The operation of the Euorpean Network for Smoking Prevention was started on time and has followed the general work plan outlined in the present grant agreement. Set objectives were generally reached. Moreover, the means for implementation of the programme could be increased while the actual project cost was reduced by 25%. The total expenditure during the project period was 394.357,23 Euro. During the project period the membership of the network increased from 342 to 382 network members. 4 new national coalitions joined the network (Norway, Switzerland, Iceland, Check Republic. A new Italiant Coalition was created. The number of theme based European networks was increased by the Tobacco Control Resource Centre working with Medical Associations. In October 2001, ENSP has moved offices to new, larger premises (144, chaussée d’Ixelles) and has installed a new computer system. Activities were monitored by the Executive Board who met 6 times and held 3 conference calls. A first ENSP network meeting/general assembly drew together 50 participants from 19 countries in Tuusula (Finland) on 16/17 November 2001. The general theme of the meeting was the future challenges of tobacco control for the health community covering socio-economic aspects, inequalitites and health, workplace health promotion, consumer issues and litigation. An important result of the meeting was the adoption of a common input from NGOs to the development of the WHO 4th action plan for a tobacco free Europe. The second network meeting took place in Athens on 19/20 Arpil 2002 drawing together 70 participants from 19 countries. The first part of the meeting discussed Smoke free workplace policies in Europe and related tools as well as the dangers of ETS for workers with asthma and respiratory diseases. The second part of the meeting dealt with the EU media campaign on youth smoking prevention. Strong concern was voiced by the participants on the chosen campaign concept/logo. The assembly called for the stablishment of an expert panel of health professionals to monitor year 2 and 3 of the campaign. A letter was sent by the conference participants to Chancellor Schröder expressing their consternation and requesting the annulment of a 11,8 million grant by the tobacco industry to the Germany health ministry to finance youth prenvetion campaigns. ENSP accounts 2001 as well as the budget and working priorities for 2002 were adopted. Concerning EU policy development, the main emphasis was put on generating support for a strong EU position in the intergovernmental negoations for a framework convention on tobacco control (INB3 and INB4) and on developing a co-ordinated approach to the proposed EU adverstising directive. ENSP continued tartgeted dissemination of information on the European status report on smoke free workplaces and initiated the drafting of smoke free workplace policy recommendations. Major input was given by ENSP to the new EU public health programme with a view to promote tobacco control as a priority on the European health agenda 2003-2008. Through permanent contacts via phone fax, e-mails and 11 meetings with national Coalitions and specialised networks, ENSP promoted the co-ordinated advocacy effort to establish a comprehensive European tobacco control policy. Information exchange was strengthened by employing a full time information officer as of April 2002. The ENSP website was redesigned to become an interactive tool easy to update

and to use. We switched from producing 4 hard copy newsletters a year to the publication of dynamic weekly European newsbulletins of which 15 were published between May and September. Additional newsflashes were produced in the form of 20 information releases on important developments on the European tobacco control scene. 13 major international conferences were attended by the ENSP staff, promoting the visibility of tobacco control in general and with peripheral health promotion networks in particular. NGO positions on tobacco control issues were promoted in meetings organised by the European Union and the World Health organisation. Between September 2001 and September 2002, ENSP has managed simultaneously three framework projects and initiated one framework grant application. Project building evolved towards a more policy oriented approach. Extensive administrative support and policy advice was given by the secretariat to the project leaders to allow smooth implementation of the projects. A consolidated final report for the grant application 2000 was introduced in December 2001, the framework project 2001/2002 was implemented, the Commission grant agreement for the year 2002/2003 was finalised and a consolidated grant application for the new EU public health programme for the period 2003-2006 was initiated comprising 7 potential subprojects. Unfortunately, under the new pubic health programme funds only reduced funds will be available for tobacco control. Efforts will be undertaken to secure additional funding for projects from the Community Tobacco Fund (a percentage of EU subsidies paid out to tobacco growers which is earmarked for partly for smoking prevention and partly for reconversion of farmers to other crops. Several new ideas came up during the project period to increase the efficiency of tobacco control activities: the necessity to increase work with the media in order to better disseminate knowledge about tobacco control issues among policy makers and the general public and increased contacts with national coalitions to promote exchange of best practice with a view to implementation of policy and legislation. Also, more work has to be done to create a horizontal approach to tobacco control with a view to socio economic determinants and their influence on the effectiveness of tobacco control measures.

Back ground information on ENSP The framework project 2001-2002 consisting of 8 subprojects is co-ordinated by the ENSP central secretariat in Brussels as overall project leader. The present covers the ENSP core grant for network activities. Separate final reports are produced for each of the other 7 subprojects. ENSP was created in 1997 as an international non profit association under Belgian law – registration Nr. 16377/97 with the aim to develop a co-ordinated approach to tobacco control activities on Europan level. The ENSP General Assembly consists of 38 full voting members which are elected delegates of the 19 national coalitions and 4 themebased networks established in the European Union, in EFTA countries and EU accession states.Between September 2001 - 2002, ENSP operated under the responsibility of an executive Board , regularly elected by the General Assembly. The Executive Board has delegated the co-ordination of the network and the daily management to its Secretary General, Sibylle Fleitmann. In 2002, ENSP draws together 382 member organisations. FUNDING Initially the ENSP Budget was foreseen for à 15 months period to allow for finalisation of reports to the European Commission. These 15 months were reduced to a 12 month activity period to connect with a follow-up grant from September 2002 to September 2003. OBJECTIVES

and support the development of effective legislation on national and European level, to create interaction and synergy between non governmental and governmental agencies on national and international level, to actively support the work of organisations in the EU accession countries and facilitate transfer of technology:

