St. Louis University NATIONAL SERVICE TRAINING PROGRAM CIVIC WELFARE TRAINING SERVICE 2 LIST OF GENERAL REQUIREMENTS FOR NSTP 2/PLEASE SEE OTHER DETAILS IN THE NSTP 2 MOPG –SUMMER 2012 A- GRADING SYSTEM: students must have scores in each of the clusters below and must have a passing grade when such scores are computed accordingly. 1.Attendance: 10% by transmutation (specific/applicable to a particular project/apostolate). It should be noted that any absence will further affect the class standing item on outputs and participations which were not attended to due to such absence. Considered under attendance are being always present, always on time, no early dismissal and escaping. This includes the general orientation, approved apostolate actual activity, scheduled and approved consultations and meetings. In case a student with excused absences voluntarily applies for remedial, and if he or she is allowed for reasons that are meritorious, to meet the highest number of exact total hours for a particular apostolate as approved by the NSTP office the adjustment in the score or grade to acknowledge the remedial undertaken will only be limited to the 10% attendance in the apostolate grading system. This means that any additional scores or grade brought about by the remedial is limited only to adjusting to a maximum of 10% of his or her attendance score. Here, the remedial in the attendance is not automatically a remedial in the outputs, for instance. 2.Character and performance evaluation: 10% by transmutation (instructor) 10% by transmutation (group mate). LIST OF GENERAL REQUIREMENTS FOR NSTP 2/PLEASE SEE OTHER DETAILS IN THE NSTP 2 MOPG –SUMMER 2012

A- GRADING SYSTEM: students must have scores in each of the clusters below and must have a passing grade when such scores are computed accordingly.

1. Attendance: 10% by transmutation (specific/applicable to a particular project/apostolate). 6. PORTFOLIO 8% by transmutation: i. Format and Organization (is format followed? Are the required parts complete and well-done), Documentation (CD picture contents, the power point and tarpaulin soft lay outs, soft copy of edited concept paper saved in the CD, Are the instructions on documentation followed, Do the pictures illustrate/show activities and procedures of the apostolate, Are the pictures complete or sufficient)

ii. Students are given perfect points then minus errors, mistakes or violations. Any excess not deducted from the success of project implementation or the supervising instructor’s character and performance evaluation to a student.

iii. After assessing the portfolio, evaluate every member for their contribution in accomplishing their group portfolio for their individual score.

7. Success of Project Implementation=25% by transmutation: panel evaluation (Overall quantity-quality output: the ratio of the out put with the number of students who created the output…is the output commensurate with the supposed effort of the number of members working on the said output?, does the output comply with the rationale, goal, specific objectives, activity proper of the apostolate, and provisions of success specific to the given apostolate, is the group able to trouble shoot well, and did the group adopt/apply principles of SLU NSTP community service, Observance of Safety Provisions: Choice of apostolate: students interest on the project, does it match the specialization of the course. Leadership, responsibility and accountability: can the student perform or yield what is expected or even beyond and can

take accountability of own actions? Can the student do things on his own (personal responsibility or leadership), or still needs or recourses to involvement of parents or guardians or others in his/her own behalf?, and Team work, maturity, and shows self-help growth and learning from the participation in the NSTP activities and projects.) 8. Note: When a student incurs a total of zero raw score in a grading item, the student also incurs a zero percentage for this item and not the computed value of such percentage. Example: if a grading item is given 20% but the raw total score from which such 20% is computed is zero, the percentage will not be the computed value which is 10% but instead is made zero percent.

B- GENERAL REQUIREMENTS AS SOURCES/INPUTS TO THE GRADING SYSTEM:1. Form 2: c/o SI2. Enlistment in an apostolate/project3. Original copy of the MOPG of NSTP 2 submit to the SI and filed in the group folder; conforme slip submitted

earlier to SI.4. Individual journal (individual report on orientation, consultations, meetings and apostolate proper): properly

accomplished, submit to the SI and filed in the group folder.5. Individual copy of the approved apostolate file (approval sheet, concept paper, apostolate procedure and

guidelines, forms, waiver, etc)6. Actual apostolate participation: recorded in the form 2.7. Culmination Activity/Post conference attendance: recorded in the form 2.8. Team Performance Evaluation: to be facilitated by respective Supervising Instructors (SI’s are instructed to

ensure confidentiality of the process and result of the evaluation. Instructors will directly supervise the said evaluation). Be guided of the performance evaluation tool of the NSTP .

a. Intra-group evaluationb. Supervising Instructor’s Performance Evaluation on the teamc. If it applies: Host Community Evaluation on the team

9. Pre-apostolate exam, final oral/written exams, and other graded activities:10. Test permit for finals: soon as available, show to SI and have it recorded in the form 2.11. Any available merits:recorded in the form 2.12. Demerits recorded in the form 213. NO remedial: cleared from excess demerits and deficiencies.14. Clearance form (use the clearance from prepared by the NSTP Office. Clearance is secured from:

a. Apostolate Team Leaderb. Host Community/agency/ Officer-in-charge if it appliesc. Supervising Instructord. NSTP Office Authorized Staffe. NSTP Coordinator: clearance by the coordinator is given only when apostolate is substantially completed

(if apostolate objectives and activities as defined in the approved concept paper or as deliberated between the NSTP Coordinator and the Supervising Instructor are substantially followed/implemented) and when the apostolate portfolio is properly accomplished by the team and duly endorsed by the SI and submitted to the coordinator. During clearance with the NSTP Coord, the apostolate portfolio with the appropriate remarks and endorsement of the Supervising Instructor must be presented to the NSTP Coordinator. Also before clearance and as deemed necessary, NSTP Coordinator finally inspects apostolate results if they warrant such clearance.

f. NOTE: process your clearance PERSONALLY and BY GROUP: clearance not processed personally means INCOMPLETE and with additional remedial if grade is passing; otherwise, FAILED GRADE IS GIVEN.

15. Photocopy of Specimen of the ID (with the picture, signature and address) of any of parents as guardian, authorization of Parents to Official Guardian (if applicable), photocopy of the ID of the authorized guardian (with picture, signature and address) and any proof of COMELEC registration, if applicable. These must be filed in the student’s individual or group folder.

16. Attendance in the apostolate, in the orientation series; and in the plenary conference17. APOSTOLATE ACCOMPLISHMENT REPORT PORTFOLIO (one clear book per apostolate)-

SEE SPECIFIC FORMAT AS ATTACHED SUBSEQUENTLY.a. Clear book with appropriate color-coding: color to be used are as follows: GREEN-CGEA, BLACK-SWMA,

PINK-MNHA, ORANGE-SCA, WHITE-MA, VIOLET-APA, YELLOW-RPA, GRAY-CLIMSA, BROWN-SORA, BLUE-YMA, RED-ESMA. The clear book and all entries therein should be in LONG BOND PAPER.

b. Supervising Intructor’s Content and Format Assessment Remarks on the submitted Portfolioc. Title page: with a listing of the major accomplishments for the current term-see attached formatd. List of Facilitating studentse. NSTP Apostolate Approval Letter signed by the University Presidentf. Concept paper (scanned, with signatures)g. Specific Project/apostolate Procedures and Guidelines

ADDITIONAL REMINDERS FOR TEACHERS: 1. Strictly follow the grading system as provided above or in the MOPG of NSTP 2.2. Any questions regarding the grading system must be personally and directly referred to the NSTP Coordinator.3. Before giving an INC, failed, dropped or WP grade to a student, please refer the matter first to the NSTP Coord.4. Before encoding your grades in the online grading sheet of the University, secure first approval from the NSTP
