CONCEPTOEnfermedad aguda, paralítica, espástica, causada por una neurotoxina del Clostridium tetani.
![Page 4: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/4.jpg)
CLOSTRIDIUM TETANI
Bacilo
movil
Gram positivo
Anaerobio obligado
Formador de
esporas Ambiente
natural:
Suelo
Polvo
Tracto gastro-intestinal de animales
![Page 5: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/5.jpg)
CARACTERÍSTICAS No invade tejidos. Las esporas sobreviven al hervor,
mueren el autoclaves. Las formas vegetativas no soportan
antibióticos, calor, ni desinfectantes habituales.
Dosis letal de tetanospasmina: 10-5 mg/kg.
![Page 6: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/6.jpg)
FORMAS CLÍNICAS Tétanos neonatal
Tétanos materno
Tétanos traumático
![Page 7: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/7.jpg)
EPIDEMIOLOGÍAEndémico en 90 países en vías de desarrollo.
Forma más frecuente: tétanos neonatal o forma (umbilical).
Mata ½ millón de neonatos al año, 80% en Asia y África tropical.
15.000 a 30.000 madres mueren por Tétanos materno.
![Page 8: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/8.jpg)
PATOGENIA Esporas germinan
Se multiplican
Producción de toxina (tetanospasmina) en el potencial de acción de oxidación-reducción bajo de la herida.
Gen de la toxina es llevado por un plásmido
Se libera con la muerte bacteriana
![Page 9: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/9.jpg)
PATOGENIA Cadena pesada y cadena ligera (100 y 50 Kd)
Se une a unión neuromuscular.
Entra al nervio motor por endocitosis
En el citoplasma de neuronas motoras, inactiva la glicina y el GABA
Inhibe a los músculos antagonistas
![Page 10: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/10.jpg)
TOXINA Cadena ligera: Endoproteasa que
contiene zinc Sustrato: sinapto-brevina
El inóculo continua en el sitio de herida
![Page 11: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/11.jpg)
MANIFESTACIONES CLÍNICAS Forma generalizada
2 a 14 días incubación - meses
Trismos
Cefalea, irritabilidad, fatiga.
Mareo, disfagia, espasmo cervical,
«Risus sardonicus»
Opistotonos
![Page 12: tetanos](https://reader035.fdocuments.in/reader035/viewer/2022081517/568164e0550346895dd742b0/html5/thumbnails/12.jpg)
OTRAS FORMAS Tétanos neonatal
3 a 12 días del parto
Tétanos localizado
Tétanos cefálico

DIAGNÓSTICO Clínico
Paciente no inmunizado con herida (o madre con herida)
Nacido en las dos semanas previas
Aislamiento del C. tetani solo en 1/3 de los casos

DIAGNÓSTICO DIFERENCIAL Trismus: abscesos dentales, faringeos o
parafaringeos
Encefalitis