MEANS TO IMPLEMENT OJBECTIVE 1 Objective 1: To intensify exchange of up to date information and best practice, to promote and support the development of effective legislation on national and European level, to create interaction and synergy between non governmental and governmental agencies on national and international level, to actively support the work of organisations in the EU accession countries and facilitate transfer of technology: 1. Management of a central secretariat in Brussles

The permanent ENSP secretariat has moved on 17 October 2001 and is now established at 144 chaussée d’Ixelles , 1050 Brussels, Belgium. This change of location from a 80 m2 office to a 144 m2 office was made necessary by the extension of the network activities and the engagement of a full time information officer as of 20 March 2002. During the project period, the network employs a full time Secretary General , (Mrs. Sibylle Fleitmann – D/B) a full time financial co-ordinator (Mr Francis Grogna -B), a part time administrative assistant (Mrs Sophie Van Damme - B) and from 25.March onwards a full time information officer (Sophie Kazan – UK). The position of full time information officer had been advertised in Brussels based international publications (September 2001 in the “European Voice” , in January 2002 in the “Bulletin” ). The selection procedure was delayed until signature of the present grant agreement (November 2001). Contracts for ENSP staff are established in compliance with the Belgian law. The permanent office has fulfilled the administrative obligations towards the Belgian State and the administrative follow-up of the EU contract obligations. In September 2002, ENSP consisted of 383 member organisations (342 the previous year) operating in the framework of national coalitions against tobacco or specialised European networks. Between Sept. 2001 and Sept. 2002, 4 national coalitions( Swiss coalition against tobacco , the Norvegian Coalition against Tobacco, the Iceland Coalition against Tobacco, the Check Coalition against Tobacco ) and 1 specialised European network (European Union of Non Smokers) applied for full membership and were accepted by the ENSP General Assembly as such, raising the total number of coalitions created through the activities of ENSP from 15 to 19 and the number of specialised networks from 3 to 4.

2. Working with national coalitions /European networks Through permanent contacts via phone fax, e-mails and perosnal visits, calls for action and invitation to meetings, national Coalitions and specialised networks became the main actors of the co-ordinated advocacy effort to promote European tobacco control policy. Belgium ENSP was invited to attend coalition meetings in order to update on recent developments in tobacco control policy on EU level. In January, we presented the findings of the European status report on smoke free workplaces and advocated development of comprehensive legislation on Belgian level. SF was requested to accompany Michel Pettiaux to present the findings of the report to the Belgian Ministry of Social Affairs. September 17 Brussels Belgian Coalition against Tobacco January 22 Brussels Belgian Coalition against Tobacco Poland On invitation of the organisers, ENSP was invited to give input in the planning of the 3rd European conference on tobacco or health to be held in Warsaw from June 20 – 22, 2002. This was an opportunity to develop links with the Polish Health Promotion Foundation and prepare collaboration in the frame of a national coalition against Tobacco in Poland in the future. October 15 Warsaw 3rd European conference on tobacco control Greece The aim of the visit was to motivate the Greek players of the national coalition to engage in co-ordinated advocacy activities and to promote their participation in the European tobacco control effort with special emphasis on the FCTC negotiations. This informal coalition had lost some of its motivation to work together through internal disputes. By asking them to organise the forthcoming ENSP network meeting in April in Athens, the members were enthousiastic to find new forms of collaboration. February 27/28 Athens Present: 10 organisations , members of the Coalition Denmark A visit was made to the Danish Coalition to discuss the structure and agenda of the forthcoming ENSP network meeting in Helsingor in order to have a maximum impact on Danish tobacco control policy and to trigger interest from Danish coalition members May 16 Cophenhagen preparation of ENSP network meeting Oct 2003. Germany A special effort was made to involve German organisations closer into the European Tobacco Control effort with a view to increase NGO activity within the national coalition and to reduce Germany’s opposition to a ban on advertising on European/FCTC level. September 26 Helsinki: Strategy discussion with a restraint project group June 7/8 Rome ENYPAT advisory board meeting European Health Networks (ENWHP,ENHPA, EUMAHP/EUHPID,ENSP,EPHA,ENMHP,ENWHP, ENMHPO,MEGAPOLESENMHP) As the new public health programme has adopted a horizontal approach across all health determinants and between programme strands, the European health networks receiving Commission funding met several times in order to develop an integrated and horizontal approach to grant applications for project funding. April 17 Brussles 10 health networks May 29 Brussels 10 health networks 3. 0rganisation of Executive Board meetings Five Board meetings had been scheduled initially. However, the need arose to organise one additional Board meeting, 3 conference calls and one informal breakfast meeting to allow the new Board to be closely involved in the ENSP activities. • ENSP Board meeting – Helsinki (Tuusula) 16 November 2001 All Board members were present. The practical organisation of the General Assembly was discussed and fine tuned and ENSP publications “European Status Report on Smoke free Workplaces “ and the “ENSP 2000-2001 Overview of Activites” were presented.before distribution to the General Assembly. • ENSP Board meeting – Brussels 4 February 2002 Juan Ramon Villalbi could not attend the board meeting. Following issues were discussed during the meeting: • Activity report SF and update on current developments (framework project - budget

ENSP - change of internal rules – new members ) • Working priorities 2002: development based on the ENSP evaluation and the Overview

of activities 2000/2001: • EU activities: FCTC – advertising directive – tobacco control directive – Tobacco fund –

new public health programme – WHO 4th action plan • Preparation of the next ENSP Conference and General Assembly

(Athens, 19/20 April 2002) highlights: workplace smoking prevention – youth smoking prevention – EU policy development - general assembly – working priorities – election to the Board – Adoption of 2001 accounts - 2002/2003 budget.

