The recipient of the David W. Parke Lecture is recognized as an individual who embodies the qualities that distinguish the professional character and life service of DavidW. Parke, MD.
Dr. Parke exemplifies the highest standard of patient care,teaching, community service and professional achievement inthe specialty of Ophthalmology. With a professional and non-professional career hallmarked by numerous accomplishments,David W. Parke, MD exemplifies the physician as humanitarian.We are deeply indebted to him for his lifelong journey of excellence, a journey that has benefited and continues to benefitthousands of patients, students of medicine, colleagues and theprofession of Ophthalmology, and that will serve as a goldstandard for future generations to come.
It is with great pleasure that our first named lecture recipientwill be his son, David Parke II, MD, and that this first lecturewill be delivered in the June 2014 annual program.
William H. Ehlers, MD Advocacy Award
The William Ehlers, MD Advocacy Award joins the David ParkeLeadership Award and the Anne McDonald Life Time AchievementAward as one of three named CSEP legislative awards adopted by theConnecticut Society of Eye Physicians Executive Board. Dr. Ehlersis being recognized for his numerous contributions during his tenureas past president of CSEP and current co-chair of the CSEP legislative Committee. Dr. Ehlers has served as legislative chair formore than 10 years, during which time he has written, reviewed and delivered hundreds of pieces of testimony and proposed bills andamendments. He has met with hundreds of legislators during thistime and has led the CSEP Day at the Capitol delegation as well asthe CT delegation in Washington on numerous visits over the past 20years. His contributions to CSEP are enormous and as a tribute to hisdedication, inspiration and leadership CSEP proudly bestows thehighest award for advocacy in his name.
The recipient of this award has demonstrated exemplary dedicationand fortitude in legislative advocacy in the effort to promote the protection of patients and the practice of medicine and surgery in theState of Connecticut. The candidate for this award is nominated andvoted on by the Executive Committee of the Connecticut Society ofEye Physicians and is selected for both his/her demonstration ofadvocacy, integrity and in recognition of his/her un-wavering dedication to the practice of medicine.

Ms. Asbell served as Staff forReimbursement for The American Society of Cataract &Refractive Surgery from 1998 until 2008 that included rep-resentation at the CPT meetings and interfacing withCMS.
K. BAILEY FREUND, M.D.Dr. Freund is a retina specialist who practices at Vitreous–Retina–MaculaConsultants of New York. He is a graduateof Williams College and the New York
University School of Medicine. Dr. Freund’s residencytraining in general ophthalmology and two–year fellow-ship in medical and surgical retina were at theManhattan Eye, Ear & Throat Hospital.Dr. Freund specializes in the diagnosis, treatment and
care of patients with all retinal disorders including macular degeneration, diabetic retinopathy and retinalvascular and inflammatory diseases. Dr. Freund is anexpert in difficult–to–diagnosis or rare conditions thathave eluded other specialists. Dr. Freund is on the faculty of the New York University

School of Medicine as a Clinical Associate Professor ofOphthalmology. He is on staff at New York PresbyterianHospital, Manhattan Eye Ear & Throat Institute, and LenoxHill Hospital. He teaches medical students, residents andretinal fellows. He is the author of over 140 articles in peer-reviewed ophthalmologic journals and has writtennumerous book chapters. He is on the Editorial Board ofthe journal, Retina, and is an Associate Editor for RetinalCases & Brief Reports.Dr. Freund has been a principal investigator in pivotal tri-als of novel treatments for retinal diseases, providing hispatients with the opportunity to gain access to new treat-ments before they had been approved for the generalcommunity. In addition, he has initiated investigationsinto novel treatments for a variety of retinal diseasesincluding Macular Degeneration, Central SerousChorioretinopathy, Angioid Streaks and Retinal VeinOcclusion.Dr. Freund is a member of numerous organizations includ-ing the American Academy of Ophthalmology, TheMacula Society, The Retina Society, Gass Club, and theAmerican Society of Retina Specialists. In 2011, hereceived the prestigious Young Investigator Award fromthe Macula Society. Dr. Freund was born and raised in Manhattan and con-

tinues to reside there with his wife of 25 years who is achild and adolescent psychiatrist and his ten–year oldson and seventeen–year–old daughter. He spends hisweekends and holidays Upstate in Dutchess County.
Dr. Freund enjoys playing the guitar and is aprominent collector of vintage magicapparatus.
WARREN HILL, M.D. Dr. Hill has been the Medical Director ofEast Valley Ophthalmology in Mesa,
Arizona for the past 24 years, specializing in consultativeophthalmology, challenging anterior segment surgery forother ophthalmologists, intraocular lens power calcula-tions and diagnostic ophthalmic ultra-sonography.Dr. Hill completed BS and BA undergraduate degrees
at the University of Maryland, received his Doctor ofMedicine from the University of Arizona College ofMedicine, completed two years of internal medicinetraining at Good Samaritan Medical Center in Phoenix,Arizona and three years of ophthalmology training at theUniversity of Rochester, in Rochester, New York. He is cer-tified in general ophthalmology by the American Boardof Ophthalmology and has subspecialty certification inanterior segment surgery by the American Board of Eye

