Specific to Shumsky’s relationship with Premier Health Partners, the focus of this document is Shumsky’s branded apparel program and our custom promotional products services.

WHEN AND HOW WILL I RECEIVE MY MVH PROFESSIONAL IMAGE APPAREL? Keep your hard copy order form you received at the Fair. If you have provided a valid e-

mail address, in addition to your hard copy receipt, you will receive an e-mail order confirmation as well.

Orders will be delivered to MVH beginning in mid December. MVH and Shumsky will establish a schedule for pick-up of apparel and will communicate

this to you. You can begin wearing your new apparel as soon as you receive it, but no later than Feb.

HOW DO I ORDER MORE MVH PROFESSIONAL IMAGE APPAREL? Go to www.miamivalleyhospital.org/employees and log in with your username (Employee

ID) and your initial password (Last four digits of your SSN). You may change your password by logging in and going to My Account, then Change Password.

HOW QUICKLY WILL I RECEIVE MY APPAREL? Apparel sizes XS-3XL are normally kept in stock and are ready for shipment within 24 hours. Please allow 2 weeks for fulfillment of sizes other than XS-3XL.

HOW DO I MAKE EXCHANGES OR RETURNS?Contact Shumsky’s customer service area to arrange exchanges or returns at (937) 221-7103 or (800) 414-8946.

EXCHANGE/RETURN POLICYShumsky offers a 100% satisfaction guarantee. You may return current, unused merchandise at any time for a full refund. HOW DO I TAKE THE BEST CARE OF MY MVH PROFESSIONAL IMAGE APPAREL?The best way to keep your Landau apparel looking its best is to follow the recommended Landau laundering instructions:

Machine wash warm Wash with like colors If needed, use non-chlorine bleach only Do not use liquid fabric softener (contains chemicals that could interact with the dye

chemicals). Use a softener sheet in the dryer instead. Tumble dry medium Wrinkling will be at a minimum if folded or placed on hangers promptly.

HOW DO I GET THE MVH LOGO EMBROIDERED ON PROFESSIONAL IMAGE APPAREL THAT I PURCHASE OUTSIDE OF THIS PROCESS?

Buy new apparel (approved in the Professional Image Program) and leave the tags on Bring the apparel to Shumsky Enterprises distribution center at 2750 E River Road,

Moraine, Ohio 45439 between the hours of 8:00AM and 5:00PM EST Shumsky will document the order and give you a pick up date Pay for order with credit card or payroll deduct Return to the distribution center to pick up your embroidered apparel