• ENSP Board meeting – Athens 18 April 2002 Göran Boëthius was excused for absence. The meeting was essentially structured around the practical organisation of the ENSP network meeting and the approval of the annual accounts. • ENSP Board meeting – London June 14, 2002 This was the first board meeting of the newly elected Board and all members were present. The meeting was organised in London to co-incide with an IUHPE policy conference. The discussion was centered around following subjects : • Appointment of officers : Vice president, Treasurer

• Breakfast meeting in Warsaw June 22, 2002 Discussion on ENSP activities and follow –up on actions taken • Board conference call June 29,2002 All members present - The Board decided on a restructuring of information collection/dissemination for a 6 month trial period. The Board expressed the wish to be more closely associated to ENSP activities and will therfore receive weekly activity reports. The Board apppointed Juan Villalbi as the ENSP representative of the Eu media campaign expert panel. • Board conference call July 16,20002 All members present – The discussion centered around the results of the meeting between ECL-ENSP-ERS –ENH and Commissioner Byrne that had taken place on 12 July, which was considered as a very positive encounter. • ENSP Board meeting August 9, 2002 Bertrand Dautzenberg was excused and did not participate in the meeting. • The Board approved the lay-out and choice of co-ordinator for the publication of the

2000-2001 project results and were informed on the composition of the EU media campaign expert panel

• The board approved the strategic plan (priorities proposed) and the project building procedure (internal call for proposal – evaluation – project building workshop) in view of developing a Framework Project grant application for 2003 – 2006 .

The network delegates and observers come together biannually at the ENSP network conferences. These conferences focus on key topics relating to tobacco control issues in Europe. European and international specialists are invited, forming a panel of experts able to speak with proficiently on the chosen topics. The objective of these Network Conferences is information exchange and the consolidation of knowledge, with the ultimate aim of building consensus. This is quite a challenge when considering the different cultural, political and legislative backgrounds of the countries involved. Conferences result in common declarations on topics of particular concern. By developing consensus amongst European representatives the network creates a powerful and united force that shapes tobacco control policy. ENSP Network meeting - November 16/17 2002 – Tuusula (Helsinki) Finland – 50 participants The aim of this meeting was to consider future challenges for tobacco control in Europe covering socio-economic aspects, inequalitites and health, workplace health promotion, consumer issues and litigation. International speakers from various backgrounds illustrated the subjects as per attached conference programme Two ENSP framework project partners reported on their findings (Euroepan Network of Smoke free Hospitals and European Network of Quit Lines). Three parallel workshops were organised to deliver NGO input on the future of the WHO 4th Action plan for a Tobacco Free Europe. The General Assembly elected one new Board member: Prof. Bertrand Dautzenberg , France and re-confirmed the co-option of Dr Göran Beoëthius, Sweden. Mrs. Marie Paule Prost resigned from her postion of co-opted member. A change of internal rules was adopted, giving the possibility to exclude coaltions and networks or their delegates that are not attending the network meetings and are not participating in the network activities. This will allow to recruit new membership to safeguard efficiency of ENSP activities. The Swiss and Norwegian Coalition were adopted as full new voting members of the General Assembly (Annex Report Tuusula)

