OnbehalfoftheHongKongCollegeofFamilyPhysicians,IwouldliketoexpressmywarmestwelcometothenewfellowsanddiplomatesofFamilyMedicineofourCollege.
Iwouldliketocongratulateournewfellowsanddiplomatesasyouarenowequippedwithmoreadvancedknowledgeandskillstoserveyourpatientsandthecommunity,andIknowyouwillexercisethesewiththehigheststandardofethics.Primarycarehasbeenrecognizedtobethemostcost-effectiveintermsofhealtheconomy.Thehighcomorbiditiesandlowprevalenceofindividualdiseasesinone’sdailypracticemaymakeclinicalguidelinesdifficulttoapply-andtheoutcomeofyourworkdifficulttomeasure.ContinuousparticipationinCMEandCPDactivitiesnotonlyfacilitatesourcontinuous improvementbutalsoprovidesopportunitiestomeetoldandnewfriendstoshareourexperience, joyandfrustration.Performingclinicalauditsallowustoshareexperienceinqualityassurance.Research,regardlessofitsscale,cananswermanyquestionsuniquetous.Ithereforeencourageallofyoutoparticipateactivelyintheseactivities.TheCollegeisyourhomeforprofessionalgrowth,andwe,asbrothersandsistersareheretoshareandsupport.
Withanagingpopulationandincreasingmultimorbidity,familyphysicianshaveanimportantroleinholisticcare.ManyofoursisterColleges,includingtheRoyalAustralianCollegeofGeneralPractitionersandRoyalCollegeofGeneralPractitioners,havesuccessfullyreviewedtheircurriculumrecently.Wehavealsosetupaworkinggrouptoreviewourcurriculumthisyeartobestsuitourcommunitydevelopmentandcaredelivery.
IwouldliketotakethisopportunitytothanktheRoyalAustralianCollegeofGeneralPractitioners,forthelong-termsupportinourvocationaltrainingandexamination;ourGovernmentforthesupportanddevelopmentinPrimaryCare;WONCAandcolleagues fromChina,Macau&TaiwanforthefriendshipandpromotionofFamilyMedicine;theUniversitiesforagreateremphasisonFamilyMedicineeducationandresearch;theHospitalAuthorityandsisterspecialtiesfortheirsupportinourvocationaltraining.Lastbutnotleast,ourfamiliesfortheunfailingsupport.Ihopewecanallworktogetherforthebetterhealthofourworld.

Thisyear’sHongKongPrimaryCareConferencewas indeedspecialas itwasalsoheld togetherwith thebi-annual4-PartyConferenceonJune6-9,2014.Thismemorableeventhadalmost700registrants includingdoctors,nursesandalliedhealthprofessionals,withaboutaquarterofthemfromoverseas.
Thethemeoftheconference,“WiththePatients,ForthePatients-AchievingHealthEquity inPrimaryCare”highlightedthe importance inaspiringtoachievehealthequity inprimaryhealthcaredelivery.The fourplenary speakershaveprovidedusavividdescriptionofhowdifferent regionshadaddressed this issueveryclosetoourheart.Weagreedthat familydoctorsandhealthcareprofessionalshadasignificantroletoadvocateforourpatients inaddressingthe issuescontributingtohealthdisparitiesandthisconferencebroughttogetherthepolicymakers,cliniciansandfrontlineprofessionalstodiscussthecurrentandfuturechallengesinachievingthisgoal.

HKCFP Best Research Award 2013TheResearchCommitteeisdelightedtoannouncethatthestudyon“TheEpidemiologyandNaturalHistoryofDepressiveDisorderinPrimaryCare”hasbeenawardedtheHKCFPBestResearchAward2013.TheCommitteewouldliketocongratulatetheprincipalresearcherDr.ChinWengYeeforherwork.
