Welcome to the Burnham Center

On behalf of building ownership and the entire building staff, we extend a warm welcome to 111 West Washington Street, the Burnham Center! We are delighted to have you as our Tenant and will do everything possible to make your tenancy enjoyable and rewarding.

This guide is intended to be a resource for the various services and amenities that are offered at the Burnham Center, as well as the general operating procedures in place at the building.

Communication is the most crucial element in implementing the policies and procedures in this guide. To ensure effective communication, Burnham Center Management requests that you designate a “tenant representative” to be the contact between your company and the management office staff. Your tenant representative will be responsible for making requests or reporting problems as well as conveying important information from our office to your employees.

BUILDING SERVICES AND OPERATIONS

Providing excellent service to you is our primary goal. Through frequent inspection of the property and careful management of the building's resources and personnel, our intent is to promptly and courteously respond to your needs and operate the building in a manner that exceeds your expectations. Therefore, we request your assistance by notifying us of any situation or condition that you feel requires attention.

BUILDING MAINTENANCE

A service request may be submitted through any of the following channels:

1. Contact the Office of the Building via email. 2. Submit an electronic request on the Workspeed website:

Note: Submitting a request on-line is the most efficient method, and will allow self-service work order entry, status-checking, and the ability to review work order history. We strongly encourage you to submit requests electronically. If you would like to learn more about how to make and manage your service requests electronically, please contact the Office of the Building for details.

Once a service request has been received, the office of the building staff will address the problem as soon as possible and, if applicable, bill your office for the completed service request on the next month’s rent statement. If the situation warrants external assistance, the Office of the Building staff will obtain pricing approval prior to performing the work.

HVAC

Heating, Ventilation and Air Conditioning (HVAC) systems in the building operate Monday through Friday during normal business hours of 8:00 a.m. – 6:00 p.m. and on Saturdays from 8:00 a.m. – 1:00 p.m. If at any time during working hours you desire adjustment to the temperature within your suite, electronically submit a work order or contact the Office of the Building for assistance. To avoid damage and minimize delays, please do not adjust thermostats. In addition, we suggest that you keep blinds closed when in direct sunlight. Keeping blinds closed at night and over the weekend will help maintain a more constant temperature in your suite.

HVAC services after business hours are available upon request and can be scheduled by submitting a service request online or by contacting the Office of the Building. There will be a charge for after-hours HVAC service.

TRASH REMOVAL

Normal building trash removal is provided as a standard building service. If you would like empty boxes or other large items removed that are too large for trash receptacles, please attach a Throw-Away sticker to the item and leave it in plain sight for the night cleaning crew to remove. Throw-Away stickers are available in the Office of the Building. You can also call the Office of the Building for immediate removal. Large and small dumpsters are available (for a charge) if you would like to dispose of large amounts of debris that will not fit in regular trash or recycling receptacles. Please enter a work order online to order a dumpster. Construction debris, such as debris following renovations in your suite, is your responsibility as the tenant. In addition, you are responsible for the removal of all medical, infectious, or hazardous waste from the premises, including any needles, syringes, and other articles or substances classified as medical, infectious or hazardous waste by law. Any such removal must be approved by the Office of the Building and performed in compliance with applicable laws.

RECYCLING PROGRAM

The building currently recycles light bulbs, ballasts, metal, paper, glass, plastic and aluminum. Collection containers will be provided at no cost at move-in. Batteries and computer/phone equipment may also be recycled at the building through our electronic recycling program. Please contact the Office of the Building for more information.

RENTAL REMITTANCE Rent should be paid by the first day of each calendar month during the term of the lease. Three methods of rent remittance are available to tenants:

TELECOMMUNICATIONS The Riser Management Company for the property is Hardt Electric. Hardt will be responsible for maintaining and servicing the telecommunications infrastructure from the Netpop (the room where all telecommunications service originates) to the service block in each space.

If you require riser management services, please contact the Office of the Building.

SATELLITE TELEVISION USA Wireless provides DirecTV satellite television services in the building. If you require their services, please contact USA Wireless:

DELIVERIES All large deliveries must be scheduled in advance through the Office of the Building. There is a charge for reserving the service elevator. Please contact the Office of the Building for delivery and installation company insurance requirements.