ENSP Network meeting – April 18/19 2002 – Athens Greece – 70 participants Authorisation from the Commission had been requested to organise this meeting in Athens instead of Brussels in order to motivate increased participation of Greek members in the ENSP networking effort. The main themes discussed at the meeting were the issue of smoke free workplaces, Youth prevention campaigns with a focal point on the EU media campaign, an update on EU policy development and the ENSP general assembly with the adoption of the annual accounts 2001, adoption of the budget 2002 and an election to the Board. The smoke free workplace session consisted of high level presentations on the effects of ETS onasthma by Dr NG. Papadopoulos, the Massachusetss experience by Greg Connelly, the WHO employers guide by John Griffith (WHO partnership project), the Trade Union’s project on tobacco control by Carola Grotzki and a presentation by J. Tostain on the need for protection of non smokers at the workplace (the European Union of Non smokers) Youth prevention campaigns were introduced with a report by WHO on the World No Tobacco Day 2002 “Sports without tobacco”, the EU Campaign presentation by the European Commission/Media Consulta. A restraint briefing for networks had already taken place the day before (17TH of April ) on request of the Commission. Greg Connolly presented the Massachusetts experience with youth campaigns with reference to the EU media campaign and the ESFA project results were presented to give input in the discussion. At the end of the session Prof. Wiebel made a presentaiton on wether sales to minors should be limited and if yes to what age limit? The discussion resulting from this session was extremely intensive and resulted in severe criticism of the EU media campaign strategy compared to available evidence. A letter was sent to Commissioner Byrne (annex) on behalf of the conference participants requesting that the Commission set up a health expert group to monitor the development of the campaign with the request to enlarge the scope of the campaign according to a change of the social environment of young people rather than targeting young people directly as done by the Tobacco industry. Furthermore, the participants agreed to send a letter to Chancellor Schröder (Annex) with a request not to compromise with the tobacco industry. The German Health Ministry had accepted a grant of 11,8 million Euro over five years from the German tobacco industry to finance youth prevention campaigns under the condition not to encriminate the product or to damage the image of the tobacco industry. 5. Management and update of an ENSP web site and ENSP conference site As of 20th March 2002, a new information officer started office. The ENSP website was redesigned and restructured and the information dissemination was streamlined. • ENSP interactive website Between March and September 2002, ENSP has been transforming its web site into a more up-to-date and interactive web site that can be used as a point of reference and information by tobacco control organisations primarily, but also by people who are interested in smoking prevention. The web site will be interactive in that parts of it can be updated instantly by the ENSP Information Officer by email from the ENSP office (or anywhere in the world) using a programme called ColdFusion. Design Below is the design of the first/home page of the site. The guidelines set for the designer were that the site should be interactive and clear with a contemporary ‘feel’. We not only want people to visit the site frequently and find useful and clear information quickly, we also want them to feel comfortable and enjoy the experience (and come again).

Contents As well as bringing the current content up to date, we have also sought to represent some of our new roles (publication of the European News Bulletin) and expand upon the materials available on our site ( by including links to reports by other institutions in our ‘Reports’ section). Since the web site is being conceived with our European tobacco control colleagues in mind, we sent out a survey to 45 coalition representatives who had attended the Athens network meeting in April to let them know that we are working on a new site and to gauge how useful it would be to them. Out of 45 coalition representatives to whom we sent the survey, only 6 replied. However scarce, their opinions confirmed the envisaged structure especially with a view to a restricted access section for ENSP members only and Country Profiles’ or a similar section linking to good tobacco sites in the EU. It was also suggested to co-ordinate with Globalink regarding similar contents and access. The website is functioning since September 2002. Restricted access section We propose a ‘restricted access’ section within our web site, so that it can house general information as well as information releases and activities that we would not like to get into the hands of the tobacco industry. Some or all of the information releases will be stored in this restricted area. This section will also allow to reduce postage cost for conference/meeting documents . Following with the idea of ‘interactivity’ and with our francophone readers in mind, we have purchased the site: www.rept.org (ENSP in French is REPT) in the next few weeks and linking it to the contents of the www.ensp.org site. • ENSP conference site –interactive information exchange In September 2002, 570 people (340 on the ENSP globalink list + 230 on a separate ENSP list) are linked to our conference site and participate in an active information exchange . European News Bulletin – 15 editions From June 2002 onwards, ENSP has agreed to take on the production of an electronic, weekly European News Bulletin (ENB) in replacement of the planned 4 European Newsbulletins in hard copy. Between March and September 15 Eurpean News Bulletins were published. Commissioner Byrne has requested to be copied personally on the European News Bulletin. What is it? The European News Bulletin is a weekly mailing containing the main articles from the international press that relate to tobacco control. The bulletin is destined for a list of subscribers (via globalink) and individuals on the ENSP mailing list. Structure It was decided to set out the mailing, so that it is consistent and can be read by all browsers as easily as possible. In addition, we will refine the contents to the European region (and accession countries), to include as much news as possible on EU legislation, WHO policy

development etc. and also to include URLs or the names and locations of other articles for readers who would like to know more. Content and collaboration The articles are collected daily from a large variety of sources – mainly via the internet – and the main stories are selected at the end of the week and inserted into the European News Bulletin template. Some articles are shortened due to space constrictions with a URL leading to the full text at the end of the article. We have co-ordinated with the editor of the French News Bulletin (FNB), Emmanuelle Beguinot. Sinead Jones of the Tobacco Control Research Project is editing the scientific news. Information Releases – 20 newsflashes What is it? An up-to-the minute mailing (no more than 1 per day) presenting information on legislation or current affairs, press releases, calls to action, news from local coalitions etc. that relate to tobacco control issues. Depending on their content, these releases are also being sent to other individuals such as European press contacts. Also depending on their content, the information releases will be sent out by the Information Officer or the Secretary General of ENSP. We will be encouraging recipients to send feedback to let us know how useful they find the releases, which will be subject to regular assessments and modifications if necessary. In future, we feel that the ENSP Information releases will rely strongly on the ENSP web site and this will allow our ability to obtain and disseminate information to grow considerably in the future. ENSP Information releases may provide links to pages on our web site or refer to other articles/reports on the site, and this will be not only help to generate traffic onto our site, but it will also provide help to disseminate information from the site. We hope that the ENSP Information releases and the web site will become a vital tool not only at the service of all those involved closely in the struggle against tobacco, but also an important resource of information for those with a passing interest in the subject, such as the press or researchers. This is why we propose a ‘restricted access’ section within our web site, so that it can house general information as well as information releases and activities that we would not like to get into the hands of the tobacco industry. Some or all of the information releases will be stored in this restricted area. 6. Attendance at conferences and/or fairs Instead of 5 planned conference, 13 conferences were attended during the period covered by this report. ENSP has made contributions to international networking with intergovernmental organisations and on issues related to EU tobacco control policy development in each of them. A great emphasis was put on networking with organisations from EU accession countries. • September 28 Brussels ENHPA – Project “Inequalities” In preparation of the ENSP network memting of Nov. 16/17 in Tuusula (Finland) connections were established with the European Network of Health Promoting Agencies. ENSP has been invited to me a member of the European Health Policy Forum set up by the European Commission. This forum is a European consultation mechanisme for non for profit stakeholdes active in public health on European level.