Dr.ChinWengYeeisagraduateoftheUniversityofWesternAustraliaandaFellowoftheRoyalAustralianCollegeofGeneralPractitioners.Sheinitially joinedtheLiKaShingFacultyofMedicineattheUniversityofHongKongasanhonoraryFamilyMedicineteacherin2006,andnowholdsjointappointmentswiththeDepartmentofFamilyMedicineandPrimaryCareandtheInstituteofMedicalandHealthSciencesEducationofHKUwheresheisprimarilyinvolvedinundergraduateFamilyMedicineteaching,curriculumdevelopmentandqualityassuranceoftheundergraduatemedicalcurriculum.
Hermainresearchfocushasbeenonmentalhealthandmedicaleducation.Sheisparticularlyinterestedinidentifyingwaysfordoctorstobetteraddresspatientswithcommonpsychologicalproblems,andinexaminingwaystoenhancedoctors’andstudents’attitudestowardsprovidingmentalhealthcare.
Inthelastthreeyears,WenghasbeenkeentohelpbuildtheresearchcapacityoffamilydoctorsinHongKong.Sheestablishedaterritory-widementalhealthpractice-basedresearchnetwork,currentlyinvolving60familydoctors,andhasbeendeliveringprimarycareresearchskillsworkshopsandmentoringyoungfamilyphysicianstoconductresearchinprimarycare.
Sheisalsocurrentlyinvolvedinevaluatingthequalityofcareofmulti-disciplinaryprimarycareclinic,focusingondeliveryofcarebynursesandalliedhealthcareworkers.
In2013,shewasawardedtheHKUFacultyofMedicine’sFacultyTeachingMedal,theFaculty’shighestawardforteaching.
HKCFP Research Fellowship 2014It isalsoourpleasuretoannounceDr.SitWingShanastheawardeeoftheHKCFPResearchFellowship2014.
Dr.SitWingShan,ReginaisaClinicalAssistantProfessorattheJockeyClubSchoolofPublicHealthandPrimaryCare,theChineseUniversityofHongKong.ShegraduatedfromtheFacultyofMedicine,theUniversityofHongKongin2003.ShereceivedherFamilyMedicinetraininginHongKongandobtainedherfellowshipsoftheRoyalAustralianCollegeofGeneralPractitioners(RACGP),theHongKongCollegeofFamilyPhysicians(HKCFP)andtheHongKongAcademyofMedicine(HKAM).ShealsoholdsfourdiplomasinthefieldsofPediatricsMedicine,PracticalDermatology,CommunityGeriatricsMedicineandInternalMedicine.DrSit‘sresearchinterestisinthefieldofmusculoskeletalpaininprimarycare.SheisalsothephysicianinchargeoftheInstituteofIntegratedMedicine,theChineseUniversityofHongKong.
HerprojectawardedtheResearchFellowshipisentitled“EfficacyofPatellaMobilizationTherapy inKneeOsteoarthritis:ARandomizedControlledClinicalTrial”.Kneeosteoarthritis(OA)isthemostcommonformofchronicarthritisworldwideandthemajorcauseofpainanddisability.TheetiologyofkneeOAisnotentirelyclear.Accumulatingevidencesuggestedthattheprogressionisoftendrivenbybiomechanicalforces.Therefore,tryingtoslowdownthediseaseusingpharmacologicaltreatmentwithoutaddressingunderlyingbiomechanicalproblemshasbeendisappointing.AlteringthemechanicalmisalignmentinkneeOAisasuccessfultreatmenttoreducesymptomsanddelaytheneedfortotaljointreplacementinsomepatients.
Recently,therehasbeeninterest inresearchoftheclinicalefficacyofmanualtherapyintreatingdifferentkindsofmusculoskeletalproblems.Thepurposeofthisstudyistoevaluatetheefficacyofpatellamobilization,oneofthemanualtherapytokneeOA.SincethepatellaisusuallypulledlaterallyinkneeOAduetoweakvastusmedialisrelativetothevastuslateralisaswellasthetightiliotibialband,theprojecthypothesizedthatbymobilizingandkeepingthepatellamedially,onecanrestorethemisalignmentandcorrectthederangedbiomachanisminkneeOA,withsubsequentimprovementinpainandfunction.