US POSTAL SERVICE AND SHIPPING INFORMATION There are US Postal Service drop boxes located in both building lobbies, near the entrances. The US Postal Service delivers mail directly to your suite each afternoon. The closest US Post Office is located at the State of Illinois Building, only one block away, at 100 West Randolph Street (entrance on LaSalle Street). The nearest Federal Express drop box is located at 33 North LaSalle Street, which is conveniently accessible through the covered walkway between the buildings along the west corridor from the Washington Street Entrance.

TENANT AMENITIES BIKE RACKS

Bike racks located in three areas on the basement level, accessible via freight elevator.

BIKE LOAN PROGRAM Bikes are available for check out. Users should bring ID and fill out a registration form in suite 1017 before being issued a key. Loaner bikes are located in the basement bike rack area and include use of a lock. Engineering can provide an air pump if needed. Bikes must be returned to the building by 5pm and checked in by a security officer.

PEDAL TO THE PEOPLE Free tune-ups are provided monthly from May-September. Call the office for upcoming dates. FITNESS CENTER A tenants-only fitness center is located on the 13th floor in suite 1315 and contains a variety of cardio, weight machines, and fitness equipment, as well as a stretch room. Each locker room features two showers and locker storage. Towel service is included. Hours: 6am-7pm. Access is granted via security card. Please contact the Office of the Building for current membership rates.

CONFERENCE ROOM Conference room meeting space is available in suite 1212. The room seats up to 36, and tables can be arranged in a board room, lecture hall, or classroom grouping style. Two white boards are mounted on one wall. A Polyphone speakerphone device is available, along with a flat screen television that can be connected to your laptop via HDMI cord. There is also an area for refreshments. *If applicable, caterers are required to provide a COI to the building 48 hours before the event. Room rental is $20/hrly, with a 4 hour minimum. Please email [email protected] to check availability. EVENT ROOM An open, flexible event space is available for rental in suite 960. High tops or other tables available by request. STORAGE Storage spaces ranging from 60 rsf up to 300 rsf are available in the building. If you require additional storage space, please contact the Office of the Building.

If you wish to display a sign or notice in any public area of the building, prior written approval from the Office of the Building is required.

TENANT FUNCTIONS AND GUIDELINES If you are planning a function (e.g., party, reception), please notify the Office of the Building at least two weeks in advance. The Office of the Building maintains certain policies and procedures that assist in coordinating events and limiting liability of the building. The Office of the Building may ask for the following: date and time of event, number of guests, presence of alcohol, parking requirements, overtime HVAC requirements, service elevator use, janitorial needs, electrical requirements (for sound equipment), security requirements, and certificates of insurance. A function may not be held in the common areas or on the building grounds without prior management approval.

LOST AND FOUND Please report any lost or found items to the Office of the Building. STAIRWAYS There are four stairwells running through the building, one at each corner of the core. All exit stairways are for EMERGENCY USE ONLY. Please notify all employees of this security restriction and ask that they cooperate fully so that we may help keep the building secure.

ENERGY CONSERVATION Tenants can assist in our efforts to reduce building operating costs through energy conservation. Please make a special effort to educate your employees about the importance of using energy wisely. Following are examples of how your employees can help in our efforts to reduce energy costs: • Window treatments should be closed during those times when there is direct sun

(including weekends). • Employees should be reminded to turn off all lights, computers, copiers, etc., when

not in use or before leaving for the evening or weekend.

JANITORIAL SERVICES Janitorial services are typically provided five (5) days a week. If you have any special cleaning requests, please contact the Office of the Building.

BUILDING ACCESS AND SECURITY ACCESS – DURING AND AFTER BUSINESS HOURS

Please provide the Office of the Building with a list of the names and phone numbers of at least three individuals who may be called to authorize admittance into the building for an employee or visitor who does not have appropriate security access identification. These individuals would also be contacted in the event of an emergency.

Access to the Burnham Center is allowed after normal building hours using an authorized access card. Please contact the Office of the Building for information.

SECURITY

You may choose to unlock or lock your suite entrances during normal business hours. Although we try to maintain a secure working environment, many people enter the building every day and we cannot guarantee complete safety. You can take several preventive measures to keep your area more secure:

• Lock all doors when leaving your suite unattended.

• Instruct employees to keep valuables, including laptops, electronic devices, and

personal property in secured areas (locked desks, file cabinets or closets) when leaving areas unattended and over weekends.

• Always keep safes, strongboxes or similar devices locked, particularly when

unattended. Do not divulge combinations of safes or leave combinations where they can be found or easily deciphered.