• December 13/14 2001 Prague WHO Partnership Project meeting

As a member of the project group, ENSP was invited to attend this meeting. The aim of the meeting was to present the output of the WHO European partnership project to reduce tobacco dependence which will end after a three year activity period. A wide audience mainly from Central and Eastern European countries attended the meeting and allowed to enlarge the circle of organisations interested to participate in the European tobacco control effort. ENSP presented the findings of the European Status report on Smoke Free workplaces.

• February 17-19 2002 Warsaw WHO Ministerial conference The purpose of this 2 day event, organised by the WHO Regional office for Europe, was to engage the 51 countries of the European Region into a European strategy for tobacco control by way of a common commitment to further action through a « The Warsaw Declaration » . The conference was attended by high level representatives of 46 member states (among which the Prime Minister of Poland, 22 Health Ministers or Secretaries of State) , the European Union, the World Bank, the International Labour Organisation and representatives of 15 European non governmental organisations.ENSP was invited to speak on behalf of European NGOs and state their position on the FCTC negotiations. . • March 18-23, 2002 – Geneva – Framework Convention negotiations ENSP participated in this 4th meeting of the intergovernmental negotiation body (INB4) which took place from 18-23 March in Geneva. NGO participation from Eurpoe was effective and well organised. The participation of NGOs from the European Region has increased considerably with delegates present from: Germany; France; Sweden; Finland; Switzerland; Bulgaria; Lithuania; Russia; Spain and the UK. Also present were major networks such as: the European Respiratory Society; Association of European Cancer Leagues/UICC; European Representative of the World Dentist Federation and the World Federation of Public Health Associations; European representative of IULTD and IUHPE, and of course the European Network for Smoking Prevention. • April 21-24, 2002 – Warsaw – CEE Smoke out conference – Zoros Foundation ENSP was an invited speaker to inform on EU possibilities of funding for accession countries. This was an opportunity to meet NGO, ministerial representatives and a media representative of each of the accession countries and to make contact for further collaboration in the ENSP network activities.

• June 11-13, 2002 – London – IUHPE public health policy conference This conference was an opportunity to promote tobacco control in the public health community and to learn more about the general policy context of public health, especially with a view to health impact assessment, a new tool to implement tobacco control policy.

• June 17/18 , 2002 Bonn, Germany – Partnership Project conference ENSP was invited as a speaker to give the European view of tobacco control with a special view of the role of Germany as a blocking element on EU and FCTC level as far as a total ban on advertising is concerned.

• June 20, 2002 Brussels European Health Policy Forum

Organised by the European Commissoin as a forum of stakeholders active in public health, ENSP had given special input on the development of a statement concerning the integration of accession countries in the new public health programme.

• June 19-20, 2002 Warswa 3rd European conference on Tobacco or

Health ENSP had participated in the definition of the conference programme and was an invited speaker in a session organised by the EuropeanCommission to illustrate EU funding in tobacco control. ENSP participated as chairman of a workshop on smoke free workplaces and in the closing session round table expert panel.

• August 23/24,2002 Berlin INWAT seminar on women smoking and inequalities

ENSP attended this expert meeting to gain insight into the issue. • September 15-18 Montreal Première conférence mondiale francophone ENSP had special permission to attend this meeting in Canada. ENSP made a presentation on the ENSP network structure and functioning of the European Union. An information release was sent to out immediately to announce the conference conclusions and the next steps towards a francophone tobacco

RESULTS OF PROJECT ACTIVITIES -OBJECTIVE 1 • to intensify exchange of up to date information and best practice The engagement of a full time information officer strengthened information activities and the dissemination of best practice. Information was collected through Internet and media research, personal contacts and 11 visits with national coalitions and networks, as well as through participation in 13 international conferences and attendance of European Parliament sessions. Activities were monitored by the Executive Board who met 6 times and held 3 conference calls. Information exchange culminated in 2 network conferences, in Athens (Greece) and Tuusula, (Finland )strenthening exchange of best practice covering socio-economic aspects, inequalitites and health, workplace health promotion and consumer issues and litigation.Consensus was built on FCTC negotiations, the new EU advertising directive. and on collaboration among stakeholders of the EU media campaign on youth smokig prevention. The ENSP website underwent major restrucuring and was totally redesigned to become a userfriendly, fast moving and interactive tool for information exchange and dissemination of best practice. 15 Eurpoean news bulletins were published reporting on tobacco control and tobacco industry activities from all over Europe. Commissioner Byrne requested to personally receive the European news bulletins. 20 Information releases on topical themes were published which were instrumental in initiating co-ordinated advocay activities. Commissioner Byrne requested to be perosnally on the distribution list for the European newsbulletin and the information releases. During the project period, ENSP increased its membership from 352 to 382 member organisations. This was due to the creation of new coalitions in Norway, Iceland, Switzerland and the Check Republic increasing the number of national coalitions against tobacco in Europe from 15 to 19 organisations deveoping a co-ordinated tobacco control strategy on national level. The European Network for Medical Associations (Tobacco control Research Centre ) joined also as full member of the ENSP General Assembly. • to promote and support the development of effective legislation on national and

