    The purpse this guiebkWe created this guidebook to help you design, construct, anduse scaffolds safely. The guide highlights Oregon OSHAs scaf-fold requirements for the construction industry, Subdivision 3/Lof the construction safety and health rules. The guide identiesunsafe work practices and shows most types of scaffolds used inconstruction work. Use this guide as a supplement to Subdivision3/L, not as a substitute. You should become familiar with Subdivi-

    The s s temprry wrk ptrmA scaffold is simply an elevated platform that supports workersand materials. Lay a board across a couple of tall buckets and youhave a supported scaffold but not a safe one. Most scaffoldsused for construction work are complex structures and workersneed to know how to erect them, dismantle them, and work from

    them safely. Unsafe scaffolds endanger workers in many ways.Components can break, collapse, or give way. Planks, boards,decks, or handrails can fail. In some cases, entire structures havecollapsed. Even on sound scaffolds, workers can slip or lose theirbalance, and without appropriate protection, they dont have tofall far to get hurt. More often than not, scaffold accidents can betraced to untrained or improperly trained workers.

    Scaffolds can be complex structures. Too often, untrainedworkers use makeshift staging, overload platforms, fail to inspectsupports, or use scaffolds for the wrong tasks. This guidebookand Oregon OSHAs Subdivision 3/L scaffold rules can help you

    learn what you need to know so that a temporary work platformwont let you down.

    definitions

    Subdivision 3/L uses many special terms. Some of them may benew to you or confusing in the context of a requirement, so weveincluded important terms here. On Pages 9-10, youll nd detailedinformation on two particularly important terms, the mpetentpersn and the quiie persn.

    These terms are also dened in Subdivision 3/L, 1926.450(b).

    ajustbe suspensin s: A suspension scaffold with ahoist (or hoists) operated by workers on the scaffold.

    Berer: A horizontal transverse scaffold member, upon whichthe scaffold platform rests, that joins scaffold uprights, posts, andpoles.

    Btswins hir: A single-point adjustable suspension scaffoldconsisting of a seat or sling that supports one person in a sittingposition.

    By hrness: Straps that can be secured about the worker todistribute fall-arrest forces over the thighs, pelvis, waist, chest, andshoulders.

    Bre: A rigid connection that holds one scaffold member in a xedposition with respect to another member, building, or structure.

    Brikyers squre s: A supported scaffold made offramed squares that supports a platform.

    crpenters brket s: A supported scaffold consistingof a platform supported by brackets attached to a building orstructural walls.

    ctenry s: A suspension scaffold consisting of a platformsupported by two horizontal and parallel ropes attached to struc-tural members of a building or other structure.

    chimney hist: A multipoint adjustable suspension scaffold thatprovides access for working inside chimneys. See Multipointadjustable suspension scaffold.

    cet: A structural block used at the end of a platform to preventthe platform from slipping off its supports. Cleats also providefooting on sloped surfaces such as crawling boards.

    cmpetent persn: One who can identify hazardous workingconditions and who has authorization to take prompt correctivemeasures to eliminate them. For more information on the compe-

    tent person, see Page 9.

    cntinuus-run s (run s):A two-point or multipointadjustable suspension scaffold made from braced scaffold mem-bers or supporting structures that form a continuous scaffold.

    cuper: A device that locks together the tubes of a tube-and-coupler scaffold.

    ledgers and a double row of uprights not supported (except withties, guys, braces) by any other structure.

    Equivent: Alternative designs, materials, or methods that theemployer can demonstrate will provide an equal or greaterdegree of safety for workers than the methods, materials, or designsspecied in a standard.

    Eye r eye spie: A loop with or without a thimble at the end ofa wire rope.

    ft (ship) s: A suspension scaffold consisting of a bracedplatform resting on two parallel bearers and hung from overheadsupports by xed-length ropes.

    frm s: A supported scaffold consisting of a platform sup-ported by brackets attached to formwork.

    Gurri system: A vertical barrier consisting of toprails, midrails,and posts. Prevents workers from falling to lower levels.

    Hist: A manual or power-operated mechanical device that raisesor lowers a suspended scaffold.

    Hrse s: A supported scaffold consisting of a platform sup-ported by construction horses (sawhorses). Horse scaffolds madeof metal are also called trestle scaffolds.

    lieine: A exible line that connects to an anchorage at one endand hangs vertically (vertical lifeline) or that connects to anchor-ages at both ends and stretches horizontally (horizontal lifeline);it connects other components of a personal fall-arrest system tothe anchorage.

    lwer eves: Areas below the working level. Examples: groundlevels, oors, roofs, ramps, runways, excavations, pits, tanks,materials, water, and equipment.

