dementiacare.

TheQOCAhomesolutionisacloud-basedplatformdesignedtoimprovesocialconnectednesstolessentheeffectsofisolationandloneliness.ThecommercialpartnerforthisprojectisTaiwan-basedQuantaComputerInc.,oneofthelargestcomputermanufacturersintheworld.Theprojectinvolvescustomizing,deploying,andevaluatingtheQOCAhomeplatform,andmorespecificallytheHomeHubtabletdevice,withseniorsandtheircircleofcareintheBaycrestcommunity.2015/16sawthedevelopmentandrefinementoftheQOCAhomeplatformanddevice,advancementsthatwillimproveusability,interface,andcontentforseniorsandtheirfamilies.

TheMemoryandAgingProgram(MAP)isanevidence-basededucationandinterventionprogramforolderadultsexperiencingnormalage-relatedmemorychanges.Throughinteractivelectures,discussion,andpracticalexercises,participantslearnaboutmemorychangesthatoccurwithage–whatisnormalandwhatisnot–andwhatcanbedonetoimprovememory.Over1,000olderadultshaveparticipatedintheface-to-faceprogramatBaycrest.ThroughCC-ABHI’ssupport,anonlineversionofMAPwillenablesignificantscalability–MAPwillbeextendedtobringbenefitstohundredsofthousandsofolderadults.

Memoryretrievalofevents,locationandnamesgetsmoredifficultwithadvancingageandcanbeexacerbatedfollowingastroke,braintumorortheresultofaneurodegenerativedisease.Triggerisaprostheticmemoryapplicationthatenablesindividualswithmemoryimpairmenttore-livethepastincontext,thewaymemoriesshouldbeexperienced.Usingtheapp,memoriescanbetriggeredwithincontextbylocation,time,andfacialrecognitiononamobiledevice.Thesolutionallowsusersofthisapplicationtobrowsememoriesinavarietyofwaysincludingbyperson,placeandtime,andsearchbytextentriesandkeywords.2015/16accomplishmentsincludedtherefinementofthemobileapplicationinordertoimprovethefacialrecognitionaspectofthesolution.

Deliveringeffectivetherapiesforbraininjuryorcognitivedeclinerequiresastructuralandfunctionalunderstandingofthepatient’sbrain,andtodaythatunderstandingiscomingintofocusthroughaground-breakinginitiativecalledTheVirtualBrain(TVB).TVBisthefirstcomputerizedmodeltocapturetheintricatedetailsofthestructureandfunctionofthebrainundernormalcircumstances;asaresultoftraumaordisease,andasitages.Currently,TVBisusedasaresearchtooltobuildmodelsthatpredictthedevelopmentandprogressofdisease.Withthegoalofexpandingitsclinicaluse,CC-ABHIispartneringwithInteraXonusingitsMUSEheadbandasatypeofmobileelectroencephalogram(EEG)thatcouldenablenewclinicaluses.

Thewayoureyesmovecanrevealtheinformationthatwehaveinourmemories,andwecanexploiteyemovementstoinvestigatechangesinmemoryrelatedtoaging.WithCC-ABHIsupport,Baycrestresearchersaredevelopinganinnovativeeye-trackingbasedassessmenttoolforearlydetectionofcognitivedecline.