WONCA Asia Pacific Regional Conference 2014 in Kuching, Malaysia
ThoughthismeetingwasbilledasaWONCAAsiaPacificRegionalConference,itwasstrikinginthediversityofrepresentation,withdelegatesfromallcontinentsoftheglobe,saveAntarctica.Therewereover1,000registrantsofwhom25wereHongKongdelegates,whichwasapleasantsurprise.Onthescientificfront,HongKongwaswellrepresentedinwhich4outofthe27freepaperoralpresentationsand11outofthe185posterspresentationswerefromHongKong.
Inthisreport,Iwillbrieflydiscussthevalueofoneoftheplenarysessions,thefreepaperpresentations,andthenetworkingopportunitiesconferredbythisconferenceandmakesomesuggestionsbasedonmyobservationswhichmaybehelpfulwhenhostingfutureconferences,orwhensendingdelegationsabroad.

ProfessorMichaelKidd,PresidentofWONCA,washonouredwiththeprivilegeofdeliveringtheWesFabOrationentitled‘Areouruniversitiesnurturingthefuturefamilydoctors?’inwhichhenotedthediversityinthisconferenceandthevaluediversitybringstoourdiscipline.DrawingonspecificinteractionshehadwithspecificMalaysianfamilydoctorsintheirpracticeshespokeofthededicationandprofessionalismeachbroughttopatientcareandtheexcitementeachpossessedtocontinuetoexploreandbringbestpracticestotheirpatients.Healsolookedbackathisownpersonaldevelopmentasadoctorandconsideredthevariousinfluenceswhichshapedhiscareerpathincludingundergraduatemedicaleducation.Thisturnedouttobesignificantbecauseitprovidedearlyopportunitiestobeinspiredbyrolemodels,andinstilledthedesiretocontinuelifelonglearningandtoshareinacommunitywhichcontinuallychallengesandsupportsoneanother.Thoughtheremaybedifferencesincultureandlanguageinthepracticeoffamilymedicinearoundtheworld,theprinciplesoffamilymedicinediscussedandexperiencedintheuniversityyearsarecommonandserveasafoundationforinspiringfuturefamilydoctors.ThiswasastrongvalidationoftheimportanceofongoingdevelopmentandstrengtheningofFamilyMedicineeducationthroughouttheundergraduateyears.

Therewere4sessionsoforalpresentationswhichwerespreadoutoverthedurationoftheconference,andgroupedlooselybytheme.ThissessioninwhichIpresentedprovidedausefulglimpseintofamilymedicineeducationresearchwhichwasbeingundertakenaroundtheworldandthevarietyofmethodologiesbeingused.Inmysession,mostpaperswerefocusedonmedicaleducationwhileothertopicsincludedchronicdiseaseorepidemiologyofdepression.Ajudgewaschargedwithaskingthepresentersquestions,withquestionsfromtheaudiencepermittedonlyiftimeallowed.Thisunfortunatelygavethesenseofjustpresentingtothejudgeratherthantheaudienceasawholebutitwasstillagoodopportunityforallparticipantstolearnfromthefeedbackgivenbyasenioracademic.
Theresearchposterswereoncontinuousexhibitionwithoutadesignatedsessionfortheauthorstobepresenttodiscusstheirwork.TherewasahugevarietyofworkondisplaywithtopicsrangingfrombarriersinophthalmologicscreeningindiabeticpatientstoICPCcodingpracticestofamilydoctorjobsatisfactionwhichagainreinforcedthediversityoftheresearchundertakeninfamilymedicine.