European level Main issues followed were the development of the EU position on the International Framework Convention on Tobacco Control (FCTC) and the development of the new proposal for an advertising directive, replacing the directive annulled by the European Court of Justic in October 2000. Advertising Directive : The EU advertising directive proposal (replacing the annulled directive 98/43/EC) only covered indirect advertising with transborder effect taking into account provisions made by the ECJ judgement. Member organisations of ENSP advocated for additional clauses which would ensure

• Protection for existing and future Member State legislation; • Extension of the Directive to cover indirect, as well as direct tobacco

advertising;

ENSP participated in the elaboration of an NGO version of an effective advertising directive. This NGO proposal for an effective directive was endoresed by ENSP member organisations and disseminated to policy makers on European and national level. ENSP attended the public hearing organised by the Legal Committee (in charge to draft the report for the European Parliament) on April 15th. and reported to the ENSP mailing list on relevant meetings which were held in the European Parliament on this subject. Due to delay tactics, the Legal Committee’s draft report was only adopted in October 2002. A campaign in support of the directive only took place in November 2002. Framework convention on tobacco control (FCTC ): INB 3 • European health NGOs supported the position of the international Framework Convention

Alliance , an international coalition of more than 160 organisations and networks world wide active in tobacco control. The FCA had developed text recommendations on main issues of the FCTC , which were adopted unanimously by the European NGOs with one exception: the definition of advertising should be all inclusive and not make distinction between direct and indirect advertising. These recommendations were adopted in October 2001. This information was transmitted by ENSP to the Commission Services, to the members of the Health Council and through national coalitions and specialised networks to their governments.

In their network meeting of November 16/17, the members of ENSP adopted a text proposal calling upon the Euruopean Union and its member states “ to give health a chance” and to show political leadership for a strong Framework Convention on Tobacco Control.

From November 22 – 24, 2002 ENSP attended the Intergovernmental negotiations in Geneva. On ENSP request the Commission representatives together with the Belgian Presidency agreed to give a briefing session on the EU position for NGOs . It was the first delegation to adopt this open approach to NGO consultation and was vastly appreciated. It set the example for consequent briefings from other delegations. A report on the negotiations was disseminated to the international health community and the ENSP mailing list. (Annexe INB 3 report)

INB4 • On February 1, 2002 the European Commission in Luxembourg organised an NGO

confernce on the FCTC development , following a request from ENSP . We had the opportunity to make a presentation on the importance of including indirect advertising in the advertising provisions of the FCTC text.

ENSP was invited to participate in the WHO Ministerial Conference for a tobacco free Europe on February 18-19, 2002 in Warsaw . ENSP was requested to develop the NGO position on the FCTC. The conference was attended by high level representatives of 46 member states (among which the Prime Minister of Poland, 22 Health Ministers or Secretaries of State) , the European Union, the World Bank, the International Labour Organisation and representatives of 15 European non governmental organisations. The conference was considered as a milestone in European tobacco control.

On 6 March 2002 ENSP endorsed a 1 page resumé of revised NGO text recommendations that was developed on international level for INB4 .ENSP dessiminated this information to the European Commission and the members of the Health group. The resumé was also disseminated through national coaltions and specialised networks to national government authorities and delegates of INB4. ENSP attended the negotiations from18-23 March 2002 in Geneva, collaborating actively within the Framework Convention Alliance. ENSP made a lunch time presentation to delegates on Passive Smoking. A report on the negotiations was circulated to the international Health Community and the ENSP mailing list. (Annex INB 4 report) • to create interaction and synergy between non governmental and governmental

agencies on national and international level Other EU issues: • EU Public Health programme ENSP gave active intput during the develoment of the new EU public health programme, emphasising the need for a consolidated tobacco control policy and continued support for NGO networks to ensure input from the civil society into the EU policy development. ENSP had close contacts with Prof. Trakatellis , rapporteur of the European Parliament during the adoption procedure between September and December 2001 , informed Mr.Sauer and the European Health Pollicy Forum of the perceived needs for tobacco control in May 2002 . In July 2002, ENSP developed a European consensus on proposals for action in the field of tobacco control in relation to the new public health programme with reference to : • Activities related to the monitoring and rapid reaction systems • Activities on health determinants • Activities related to legislation • Activities related to consultation, knowledge and information • Promotion of co-ordination at the European level of non-governmental organisations (Annex: ENSP working priorities) • EU communication campaign on youth smoking prevention ENSP has been invited by the European Commission to act as a link between the Commission services and the NGO community in the development of this EU wide campaign to ensure NGO suport. Together with ENYPAT, ENSP attended a meeting on January 15th, where the EU media campaign concept was presented to Commission officials. Updates on development were circulated to the ENSP list server. ENSP assisted the Commission services in identify international experts to write scientific background information for the brochure published for the launch of the campaign. On April 18th, ENSP orgnaised a restraint consultation meeting for the Commission at the occasion of the ENSP General Assembly in Athens as well as discussions during the subsequent network meeting.