Virtual CareExtending Care Beyond the Hospital
VirtualCareisaninnovativeprojectthatintendstobringhealthservicestotheclient,ratherthanbringingtheclienttotheclinician.Theprojecthasthreesub-projects:ResponsiveBehavioursVideoCapture,BedsideTelemedicine,andPreventionofFallswithInjury.TheVirtualCareprojectisaimedatprovidingthefollowingbenefits:behaviouralsupportforpatientsandcaregiverslivingwithdementiaduringtransitionsbetweencaresettings,sectors,andteams;enhancedaccesstointegratedandspecializedcareteamsforresidentsoflong-termcarehomesandcommunitydwellingclientsthroughtelemedicine,andreducedincidenceofinjuriousfallsandavoidableemergencydepartmenttransfersforresidentsandpatientswithdementialivinginlong-termcareandin-patientrehabunits.
2015/16wasayearforplanningandlayingthefoundationsforworkthatwillbeginintheSpringof2016.In2015/16theteamestablishedprojectcharters,beganthedatainfrastructureupgradesrequiredtosupporttheroll-outoftheprojects,executedkeyenvironmentalscans,andbeganthedevelopmentoftheevaluationframeworksneededtoproperlytestandevaluatetheefficacyoftheseprojects.
neuroVaSCular reaCtiVityUsing MRI Imaging to Improve Early Detection of Dementia
VasculardiseaseinthebrainhasbeenimplicatedinthedevelopmentofvasculardementiaandAlzheimer’sdisease,thetwoleadingformsofdementia.Theabilitytomapcerebrovascularreactivity,akeyindicatorofvascularhealthinthebrain,couldleadtoimprovedwaystodetectthediseaseinitsearlystagesaswellastonewtreatmenttargetstoalleviatediseasesymptoms.TheCerebrovascularReactivityprojectinvolvesdevelopingnovel,non-invasivemethodsusingMRItechnologytomeasurevascularfunctioninthebrainasthekeysupportingmechanismforbrainactivity.In2015/16thedevelopmentandrefinementofanewcerebrovascularreactivitymethodforMRItomeasurebloodflowinthebrainmarkedtheongoingprogressforthisexcitingCC-ABHIfundedproject.Thedevelopedsequencecouldresultinasignificantreductioninscanningtimes,whichinturnmayimpactoverallhealthcarecostsacrossthesector.

Intheireffortstounderstanddementiaandotherbraindisorders,scientistshaverecognizedthepromiseofbigdata–verylargesetsofinformationthatmaybeanalyzedtorevealpatterns,trends,andassociationsrelatedtobrainstructure,brainfunction,andhumanbehaviour.However,thestandardsforgatheringandanalyzingdatamayvaryacrossinstitutions,limitingthegatheringandoptimalextractionofinformationforresearchandclinicaltrials.
TheaimoftheMRINeuroinformaticsprojectistodevelopstandardizedarchitecturetoshareneuroimagingdata,essentialhealthinformation,andanalysistoolsnationally.Thiswillcreaterobust,reproducibleMRIdataprocessingandanalysispipelinesthatcanbeusedinpartnercentres.
Dr.StephenStrotherledtheeffortin2015/16todevelopapre-processingsoftwarepipelinetoanalyzebrainimagingdata.Further,Dr.BradBuchsbaumhelpedcreate2machine–learningalgorithmsandsoftwaretoolsfortheclassificationandpredictionofbrainconditionsfrombrainimagingdata.Bycombininglargedatasetsinastandardizedway,thisprojectcanleadtowiderspreaddataanalysesandoffermorestatisticallyinformedoutcomes.