    Mutieve suspene s: A two-point or multipoint adjust-able suspension scaffold with platforms at various levels that reston common stirrups.

    Mutipint justbe suspensin s: A suspension scaf-fold consisting of a platform suspended by more than two ropesfrom overhead supports that can be raised and lowered to desiredwork levels. Includes chimney hoists.

    Neee-bem s: A platform suspended from needle beams.

    open sies n ens: The edges of a platform that are more than14 inches away from a sturdy, continuous, vertical surface (such asa building wall) or a sturdy, continuous, horizontal surface (such asa oor), or a point of access. Exception: For plastering and lathingoperations, the horizontal distance is 18 inches.

    outrigger: The structural member of a supported scaffold that in-creases the scaffolds base width, increasing support and stability.

    outrigger bem (thrustut): The structural member of a suspen-sion scaffold or outrigger scaffold that supports the scaffold byextending the attachment point out and away from a structure.

    outrigger s: A supported scaffold consisting of a platformresting on outrigger beams projecting beyond the wall or face of astructure; the inboard ends are secured inside the structure.

    overhn brikying: Laying bricks and masonry units so thatthe surface of the wall to be jointed requires the mason to leanover the wall to complete the work.

    Persn -rrest system: An anchorage, connectors, and abody harness used to arrest a workers fall. May also include alanyard, deceleration device, or lifeline.

    Ptrm: A work surface raised above lower levels. Platforms

    are made of individual wood planks, fabricated planks, fabricateddecks, and fabricated platforms.

    Pe s: See Single-pole scaffold and Double (indepen-dent) pole scaffold.

    Pwer-perte hist: A hoist powered by other than humanenergy.

    Pump jk s: A supported scaffold consisting of a platformsupported by vertical poles and movable support brackets.

    Quiie persn: One who, by possession of a recognized degree,

    certicate, or professional standing, or who by extensive knowledge,training, and experience, has successfully demonstrated the abilityto solve problems related to the subject, the work, or the project.See Page 10 for more information on the qualied person.

    Rte : The manufacturers specied maximum load to be liftedby a hoist or to be applied to a scaffold or scaffold component.

    Repir brket s: A supported scaffold consisting of aplatform supported by brackets secured around the circumfer-ence or perimeter of a chimney, stack, tank, or other supportingstructure.

    R-brket s: A rooftop-supported scaffold consisting ofa platform resting on angular-shaped supports.

    Runner (eger r ribbn): The lengthwise horizontal spacing orbracing member that supports the bearers.

    S: Any temporary elevated platform (supported or suspended)and its supporting structure (including points of anchorage) thatsupports workers and materials.

    Se-ntine justbe s: A combination supported andsuspension scaffold consisting of an adjustable platform mountedon an independent supporting frame not a part of the object

    worked on. Examples: rolling roof rigs, rolling outrigger systems,and some masons adjustable supported scaffolds.

    Shre s: A supported scaffold placed against a structureand held in place with props.

    Singe-pint justbe suspensin s: A suspensionscaffold consisting of a platform suspended by one rope from anoverhead support and equipped to move the platform to desiredwork levels.

    Singe-pe s: A supported scaffold consisting of a platformresting on bearers. The outside ends are supported on runnerssecured to a single row of posts or uprights and the inner endsare supported by a structure.

    Stir twer (s stirwy/twer): A tower that contains inter-nal stairways and rest platforms. Used to provide access to scaffoldplatforms and other elevated points such as oors and roofs.

    St : The load at which a power-operated hoist stalls or thepower is automatically disconnected.

    Step, ptrm, n treste er s: A platform restingdirectly on the rungs of stepladders or trestle ladders.

    Stits: A pair of poles or supports with raised footrests, used towalk above the ground or working surface.

    Supprte s: One or more platforms supported by outrig-ger beams, brackets, poles, legs, uprights, posts, frames, or similarrigid support.

    Suspensin s: One or more platforms suspended by ropesor other nonrigid means from an overhead structure(s).

    System s: A scaffold consisting of posts with xed connec-tion points that accept runners, bearers, and diagonals intercon-nected at predetermined levels.