Networking with colleagues
Betweensessionsitwasdifficulttointeractwithnewcolleaguesbeyondcursorysocialsmalltalkasthetimewasfairlytight,butIfoundthattheconferencedinnerturnedouttobetheeventinwhichsomeofthemostfascinatingandusefulinteractionstookplace.InadditiontoseveralcolleaguesfromHongKong,mytableincludedProfessorKassaifromFukushima,Japan(whohadpreviouslyvisitedHongKongasaninvitedspeaker)andalsoaNewZealandfamilyphysicianwhowasacquaintedwithProfessorBruceArroll,apreviousHKCFPvisitingprofessor.ProfessorKassaisharedthathekeptablogfollowingthetragedyinFukushimawhichIthinkwouldbeapowerfuldoctors’narrativeforuseinmedicaleducationtohelpstudentsappreciatethehumanityoffamilydoctorandthesocialresponsibilityshoulderedbyourprofession.
Suggestionsforconsideration:
1. ItwasunfortunatethattherewasnodesignatedforumforallthoseintheHongKongcontingenttospecificallyconveneandbondthemtogetherasagroup.Ofthe25HKdelegates,Imet2coincidentallyanddidnothavetheopportunitytomeettheothers.Workingwiththeorganizingbodytofacilitateagatheringofdelegatesfromthesamecity,evenforacoffeebreakorphoto,wouldinstillasenseofcamaraderieandenabledelegatestomeetthosefromtheirowncitywhomayhavesimilarinterestsandperhapsleadtofuturecontactandcollaborationuponthereturnhome.
2. Itwouldbehelpfultothepresenterstohavethejudgingcriteriapublishedsothatpresenterscouldtailortheirpresentationaccordingly.Audiencemembersshouldbeallowedtoaskquestions,inadditiontothejudges,toencourageinteractivity.
3. Thosewhosubmitpostersshouldbegivenanopportunitytoorallypresenttheirwork.Therewasahugeroomfullofposterswhichcoveredadiverseandinterestingrangeofresearchwhichdemonstratedagreatdealofeffortandwhichworthshowcasing.Inotherconferencesthishasbeeneffectivelyaddressedbyhavingpostersarrangedinthemedgroupsandinvitingauthorstomakeshort(3-5min)presentationsoftheirworktotheirgroupandotherinterestedparticipants.Eachsessioncouldberunundertheguidanceofafacilitatorasascheduledresearchpresentationsession.
4. Otherthanthesocialandculturalopportunitiesprovidedbytheconferencedinner,itisalsoamosteffectivesettingfornetworking.Theenvironmentislessfranticandthereisabuilt-insmallgroupwithwhichtointeractoveranextendedperiodoftime.Thisisanunexpectedlyvaluablepartoftheconferencewhichshouldnotbeoverlooked.

TheResearchCommitteeiscommittedtopromote,facilitate,buildcapacityinandcoordinateresearchinfamilymedicineandprimarycare.Everyyear,theCommitteeorganizestworesearchawards,theHKCFPBestResearchAwardandHKCFPResearchFellowship,grantingsuccessfulapplicantsfundsuptoHK$100,000.Since2013,the“TraineeResearchFund’and“ResearchSeedFund”havebeenintroducedtoprovidesmall researchfundingandsupporttofurtherencouragemorepractitionersespeciallythosewithintrainingorinprivatepracticetotakepartinresearch.
Thecommitteeviewsresearchasanimportantandessentialpartwithintheprofessionaldiscipline,andtherearemanyexcitingdevelopmentonresearch intherecentyearssuchasthe introductionof researchaspartof theHKAMFellowshipExaminationand theexpansionof familymedicinefaculties inbothuniversities inHongKong.Atthesametimeitsdevelopmentisnotwithoutthreatse.g.primaryareresearchinHongKongisstillatitsinfancyandtherelativelysmallcriticalmass-fromresearchuserstoresearchleaders(figure1)arestillverysmallcomparedtotheotherdisciplinesorbyinternationalstandard. Updates on Herpes Zoster Vaccine
IntroductionHerpeszostercanoccuratanyage,buttheincidenceincreaseswithage,atthesametimetheseverityofherpeszostergenerallyincreaseswithageandcancausedevastatingpostherpeticneuralgia(PHN).Herpeszostervaccineprovidesameansofpreventioninpatientswithpriorchickenpoxinfection.
