How I got here
Medical doctor since 1984
Pediatric oncologist at the Dana Farber Cancer Institute, Boston, Massachusetts
Too many of my patients were dying from being in the hospital
Masters in Architecture & Engineering
Started focusing on decreasing infections
Now studying impact of enclosures on all occupants
Thank you for this opportunity to meet!

What is going on?
Patients are dying FROM being in the
hospital
Antibiotic resistance is unstoppable
Chronic diseases are increasing
Infectious diseases are re-emerging
This is confusing.....
![Page 6: The Essential Role of Indoor Air Quality in Patient Outcomes · The Essential Role of Indoor Air Quality in Patient Outcomes Stephanie Taylor, MD, ... Asthma has reached epidemic](https://reader033.fdocuments.in/reader033/viewer/2022042400/5f0ea91b7e708231d4404fe3/html5/thumbnails/6.jpg)
Autoimmune diseases have increased
Learning disabilities have risen 50 % in
the past 10 years
Endocrine disorders such as diabetes
have increased 30% since 1985
Asthma has reached epidemic
proportions, now the No. 1 cause of school
absenteeism
Birth defects are the leading cause of
infant mortality in the US
Chronic diseases are affecting more and more people

Humans spend
95%of our indoors!

Is this true ??
Are our buildings really protecting our health?
“We shape our buildings, then they kill us!”

We are more microbial than human!
Each of us is an ecosystem with ~ 100 trillion other microscopic organisms living in and on us
Microbial cells ~ 100 trillion
Human cells ~ 30 trillion
![Page 16: The Essential Role of Indoor Air Quality in Patient Outcomes · The Essential Role of Indoor Air Quality in Patient Outcomes Stephanie Taylor, MD, ... Asthma has reached epidemic](https://reader033.fdocuments.in/reader033/viewer/2022042400/5f0ea91b7e708231d4404fe3/html5/thumbnails/16.jpg)
The expanded tree of life
Genetic analysis has revealed diverse microbial populations that were unknown before cultivation-independent approaches

We send our microbes into
buildings
Buildings send their microbes to us
Our microbes interact with the indoor environment
Humans emit approx. 10^7 genome copies of bacteria, and 10^6 fungi into the air per person-hour

Bacteria spread through the air when
the outdoor humidity is low
“Once the humidity exceeds 40%, the
epidemic ends”
Dry weather brings meningitis outbreaks
![Page 29: The Essential Role of Indoor Air Quality in Patient Outcomes · The Essential Role of Indoor Air Quality in Patient Outcomes Stephanie Taylor, MD, ... Asthma has reached epidemic](https://reader033.fdocuments.in/reader033/viewer/2022042400/5f0ea91b7e708231d4404fe3/html5/thumbnails/29.jpg)
Dry seasons promote many bacterial epidemics
![Page 30: The Essential Role of Indoor Air Quality in Patient Outcomes · The Essential Role of Indoor Air Quality in Patient Outcomes Stephanie Taylor, MD, ... Asthma has reached epidemic](https://reader033.fdocuments.in/reader033/viewer/2022042400/5f0ea91b7e708231d4404fe3/html5/thumbnails/30.jpg)
Dry air is a greater factor than cold temperatures
Low RH was associated with higher pathogenicity
This connection was more significant than humidity
ratio or temperature
![Page 31: The Essential Role of Indoor Air Quality in Patient Outcomes · The Essential Role of Indoor Air Quality in Patient Outcomes Stephanie Taylor, MD, ... Asthma has reached epidemic](https://reader033.fdocuments.in/reader033/viewer/2022042400/5f0ea91b7e708231d4404fe3/html5/thumbnails/31.jpg)
Bacteria from spaceships reveal robust survival tactics
Microbes in dust from the Russian ISS modules survived desiccation, ultraviolet radiation and heat shock through developing “extremo-tolerant” characteristics such as:
(i) spore-forming ability(ii) resistance against radiation, pressure,
desiccation (iii) the increased expression of antibiotic
resistant genes