    Tnk buiers s: A supported scaffold consisting of a plat-form resting on brackets directly attached to a cylindrical tank or

    attached to devices that are attached to a tank.Tp-pte brket s: A scaffold supported by brackets thathook over or are attached to the top of a wall. Similar to carpen-ters bracket scaffolds and form scaffolds and used in residentialconstruction for setting trusses.

    Tube-n-uper s: A supported or suspended scaffoldconsisting of a platform or platforms supported by tubing, erectedwith coupling devices connecting uprights, braces, bearers, andrunners.

    Tubur wee-rme s: See Fabricated frame scaffold.

    Tw-pint suspensin s (swing stge): A suspensionscaffold consisting of a platform supported by hangers (stirrups)suspended by two ropes from overhead supports and equippedto raise and lower the platform to desired work levels.

    Unstbe bjets: Objects that could become dislocated, shift, andnot support the loads imposed on them. Unstable objects do notconstitute a safe base support for scaffolds, platforms, or workers.Examples: barrels, boxes, loose brick, and concrete blocks.

    Subdivision 3/L frequently refers to mpetent persns andquiie persns. The terms apply to those who have specialskills and are given specic responsibilities under Subdivision

    3/L rules.

    cmpetent persnA competent personcan identiy hazardous working conditions andhas authorization to take prompt corrective measures to eliminatethem.The competent person, who has primary responsibility forsupervising and directing all scaffolding erection, dismantling, andaltering work, must:

    Know Subdivision 3/L requirements applicable to the types ofscaffolds used

    Be able to identify and correct hazards encountered in scaffoldwork

    Be trained in the structural integrity of the types of scaffoldsused

    Have authority to promptly abate hazardous worksite condi-tions

    A competent persons duties can be shared as long as each personis qualied to perform the duty and has authority to abate hazardspromptly. Subdivision 3/L requires that only competent personsdo the following:

    Determine feasible safe access for persons erecting and dis-mantling scaffolds

    Inspect scaffolds and components for hazards before eachwork shift and after any event that could affect the scaffolds

    structural integrity Supervise and direct all scaffold erection, dismantling, and

    altering work

    Determine the feasibility of providing fall protection for eachscaffold erection and dismantling operation

    Determine whether it is safe to work on scaffolds during stormsor high winds

    Determine whether scaffold components made from differentmetals can be used together

    Determine whether scaffold components made by different

    manufacturers can be used together

    Identify the cause and signicance of a deteriorated scaffoldcomponent and correct the hazard

    Inspect ropes used in suspension scaffolds and identify defects

    Determine if multipoint suspension scaffolds need to be se-cured to prevent swaying

    Inspect manila or synthetic rope used for toprails or midrailsto make sure it meets the 200-pound capacity requirementsin 1926.451(g)

    NOTE: Many employer groups, vendors, apprenticeship programs,and labor organizations offer training on Subdivision 3/L scaffold-ing requirements. However, attending one of these programs doesnot necessarily make one competent (or a competent person).Competency must be demonstrated; its usually the result of manyhours of in-class training and on-the-job experience.

    Quiie persnA qualied person has a recognized degree, certifcate, or proessionalstanding or by extensive knowledge, training, and experience, hassuccessully demonstrated the ability to solve problems related to thesubject, the work, or the project.

    Scaffolds must be designed by a qualied person but not necessar-ily by an engineer. Exceptions: Connections for masons adjustablemultipoint suspension scaffolds, pole scaffolds more than 60 feethigh, coupler and fabricated-frame scaffolds more than 125 feethigh, and outrigger scaffolds must be designed by a registeredprofessional engineer.

    S pitiesScaffolds must be able to support their own weight and at least fourtimes the maximum intended load applied to them. Unless otherwisenoted in Subdivision 3/L, scaffold components have to meet the4-to-1 safety factor too, but only for that portion of the maximumintended load applied to them. The maximum intended load fora component depends on the scaffold type and its conguration.

    Note that scaffolds and components must be able to support fourtimes the maximumintended load notthe rated load. The in-tended load includesworkers, equipment,and supplies. The in-tended load shouldnever exceed therated load unless thedesign is approved byan engineer and the

    manufacturer.

    Direct connections and counterweights used to balance adjustablesuspension scaffolds must resist at least four times the tipping forceof the scaffold. A competent person who directs the rigging of thescaffold must calculate the potential loads.

