รมต.นพ.ประดิษฐ์ สินธวณรงค์ ทิศทางการทํางานในระบบหลักประกันสุขภาพชาติ ใน 10 ปีต่อไป

การพัฒนาระบบข้อมูลข่าวสารให้เป็นระบบและมีการนํา

เทคโนโลยีมาใช้เพื่อเพิ่มประสิทธิภาพและคุณภาพ เพื่อใช้เป็น

ฐานข้อมูลระดับชาติ (National Health Information) และ

ภายใน 1 ปี ตั้งเป้าหมาย มีหน่วยงานกลาง ในการจัดทําธุรกรรมการ

เบิกจ่ายระดับชาติ (National clearing house) เกิดขึ้น โดยมี

คณะกรรมการดูแลระบบที่มีส่วนร่วมของผู้เกี่ยวข้องทุกภาคส่วน หน่วย

กระตุ้นและเสนอให้ทุกประเทศสมาชิกขององค์การอนามัยโลกดําเนินการ

วางโรดแม็ป (Roadmap) การดําเนินการให้เกิดระบบข้อมูลสุขภาพ อิเล็กทรอนิกส์และมาตรฐานข้อมูลสุขภาพทั้งในระดับประเทศและในระดับท้องถิ่น

พัฒนากลไกทั้งด้านนโยบายและกฎหมายที่จะสนับสนุนการพัฒนาระบบข้อมูลสุขภาพอิเล็กทรอนิกส์และมาตรฐานข้อมูลสุขภาพ ร่วมถึงสนับสนุนให้เกิดการใช้งานและปฏิบัติตามมาตรฐานและให้เกิดการแลกเปลี่ยนข้อมูลกันทั้งในส่วนของรัฐและเอกชน ทั้งนี้ต้องคํานึงถึงการรักษาความเป็นส่วนตัวข้อมูลทางคลินิก

▫ คณะกรรมการกําหนดระบบบริหารยา เวชภัณฑ์ การเบิกจ่ายค่า

ตรวจวินิจฉัยและค่าบริการทางการแพทย์ มีมติให้ สปสช. และ

ประกันสังคม นํา TMT ไปทดลองใช้ (4 เมษายน 2556)

▫ กรมบัญชีกลางประกาศให้ใช้ TMT ในการเบิกจ่ายยา ผป.นอก

ของกรมบัญชีกลาง (หนังสือ กบก. ว 178 14 พ.ค. 2556)

▫ สํานักงานพัฒนาธุรกรรมอิเล็กทรอนิกส์ (สพธอ.) คณะ ประกาศ

TMT ให้เป็นมาตรฐานรหัสยาสําหรับการทําธุรกรรม

อิเล็กทรอนิกส์การบริการทางการแพทย์ของไทย (Healthcare

services eTransaction)

30 ตุลาคม 2556 คณะกรรมการกําหนดระบบบริหารยา เวชภัณฑ์ การเบิกจ่าย

ค่าตรวจวินิจฉัยและค่าบริการทางการแพทย์ มีมติ

• ให้กระทรวงสาธารณสุขนํา TMT ไปดําเนินการประกาศให้เป็นมาตรฐานบัญชี

รายการยาและรหัสยาของประเทศ เพื่อเชื่อมโยงรหัสยาของโรงพยาบาลต่างๆ

การเบิกจ่ายในระบบสวัสดิการรักษาพยาบาลข้าราชการ

• ในระยะต่อไปให้นําTMTผนวกเข้าไว้ในโปรแกรมของโรงพยาบาล เพื่อใช้

สําหรับการสั่งซื้อและการเบิกจ่ายทั้ง 3 กองทุน

• ในระยะยาวให้ใช้สําหรับจัดซื้อยาตามระเบียบพัสดุ รวมทั้งจะขยายต่อยอด

กบัระบบยาอื่นๆ เช่นการขึ้นทะเบียนตํารับยา การกระจายยา

• พรฎ.ว่าด้วยวิธีการแบบปลอดภัยในการทําธรุกรรมทางอิเล็กทรอนิกส์ พ.ศ. 2553▫ วิธีการแบบปลอดภัย มี 3 ระดับ (พื้นฐาน, กลาง, เคร่งครัด)▫ จําแนกตามประเภทของธุรกรรมทางอิเล็กทรอนิกส์ (ธุรกรรมที่มีผลกระทบต่อความมั่นคงหรือความสงบเรียบร้อยของประเทศ หรือต่อสาธารณชน) หรือจําแนกตามหน่วยงาน (ธุรกรรมของหน่วยงานหรือองค์กรที่ถือเป็นโครงสร้างพื้นฐานสําคัญของประเทศ หรือ Critical Infrastructure)

“วิธีการแบบปลอดภัย”

ธุรกรรมทางอิเล็กทรอนิกส์ ประเภทต่อไปนี้• ด้านการชําระเงินทางอิเล็กทรอนิกส์• ด้านการเงินของธนาคารพาณิชย์• ด้านประกันภัย• ด้านหลักทรัพย์ของผู้ประกอบธุรกิจหลักทรัพย์• ธุรกรรมที่จัดเก็บ รวบรวม และให้บริการข้อมูลของบุคคลหรือทรัพย์สินหรือทะเบียนต่างๆ ที่เป็นเอกสารมหาชนหรือที่เปน็ข้อมูลสาธารณะ

• ธุรกรรมในการให้บริการด้านสาธารณูปโภคและบริการสาธารณะที่ต้องดําเนินการอย่างต่อเนื่องตลอดเวลา

วิธีการแบบปลอดภัยในระดับเคร่งครัด

•ผลกระทบต่อจํานวนผู้ใช้บริการหรือผู้มีส่วนได้เสียที่อาจได้รับอันตรายต่อชีวิต ร่างกาย หรืออนามัย▫ ต่ํา: ไม่มี▫ ปานกลาง: ผลกระทบต่อร่างกายหรืออนามัย 1-1,000 คน▫ สูง: ผลกระทบต่อร่างกายหรืออนามัย > 1,000 คน หรือต่อชีวิตตั้งแต่ 1 คน

•ผลกระทบด้านอื่นๆ• ถ้ามีผลประเมินที่เป็นผลกระทบในระดับสูง 1 ด้าน ให้ใช้วิธีการแบบปลอดภัยระดับเคร่งครัด

ระดับผลกระทบกับวิธีการแบบปลอดภัย

• อ้างอิงมาตรฐาน ISO/IEC 27001:2005•มีผลใช้บังคับเมื่อพ้น 360 วัน นับแต่วันประกาศในราชกิจจานุเบกษา (19 ธ.ค. 2555) คือ 14 ธ.ค. 2556

• ไม่มีบทกําหนดโทษ เป็นเพียงมาตรฐานสําหรับ “วิธีการที่เชื่อถือได้” ในการพิจารณาความน่าเชื่อถือในทางกฎหมายของธุรกรรมทางอิเล็กทรอนิกส์ แต่มีผลในเชิงภาพลักษณ์และน้ําหนักการนําข้อมูลอิเล็กทรอนิกส์ไปเป็นพยานหลักฐานในการต่อสู้คดีในศาลหรือการดําเนินการทางกฎหมาย