Current available Zoster vaccineThisvaccineisaliveattenuatedvaccinebasedontheOka/Merckstraindevelopedthroughserialpassagesintissueculture.Thevaccinecontainsthesamecomponentsasthevaricellavaccinebutwithanapproximately14-foldorhigherconcentrationofthevirus.Zostervaccineshouldbeadministeredasasingle0.65mLdosesubcutaneouslyinthedeltoidregionoftheupperarm.
EfficacyInaclinicaltrialinvolvingmorethan38,000adultsaged60yearsolder,thevaccinereducedtheoverallincidenceofshinglesby51%andtheincidenceofPHNby67%.Theefficacyofthevaccineinpreventingshingleswashigherintheyoungeragegroup(60-69years;vaccineefficacywas64%)thanintheolderagegroup(olderthan70years;vaccineefficacywas38%).Studiesareongoingtoassessthedurationofprotectionfromonedoseofzostervaccineandtheneed,ifany,forboosterdoses.Therearenocomprehensivedataontheeffectivenessofzostervaccineintreatingshinglesonceitoccurs,andthevaccineisnotlicensedforthisindication.
Contraindications1. Priorrecipientofvaricellavaccine2. Historyofanaphylactic/anaphylactoidreactiontogelatin,neomycin,oranyothervaccinecomponent3. Historyofimmunosuppressionorimmunodeficiency4. Individualswhohaveactiveacutesevereillness5. Pregnancyorpossiblepregnancy
PrecautionsThevaccinecontainsaliveattenuatedvaricella-zostervirus,andisonlyrecommendedforuseinimmune-competentpeople.Deferralshouldbeconsideredinacuteillness(forexample,inthepresenceoffever)orinpatientswithactiveuntreatedtuberculosis.Pregnancyshouldalsobeavoidedfor3monthsfollowingvaccination.
Adverse reactionsThemostfrequentadversereactionswereheadacheandinjection-sitereactions.Veryrarely,achickenpox-likeillnesswasreportedinfewerthanonein10,000people.
Recommendations of use of zoster vaccines in different countriesAdvisorycommitteeonImmunizationPractices(ACIP)fromCentersforDiseaseControlandPrevention(CDC),USA:ACIPrecommendsroutinevaccinationofallpersonsaged>60yearswith1doseofzostervaccine.Personswhoreportapreviousepisodeofzosterandpersonswithchronicmedicalconditions(e.g.,chronicrenalfailure,diabetesmellitus,rheumatoidarthritis,andchronicpulmonarydisease)canbevaccinatedunlessthoseconditionsarecontraindicationsorprecautions.
USA:FoodandDrugadministration(FDA):In2011,FDAexpandedtheageindicationforzostervaccinetoincludeadults50through59yearsoldforpreventingherpeszoster.Thisdecisionwasbasedonalargestudyshowingthatthevaccinereducedtheriskofherpeszosterbyapproximately70percent.
Canada:NationalAdvisoryCommitteeonImmunization(NACI):Zostervaccineisrecommendedforthepreventionofherpeszosteranditscomplicationsinpersonsaged60yearsorolderwithoutcontraindications.
UK:Immunisationagainstinfectiousdisease(AKAtheGreenBook):Theaimofthenationalshinglesimmunisationprogrammeistolowertheincidenceandseverityofshinglesinolderpeople.Itisrecommendedtoberoutinelyofferedtopeopleaged70yearsbutitcanbegiventoindividualsupuntiltheeightiethbirthday.