    Safety factors for the counterweights, riggings, and direct con-nections to roofs, oors, and suspension ropes of adjustablesuspension scaffolds should be based on the rated load andthe stall load of the hoist, not the maximum intended load.

    The stall load of any scaffold hoist cannot exceed three times itsrated load. This safety factor ensures that suspension-scaffold sup-port systems are not overloaded. 1926.451(a)(5)

    The front edge of a scaffold platform cannot be more than 14inches from the face of a structure unless guardrails or personalfall-arrest systems are used to protect workers from falling betweenthe structure and the platform. There are two exceptions to thisrequirement: (1) the front edge distance for outrigger scaffolds mustbe no more than 3 inches, and (2) scaffolds used for plasteringand lathing work can be no more than 18 inches from the face ofa structure. 1926.451(b)(3)

    A platform 10 eet or less in length must extend at least 6 inches,

    but no more than 12 inches, beyond its support unless the excesslength is guarded or can support workers and material withouttipping.     When platform planks are abutted to create a long platform, eachabutted end must rest on a separate support. Abutted planks touchend to end on separate support surfaces; they do not rest on oneanother. 1926.451(b)(6)

    Platform planks overlapped to create a long platform must over-

    lap at least 12 inches over supports unless the planks are nailedtogether or otherwise restrained so they do not move. 1926.451(b)(7)

    Any platform that rests on a bearer at an angle other than a rightangle must be laid rst. Platforms that rest at right angles over thesame bearer must rest on top of the rst platform. Make sure thatall platform planks are fully bearing to eliminate potential instabil-ity. 1926.451(b)(8)

    Pint (pque) inishes

    Wood platforms cannot be covered with opaque nishes, becauseopaque nishes cover defects in wood. Wood platform edges,however, may be marked for identication. Preservatives or slip-resistant and re-retardant nishes are acceptable as long as thenish does not cover structural defects or make them hard to spot.1926.451(b)(9)

    Scaffold components made by different manufacturers cannotbe mixed unless they t together easily and do not change thescaffolds integrity. Components made by different manufactur-

    ers cannot be modied to intermix unless a competent personapproves. 1926.451(b)(10)

    Scaffold components made from different metals cannot be usedtogether unless a competent person approves. If a competentperson determines that mixing components made from differ-ent metals could reduce their strength, the employer must takecorrective action. If a competent person cant make the determina-tion, then different metals must not be used. 1926.451(b)(11)

    aess t ssEmployers must provide all workers with safe access to scaffoldsand scaffold platforms. Workers must use ladders or stairwaysto reach platforms that are more than 2 feet above or below theaccess point. Do not usecross braces as a means ofaccess. Note that permanent

    stairways or portable laddersmust meet the requirementsof Subdivision 3/X (stairwaysand ladders) of the con-struction safety and healthstandards.

    A personal fall-arrest system consists of an anchorage, connectors,and a body harness. It may also include a lanyard, decelerationdevice, or lifeline. Persons who work from a boatswains chair,catenary scaffold, oat scaffold, needle beam scaffold, or ladder jack scaffold must be protected by a personal fall-arrest system.1926.451(g)(1)(i)

    On straight-run scaffolds, make sure longitudinal bracing acrossthe inner and outer rows of posts is installed diagonally in bothdirections. The bracing must extend from the base of the endposts upward to the top of the scaffold at approximately a 45-degree angle.

    When you cannot attach bracing to posts, attach it to the runnersas close to the post as possible.

    Install bearers transversely between posts. Extend bearers beyond the posts and runners. They must have

    aeri itsAn aerial lift is an extensible or articulating device designed to positionworkers and handle materials. Aerial lifts can be powered or manuallyoperated and do not necessarily have to rotate about a vertical axis. Sub-division 3/L covers only vehicle-mounted elevation and rotating aeriallifts (ANSI A92.2 device). However, Oregon OSHA has added additionalrules to Subdivision 3/L covering three other types of aerial lifts:

    Effective training programs dont just happen. They requirecareful planning, explicit goals and objectives, dedicatedinstructors, and motivated students. It doesnt matter whether

    the activity is athletics, academics, or occupational safety andhealth. The underlying training concepts are similar: (1) design atraining program, (2) conduct training, (3) evaluate training effec-tiveness, and (4) improve training through feedback.

    Employers must establish procedures to ensure that aworker who falls from a scaffold receives immediate atten-tion. Emergency procedures should be fully documented

    Talk to the victim; determine the victims condition, if possible.