Children and seniors are especially vulnerable to the ill-health effects of low RH
Children Seniors
• Delicate fluid balance • Higher water loss through
skin• No control over fluid input• No control of clothing
• Sense of thirst is reduced• Bedridden people have less
autonomy• Seniors often limit drinking
to reduce toilet visits
![Page 36: The Essential Role of Indoor Air Quality in Patient Outcomes · The Essential Role of Indoor Air Quality in Patient Outcomes Stephanie Taylor, MD, ... Asthma has reached epidemic](https://reader033.fdocuments.in/reader033/viewer/2022042400/5f0ea91b7e708231d4404fe3/html5/thumbnails/36.jpg)
Proper air hydration is essential for our respiratory system defenses
Key functions of respiratory cells:
• Cilia wash particles away from delicate
lung tissue
• Mucus layer allows healthy immune
modulation to reduce allergic reactions
• Mucous from goblet cells trap pathogens
Dry inhaled air causes:
• Increased susceptibility to infections
• Increased wheezing from allergic
disease
![Page 37: The Essential Role of Indoor Air Quality in Patient Outcomes · The Essential Role of Indoor Air Quality in Patient Outcomes Stephanie Taylor, MD, ... Asthma has reached epidemic](https://reader033.fdocuments.in/reader033/viewer/2022042400/5f0ea91b7e708231d4404fe3/html5/thumbnails/37.jpg)

With RH of 40%–60%, infectious droplets settle out of the airborne environment
Particle behavior with increased air hydration
Pathogens are removed from
surfaces during room cleaning
Bacteria or virus are
expelled into the hospital environment
in droplets
Airborne spread to
other in-patients is halted, resulting in
fewer HAIs.
When the Patient Room RH is maintained at a comfortable RH of 40-60%, infectious droplets settle quickly and are removed by effective room cleaning, greatly decreasing airborne spread to other patients and mechanical systems.

ABSTRACT
An air-to-air heat exchanger may be used in cold, dry winter conditions to improve building Indoor Air Quality (IAQ). Using the heat generated by electronics,
lights and people inside a building, a heat pipe heat recovery design may be employed to add beneficial humidification and maintain winter indoor Relative
Humidity (RH) above 40 % with lower additional energy cost to the owner. Recent studies have shown that a room RH below 40 % increases the risk of cross
infection between office building employees. Airborne pathogens, such as the Flu, stay buoyant and viable longer in dry environments. Using a rigid media Direct
Evaporative Humidifier (DEH), overheating the outdoor air furnished to the air side economizer allows a 50 oF (10 oC) to 60 oF (15.5 oC) VAV delivery
system, in Seattle Washington, to add free humidity while exceeding the minimum outdoor air requirements of ASHRAE Standard 62.1.
THE HEAT RECOVERY ECONOMIZER
Several Recent scientific studies have identified the relationship between indoor RH and the increased risk of infection
from airborne Flu virus. Research indicates that aerosolized influenza virus remain buoyant and survive longer at low room
RH levels. Other studies have examined the size of “droplet nuclei “ expelled by infected humans, during coughing and
sneezing, and the influence of droplet size and room air turbulence on the duration of suspension of these particles in the
human breathing zone. It turns out that we get sick in winter because the indoor environment is dry in winter. Infectious
microorganisms remain in the breathing zone indoors longer when RH drops below 40 %.
An all air VAV cooling system with overhead delivery may either contribute to or reduce the spread of airborne
infection. With proper humidification, a VAV design, using the new Dual Maximum VAV Box Control Logic at the terminal
units, may be used to reduce room air change rates and room turbulence during the dry-cold winter season. An article by a
Medical Doctor who is also an Architect concludes that reducing room air change rates will lead to less in room turbulence
and will reduce the risk of cross infection for humans. The author goes on to say that a relative humidity between 40-60 %
RH along with reduced air change rates is key to reducing infection rates. With properly selected terminal units and ceiling
diffusers and using the Duel Maximum VAV control logic, room air ventilation may safely be reduced as low as 0.15 CFM
per sqft (0.76 x 10-3 m3/s per m2).
A VAV design concept that will provide both humidification in winter and reduced room air change rates while
maintaining a room air Dry Bulb (DB) temperature between 72 oF (22.2 oC) to 78 oF (25.5 oC) is called a Heat Recovery
Energy savings with proper indoor air hydration
