PCA CARE CONNECTIONS IS PHILADELPHIA’S
NEWEST PROVIDER FOR OLDER ADULT LONG-
TERM SERVICE COORDINATION
Philadelphia - PCA Care Connections officially opens its
doors as a premier provider of service coordination for older
adults and individuals with disabilities residing in the five
counties of Southeastern Pennsylvania. PCA Care
Connections, a separate 501(c)(3) organization created by
Philadelphia Corporation for Aging, will help to meet the long
-term care needs of adults who are eligible for Medicaid and
in a nursing facility or for those who are dually eligible for
Medicare and Medicaid. PCA Care Connections is a
contracted provider with two managed-care organizations
participating in Community Health Choices, Pennsylvania’s
new managed-care program: Keystone First Community
Health Choices and Pennsylvania Health and Wellness. Those
who are eligible and need long-term supports and services are
provided with a service coordinator through their managed-
care organization. PCA Care Connections was created to
adhere to federal and state regulatory changes regarding the
provision of service coordination and the delivery of other
Medicaid services such as home-delivered meals and home-
modifications. The creation of PCA Care Connections
eliminates any potential for a perceived conflict of interest,
while still providing quality service to those receiving long-
term services and supports. PCA Care Connections will
continue Philadelphia Corporation for Aging’s longstanding
work as a provider of service coordination with the same
caliber of staff that service recipients have come to expect.
“Philadelphia Corporation for Aging was founded on its
mission to provide vital services to older Philadelphians,
preserving their quality of life,” Philadelphia Corporation for
Aging President and CEO Holly Lange said. “I am proud to
announce that PCA Care Connections will continue in that
tradition, as service coordination is essential to independent
and dignified living for many adults.” Service coordinators are
responsible for working with individuals who may face
challenges with daily living tasks. They ensure an individual’s
goals and expectations for long-term care will be met through
various services and supports.
PCA Care Connections is headquartered at 642 N. Broad
Street, Suite 300 in Philadelphia. For more information about
PCA Care Connections, please call (215) 282-6500.

A participating parishioner is a member of Saint Timothy Parish who faithfully attends Mass on Sunday & who con-tributes to the support of our parish at that time through the use of Sunday envelopes with a minimum contribution of $11.00 per week as the parish subsidy has been increased to $1,150 per student. In addition, High School students should continue to use their envelopes as well. All contribu-tions will be reviewed & tuition will be charged accordingly.
Sanctuary candle: A sanctuary candle burns in both our upper and
lower church to remind us of the sacramental presence of Jesus Christ in the Holy Eucharist. If you wish to have the sanctuary candles in the upper or lower church burn for a full week in memory of a loved one, please contact the rectory.
The suggested offering is $10.
Bread and Wine: In the early Church it was customary for the
People of God to provide the bread and wine to be used at the Celebration of the Eucharist. If you wish to participate in the ancient tradition of providing the bread and wine for the Celebration of the Eucharist in either the upper church or lower church as a memorial for your Family or your deceased loved ones, please call the rectory. Suggested offering: $10.
Sanctuary Memorial Fund: This fund is an ideal way to
memorialize deceased loved ones and friends. At the time of a funeral, funds can be directed to the Parish Sanctuary Fund. An acknowledgement of the gift is sent to the Family of the deceased. The fund is used for the upkeep of Saint Timothy Sanctuaries. Mass is offered for Memorial Fund Benefactors privately.