During the meeting much controversy was raised concerning the concept of the campaign and the co-ordination mechanisms with the NGO community. The main concern was that the EU campaign used a campaign slogan close to tobaco industry “pseudo” prevention campaigns ( UK and the Netherlands). A letter was sent to Mr Byrne expressing these concerns signed by the conference participants. On the insistance of the health organisations, the European Commission agreed to create an expert panel that would monitor the campaign and advise the Commission on effective, evidence based concepts to reduce youth smoking through media campaigns. • Meeting with Commissioner Byrne ENSP initiated and organised a meeting between Health organisations active in tobacco control and Mr. Byrne on July 12th. At the meeting, Health Organisations expressed their concern for a comprehensive tobacco control policy with reference to the new advertising directive, the new Council Recommendation , the EU media campaign for youth prevention and on the blocking attitude of Germany in the European effort to develop a comprehensive tobacco control policy. The meeting succeeded to re-establish a trustful relationship between the Commission and NGOs active in tobacco control. Commissioner Byrne proposed to hold regular meetings to keep communicaiton channels open. GERMANY • Letter to Chancellor Schröder The German Health Ministry had accepted 11,8 million Euro from the Tobacco Industry over 5 years to be spent on youth prevention campaigns. 29 organisations form 17 countries signed the letter urging Chacellor Schröder not to compromise with the tobacco industry, to return the money to the Tobacco industry and to liberate a budget in the same amount to finance Germany’s tobacco control policy . WORLD HEALTH ORGANISATOIN • Committee for a tobacco free Europe As a member of the Committee for a tobacco free Europe, ENSP attended a CTE meeting on September 25th in Copenhagen. The aim of the meeting was to prepare the WHO Europe Minsiterial conference on tobacco control, to develop input from the CTE in the draft “Warsaw declaration” and to have insight in the research findings for the European Report on Tobacco control policy. • WHO Europe Ministerial conferenece on tobacco control - February 17-19 2002 ENSP was chosen by WHO to represent the NGO tobacco control community at the WHO Europe Ministerial conference on Tobacco control. In a plenary speech, ENSP exposed the NGO postion on the FCTC and made a special please to have a strong postion of the EU member states on a total ban on advertising. • WHO Europe Partnership Project to reduce tobacco dependence This project came to and end in 2002 and presented its conclusions at the international conference in Prague (Czs) on December 14/15, 2002; ENSP was invited to present the recommendations of the European Status report on smoke free workplaces in a worshop session. ENSP was also attended the conclusion conference of the German Partnership project on June 18th as an invited speaker. We had the opportunity to point out the blocking role that Germany was playing in the European tobacco control effort in the presence of Mrs.Caspers Merck, secreteary of state in charge of tobacco control issues.

• to actively support the work of organisations in the EU accession countries and facilitate transfer of technology

As visits to accession countries could not be financed under the present project grant, ENSP had to identify opportunities to be invited as speaker/expert in conferences taking place in accession countries. We participated in two major conferences in Warsaw. The CEE Smoke out conference organised by the Zoros Foundation was the opportunity to meet NGOs and Government representatives who are interested to build national coalitions in tobacco. Close contacts were established with representatives of Slovakia, Slovenia, Lithuania, Roumania and Albania who were interested in becoming members of ENSP. Contacts with Slovenia and Slovakia have resulted in participation at ENSP meetings in October 2002. The 3rd European conference on Tobacco or Health in Warsaw was an opportunity to intensify contacts, inform on the ENSP project building procedure and promote participation of accession countries in EU grant applications. New contacts were made with Poland and Hungary in view of closer collaboration and a possible creation of a national coalition against tobacco.

• Financial and administrative co-ordination of ENSP Framework project 2001/2002 – Grant Agreement N° S12.324433 (2001CVG2-008) – Current project

Besides fulfilling its contractual obligations for the ENSP core grant activities, ENSP has co-ordinated the financial administration of the present project. Personal assistance and support was given in the initiation of the sub-projects, advice on budget distribution and contractual obligations, collection, collation and preparation of the consolidated interim and the final technical and financial report. One project was cancelled during the project period due to difficulties of rising its own fiancial contribution. The project « Formation des médecins sapeurs pompiers à la prévention et à l’aide au sevrage tabagique pour ouvrir des centres départementaux de tabacologie pour les sapeurs pompiers et leur famille » had looked like a promising programme media wise, linking smoking cessation and firemen. Unfortunately the co-ordinator withdrew and the project never started.. The Irish partner dropped out of the GPs empowerment Phase II. and the Italian partner dropped out of the project “A look on anti-tobacco legislation in restaurants and bars” Minor amendments were made in the work plan of several projects and amendments had been requested to the grant agreement accordingly. • Finalisation project grant 2002/2003 - Ref 2002/203506-00.