• Record serial numbers of all valuable office equipment. If anything is stolen or missing, a record of serial numbers will aid in the recovery of the items.

There is 24-hour security seven days a week at the Burnham Center. If you are working after-hours, feel free to let security know you are on site. After normal business hours, please make sure that all entry doors to your suite are closed and locked.

PREVENTION

To reduce crime, emphasis should be placed on preventive rather than reactive measures. Preventive measures against office thefts and crimes against persons can best be achieved through the individual efforts of each employee. All suspicious or criminal activities should be reported immediately, first to the police department and then to the Office of the Building. Security protocol at the Burnham Center requires anyone exiting the building with large electronic items, boxes, or furniture to produce a property removal form signed by one of the property removal contacts for the suite. This form follows on page 15.

SOLICITING AND LOITERING

Canvassing, soliciting, peddling and loitering are not allowed anywhere on the property. If you are approached by a solicitor of any kind, or if you observe an individual engaged in such activities, contact the management office as soon as possible.

MOVING

GENERAL INFORMATION

All tenant moves must be coordinated with the Office of the B uilding. Please contact our office with your proposed moving date, and we will work with you to schedule the freight elevator. Freight rental is $55/hrly on weekdays. Weekends require a 4 hour minimum rental. In general, w e e k d a y moves can only be undertaken during non-business hours (4pm-7am) to simplify access to the building and minimize any disturbance to other tenants, but the Office of the Building will attempt to accommodate your schedule in every way possible. Moving companies must provide a certificate of insurance to the Office of the Building at least five days prior to the day of the move and according to the requirements appended below. Moving companies must also provide protection for building floors, walls, and elevators during loading and unloading. If you do not wish to hire a moving company, we can provide moving assistance at our labor rates. Weekends require a 4 hour minimum.

VENDOR INSURANCE REQUIREMENTS

Unless Contractor is only providing professional consulting services and no other services, Contractor shall maintain Commercial General

Liability coverage for itself and all Additional Insureds for the duration of all work performed and maintain Completed Operations coverage

for itself and each additional insured for the length of the state’s statute of repose.

On or before the date that Contractor begins any work, Contractor shall furnish to the Certificate Holder (defined below), insurance

certificates completed by a duly authorized representative of Contractor’s insurer stating all policy deductibles and certifying that the

Required Insurance is in effect and that Certificate Holder will be given not less than thirty (30) days prior written notice of any cancelation

thereof or material change thereto.

REQUIRED INSURANCE POLICY PROVISIONS

1. All policies shall be issued by an insurer having an A.M. Best rating of at least A-VII. 2. Unless Contractor is only providing professional consulting services and no other services, Contractor’s Commercial General

Liability policy shall contain an additional insured endorsements in ISO Form CG 20 10 11 85 or equivalent. 3. Contractor’s Commercial General Liability policy shall provide that such insurance is “primary and non-contributory” for the

Additional Insureds. 4. Contractor’s Commercial General Liability, Automobile Liability, Workers Compensation/Employers Liability and, if applicable,

Pollution Liability policies shall contain a provision waiving underwriters’ rights of subrogation against the Additional Insureds, their subsidiaries, affiliates, partners, officers, directors, and employees.

PLEASE NOTE: THIS DOCUMENT SERVES AS A REMINDER TO TENANTS AND VENDORS THAT BURNHAM CENTER RECOGNIZES ONLY “UNION” TRADE LABOR. This means that any vendors/contractors doing any type of work at 111 West Washington Street must be union affiliated. All vendors will be asked to provide union affiliation/identification card before the commencement of any such work is permitted. Vendors that are not union affiliated or cannot provide the proper identification will not be allowed into the building; there will be no exceptions

ALTERATIONS AND REMODELING

Alterations to tenant suites require the approval of the Office of the Building and may require a building permit. Requests to make alterations should be sent in writing to the Property Manager. Only contractors approved by the Office of the Building can perform alterations. All construction contractors including general, electrical, plumbing and phone contractors must register with the Office of the Building at least one week prior to performing any work in the building. Installing or maintaining systems that affect the building’s infrastructure, such as HVAC equipment, electrical, or communication equipment, computer or alarm systems, power or fire protection, must also be coordinated with and approved by office of the building.

No work can be performed without an approved certificate of insurance on file.