Thediscoveryofnewtreatmentsforbraindisorderssuchasstroke,traumaticbraininjury,dementia,andlate-lifedepressionoftentakesatrial-and-errorapproach.Newtreatmentsareoftentestedwithlittleconsiderationofthespecificunderlyingbrainchangesinadiseaseandoftheimpactofapotentialnewtreatmentontherelevantbrainstructureandfunctionmechanisms.
SupportedbyCC-ABHI,researchersatBaycrestareusingstate-of-thearttechnologyincludingmagneticresonanceimaging(MRI)andmagnetoencephalography(MEG)tounderstanddisease-relatedandacutetreatment-relatedchangesinbrainstructureandfunctioninindividualswithvariousbraindisorders.Additionally,informationisbeinggatheredaboutphysicalandphysiologicalhealthinordertoconstructaholisticpictureofwhatpromoteshealthybrainaging.Thecontinuedadvancementofthisprojectwillallowresearcherstogatherinformationinsmallernumbersofindividuals,acceleratingthetimeframefromquestiontoanswer;researcherswillalsobeableuseanapproachthatinvolvesacomprehensivesetofbrain-imagingandphysical-healthassessmentstoevaluateexistingbraininterventionsandunderstandwhycertaininterventionsworkforsomepatientsbutnotothers.Theworkaccomplishedin2015/16ishelpingtopavethewaytostrongerclinicaltrialsthatsavetimebyfocusingontreatmentsthatappropriatelyimpacttherelevantstructures,processes,andfunctionsofthebrain.
CommuniCationS neuroSCienCeUnderstanding how the Brain Hears
Researchisincreasinglyshowingthatcommunicationdifficultiesarerelatednotonlytotheearbutalsotothebrain,givingrisetothefrequentcomplaint,“Icanhearyou,butIcan’tunderstandyou.”SupportedbyCC-ABHI,Baycrestresearchersareworkingtoincreasetheirunderstandingofhearing,speechperception,andspeechproduction,sotheycanapplythisknowledgetonewandbettertreatmentsandimprovecommunication.Byincreasingourunderstandingofthebrainregionsresponsibleforprocessinglanguageandsounds,thisresearchcontributestonewknowledgethatcanbeappliedtodevelopingnewandimprovedbraininterventionstohelppeoplewithcommunicationimpairmentsenjoybetterconnectionswithothers,leadingtoabetteroverallqualityoflife.
In2015/16Dr.BernhardRosscontinuedthedevelopmentandtestingofanewanalysismethodthatallowsfordigitizationoftheheadin3D,whichcanbeusedinthecontextofanalysingneuroimagingdataacquiredforexamininghearingandlanguageproblemsinseniors.Aswell,Dr.ReginaJokelmaintainedmomentumindevelopingsoftwarethataimstoanalysespontaneousspeechthatwilldifferentiateanddiagnosedisorderslikedepressionordementia.

Olderadults,especiallythosewithcomplexhealthissues,havereducedaccesstorecreationalactivitiesthatprovidecognitiveandsocialengagement,thiscanresultinfeelingsofisolationandloneliness.Art-on-the-Brainusesartworkasavehicleformeaningfulintellectualandsocialengagementforadultswithandwithoutdementia.Theprimarygoaloftheapplicationistopromoteimprovedhealthandwell-being,whilethesecondarygoalistoprovidephysiciansandcareproviderswithaprovenandsafetoolforpatientuse.ThecommercialpartnersforthisprojectareArtGalleryofOntario,CanadianArtMagazine,Memory&Company,QoCHealth,MaRSInnovation,andQuantaComputerInc.
InJune2015themobileapplicationbetaprototypewasdevelopedataHackingHealtheventinToronto,leadingtoArt-on-the-BrainwinningtheGevityInnovationAward,aswellasbeingawardedtherunner-upprizeasOntarioTelemedicineNetwork(OTN)MostInnovativeSolution.Followingthisevent,andthroughout2015/16,theprojectteammetwithpartnercandidatessuchassoftwaredevelopers,designers,andmobileexpertswhohavetheabilitytodevelopthebetaversionofthissolutionintoafullyfunctioningmobileapplicationthathasthepotentialforwidespreadcommercialdissemination.

Earlyidentificationofpotentialcognitiveproblemscouldleadtoearlydiagnosisalongwithappropriatecaredesignedtohelptheindividualmaintaintheircognitivehealthandindependenceforaslongaspossible.CognicitiBrainHealthAssessmentisascience-basedbrainhealthtestdevelopedbyclinicalneuropsychologistsandcognitivescientistsatBaycresttohelpdetermineifcertainmemorychangeswarrantavisittothedoctor.Thetest,foradults50to79,wasco-developedbyCognicitiInc.,whichisownedbyBaycrestandpartnerMaRSDiscoveryDistrict.Todate,morethan40,000adultshavecompletedthetest.ThecommercialpartnerforthisprojectisJohnsonandJohnsonInc.