Surgery. Dr. Hill is also a member of the International Intra-Ocular Implant Club, a Fellow of the American Collegeof Surgeons, the American Academy of Ophthalmology,the International College of Surgeons and the AmericanCollege of Eye Surgeons. In 1995, 1996, 1998, 2002 andagain in 2004 Dr. Hill was voted a "Top Doc" in the special-ty of ophthalmology by medical peers in PhoenixMagazine. In 2005 and 2006 Dr. Hill was voted "One of theTop 50 Opinion Leaders in Cataract and Refractive
Surgery" by the readership of Cataract andRefractive Surgery Today.
LOUIS R. PASQUALE, M.D.Louis Pasquale graduated from ManhattanCollege in New York City and then went onto receive an MD with Distinction in
Research degree from SUNY Stony Brook. After internshipat Bronx Municipal Hospital affiliated with Albert EinsteinSchool of Medicine, he completed his ophthalmologicaltraining at Temple University School of Medicine inPhiladelphia, PA. Motivated by his Chairman, Dr. GuyChan's favorite saying, "There are no problems, only chal-lenges" he embarked on a 2-year fellowship inGlaucoma at the Wilmer Eye Institute in Baltimore, MD.Immediately after fellowship in 1992, he built a large glau-

coma practice at Brigham and Women's Hospital,where he also forged important research collabora-tions that drive much of his research efforts today. In1996, he joined the Glaucoma Service at Mass Eye andEar Infirmary and rose through the ranks to becomeService Director in 2010. Currently, Dr. Pasquale isAssociate Professor of Ophthalmology at HarvardMedical School. In addition to directing the GlaucomaService at Mass Eye and Ear Infirmary he directs theGlaucoma Fellowship Program, the MEEI TeleretinalProgram (2008 to present), and co-directs Harvard’sGlaucoma Center of Excellence. He also has appoint-ments at Channing Laboratory and Schepens EyeResearch Institute in Boston where he is AssociateEpidemiologist and Assistant Scientist, respectively. Heis a Heed ophthalmic fellow and ARVO silver fellow aswell as recipient of the AAO’s Achievement Awardand Secretariat Award. In 2009 he was the recipient ofResearch to Prevent Blindness’ Physician ScientistAward. He is also an NIH Principal Investigator with con-tinuous support since 2006. In 2011, he was appointedas Distinguished Scholar in Ophthalmology at HarvardMedical School. His research focuses on the discoveryof primary prevention strategies in the open-angle

glaucomas. Dr. Pasquale has dedicated himself tobecome a physician scientist who seeks opportunitiesto translate basic science discoveries into better treat-ments for glaucoma patients. He has published 94peer-reviewed articles in scientific journals, 54reviews/book chapters/editorials, and has given
numerous talks/courses around theworld.
STEPHEN PEDNEAULT, CPAStephen Pedneault is the principal ofForensic Accounting Services, LLC ofGlastonbury, CT. His firm specializes only
in forensic accounting, fraud, and litigation supportmatters. Steve has been in the field for 24 years andhas authored four books, and numerous articles onfraud prevention, embezzlement, fraud, and forensicaccounting. He has also spoken over 200 times locallyand nationally, and works as an adjunct professor atthe University of Connecticut in the Master’s of Sciencein Accounting program.

GEOFFREY TABIN, M.D. Dr. Tabin is Professor of Ophthalmologyand Visual Sciences and Co-Director ofthe Division of InternationalOphthalmology at the University of Utah’s
John A. Moran Eye Center. He specializes in cornea,cataract and refractive surgery. Dr. Tabin graduatedfrom Yale College and earned an M.A. in philosophy atOxford as a Marshall Scholar, followed by an M.D. fromHarvard Medical School. Dr. Tabin is committed to providing quality ophthalmiccare and education to all the patients he serves. Hehas traveled the world extensively conducting sightrestoring surgeries and training local doctors. In 2009,Dr. Tabin was named an "unsung hero" by the DalaiLama for his dedication to eradicating unnecessaryworld blindness and implementing a model for sustain-able ophthalmic care in the developing world. An avid mountaineer, Dr. Tabin was the fourth personin the world to climb the "7 summits," the highest pointof all seven continents.

*Pitfalls in Chart Documentation – This course reviews chart docu-mentation problems found in electronic medical records (EMR) andgives a step-by-step critique on how to identify and then cornrectthem. Suggestions for formatting your EMR as well as problemsencountered in audits and under review by the OIG will be dis-cussed. Identification of erroneous EMR programming that is foundfaulty in Medicare audits and by the OIG will be discussed.Objectives: At the conclusion of this course the participant will be ableto: 1. Identify the appropriate roles of ancillary personnel and their permitted roles in chart documentation. 2. Review of the necessary elements for compliance for the Key Components of History, Examinationand Medical Decision making in E/M coding as well as the Eye Codes. 3. Identify proper documentation for orders for diagnostic tests and signatures. 4. Supervise the preparation of a Medicare compliantInterpretation & Report for all diagnostic tests.