GoalManagementTraining®(GMT)isaBaycresttherapywithprovenefficacyintherehabilitationofclientswithexecutivedysfunctionresultingfrombraindisorders.GMTisstructuredintoninemodules,withinteractivediscussionsdesignedtoraiseawarenessofvariousaspectsofgoalmanagement,tasksthatillustrategoalmanagementconceptsinaction,andhomeworkassignmentsdesignedtofacilitatethetransferofconceptstoreallife.
In2015/16theprojectbegandevelopinganonlineversion,whichwillprovidewideraccessforGMTtherapiststoreachmorepatientssuchasthosewhohavelimitedornoaccesstoin-persontherapyduetogeographicalorphysicalrestrictions.

Forallhealthcareprofessionals,continuouseducationisanimportantpartofensuringconsistentknowledgeandqualitypatientcare.ThisprojectinvolvesdevelopingandimplementingarigorousandcomprehensiveLearningManagementSystemthatprovidese-learningcoursesforhealthcareprofessionalsandthosewhointeractwiththehealthcaresystem,includingseniors,theirfamilies,andtheircaregivers.Thisinitiativewillseetheacquisition,configuration,integrationanddeploymentofCC-ABHI’sLearningManagementSystem.
In2015/16thisprojectdeveloped3modulestoeducatecliniciansonhowtobettercarefortheirpatients,modulesthatwilleventuallypopulatethee-Learningplatform.Onlinemodulesweredesignedaspartofablendedlearningcoursetotrainstaffonhowtoworkwitholderadultsandseniors.Trainingforstaffwasalsoincludedtohelpthemunderstandmoreaboutthecomplexneedsofolderadults,includingnormalagingandsensoryandcognitivechangesthatcanaffectcare.Staffcompletedtheon-linemodulesindependentlyandthenparticipatedinafulldayface-to-faceworkshoptoapplywhattheyhadlearnedonline.Thefirstdeploymentoftheon-linematerialwasfor15cliniciansatSt.Michael’sHospitalinToronto.
Brain health promotionResearching the Impact of Healthy Habits on a Healthy Brain
Arecentstudysuggestedthata10-25percentreductioninriskfactorssuchasdiabetes,mid-lifehypertension,mid-lifeobesity,smoking,depression,andphysicalinactivitycouldpotentiallypreventasmanyas1to3millioncasesofAlzheimer’sDiseaseworldwide.Acomprehensiveunderstandingofhowlifestylefactorscontributetobraindisordersanddevelopingnewinterventionsthatwouldpromotesustainedhealthylifestylebehavioursthroughoutthelifespancouldultimatelyreducetheincidenceofdementiaandotherage-relateddisorders.
SupportedbyCC-ABHI,researchersatBaycrestareinvestigatingfactorsthataffectbrainhealth,includingdiet,socialengagement,andmood,todevelopnon-pharmacologicalinterventionsandprocesses(e.g.lifestylechanges)topromoteandoptimizebrainhealthacrossthelifespan.
Thegoaloftheprojectistoreducethelikelihoodofdevelopingbraindisorderssuchascognitiveimpairment,dementia,andlate-lifedepression.Thisknowledgewillserveasabasisfordevelopingnewinterventionsthattakeintoaccounttheindividual’spsychosocialenvironment,activities,andotherstrategiestopromotesustainedchangesinlifestylebehaviours.
2015/16accomplishmentsincluderesearchperformedbyDr.NicoleAndersonthatexploredthefeasibilityandefficacyofusinglong-termcarehomes’volunteerstoadministeracognitivestimulationprogramtoresidents.Thisresearchdiscoveredthatthevolunteer-administeredcognitivestimulationprogramdemonstratedbothfeasibilityandefficiency.Withadditionalresearch,andlargersamplesizes,thelong-termimpactmayleadtoanefficientandscalablewaytoimprovecognitioninseniorslivinginlong-termcarehomes.