Between September 15th and October 31, 2001 ENSP assembled, collated and introduced a consolidated framework grant application comprising 11 projects for a total amount of 4.585.793 drawing together 88 organisations of 17 countries. A financial support of 2.998.937 Euro was requested representing 68,91% of the total project budget. One subproject was not accepted for funding (Evaluation project of the international Quit and Win for Health Progfessionals) reducing the final project budget to 3.932.735,91 Euro with a Commision grant of 2.625.608,70 representing 66,76% covering 11 subprojects including the ENSP core grant. ENSP lead the administrative finalisation of the project grant from May to October 2002. The grant agreement was signed in November 2002, however activities had already started as of 15 September 2002 in order to avoid lack of time in the realisation of the project objectives. 1. Building a Framework grant application for 2003/2006 As already mentioned in the implementation of objective 1, several meetings were organised with European health networks in order to develop a horizontal approach across health determinants. ENWHP , ENHPA, EUMAHP/EUHPID, ENSP, EPHA, ENMHP, ENWHP,ENMHPO, ENMHP,EHN, ENHPS. The discussions concerned the possibilities to develop a co-ordinated grant application by European health promotion networks in the frame of the new EU Action programme on Public Health 2003-2008 (latest document available: Conciliation text). The basis for discussion was a draft document of the Commission “Concept paper on the implementation of strand 3 on addressing health determinants in the future Action Programme on Public Health ”. In order to lay the baseline for a co-ordinated grant application it was agreed that collaboration could be organised through consortia of health determinants with common priorities and common tasks. Each consortium could be led by one of the European Health Networks. Each consortium would involve a number of European Health Networks as project partners with the aim to attain a truly horizontal approach to health promotion after 3 years. • Common priorities: Monitoring , Equity, Health advocacy/capacity building • Common tasks : Sharing good practice, Policy development, Development of

implementation strategies, Building infrastructure, Evaluation/benchmarking

ENSP developed in consensus with its member organisations, a European proposal for action in the field of tobacco control based on the new public health programme’s legal text, the agreement with health European health networks to establish as far as possible an interactive and horizontal approach as well as established ENSP priorities. These priority actions were the baseline for an informal ENSP call for proposals to the European tobacco control. Community in July. 17 expressions of interest/project proposals were received. A project building workshop was organised in Brussels on September 10-11, 2002 with 1 delegate per coaltion/network + the authors of selected project proposals/having expressed interest in the procedure. In total 24 delegates attended. (Annex list of project proposals) The project building meeting resulted in agreement to build following projects for a three year project term (Sept 2003- Sept. 2006): 1. ENSP networking (emphasis on monitoring the tobacco industry / prepare advocacy

documents/ guidelines, identify other funding sources) 2. Support for Globalink the electornic tobacco advocacy network 3. Report on environmental tobacco smoke (exposure of workers, nicotine content in the air,

expert conference ) 4. TCRC – promotion of media advocacy of medical associations 5. Quit lines – development of best practice and benchmakring tools 6. Health professionals - adoption/implementation of cessation guidelines on

European/national level 7. Best practice guidelines for teachers/parents/adults working with young

An additional workshop was organised in Brussels with D. Piette of the Belgium School of Public Health and representatives of the Danish Cancer Society. The different project objectives have been elaborated and partners contacted. The final programme development will take place once the European Commission’s workplan 2003 is established and the offical call for proposal published. RESULTS OF PROJECT ACTIVITIES -OBJECTIVE 2 • Framework project 2000-2001 The project resulted in a publication of the summarized final reports which were disseminated to policy makers and health professionals on European level both as hard copy and on the ENSP and EU public health websites.

The final reports (of which some are in French) were compiled into one print-ready scientific document of 150 pages by an indepented editor (Hilary Solly) . The aim was to streamline the information and build it into one coherent document according to following outline : • Project title, ref. Number - co-ordinator, web-link • Objectives – budget – funding sources • Executive summary • Means by which the project was implemented • Results : output – dissemination-evaluation • Conclusion – strengths and weaknesses • List of project partners Where essential parts of information were lacking, the co-ordinators of the projects were contacted to complete the missing information. Project co-ordinators were asked to validate the print ready document. The document was printed in 300 copies. Framework project 2001 – 2002 On policy level the projects produced scientific evidence and policy recommendations on protection of non smokers in bars and restaurants, measurements were produced to show a comparison of nicotine contents in schools, airports, hospitals, restaurants and night clubs, a research was conducted in which way the regulation on health warnings of the tobacco

regulation directive met the expectation of the consumer, evidence was established as to the effectiveness of increasing tobacco prices as a tobacco control measure in a selection of European countries . Networks were consolidated and increased their membership such as the European Network for Smoking Prevention, the International Network on Women and Tobacco focused on women, smoking and inequalities, the international electronic network GLOBALINK redesigned the website and developed additional support structures for national coalitions, the Tobacco Control Resource Centrer mobilised Medical Associations in Europe to speak up for tobacco control . The Second European Symposium on Smoking in pregnancy and passive smoking and children promoted international exchange of experiences on effective intervention strategies. The GPs empowerment project progressed in the knowledge of doctors’ attitudes to smoking and smoking prevention/cessation and last but not least, the European Network of Quitlines enlarged the participation in Europe from 9 to 14 countries and established a common approach to Quit line management and evaluation procedures. The consolidated project results will be dissinated during the next project period according to an agreed procedure in order not to jeopardise scientific publication on one side but to allow at the same time to use the results immediately as policy tools. • The media : no specific press conference will be held – but media will be invited to

scientific conferences where the subject is presented/discussed • To policy/decision makers : Develop a fact sheet from the executive summary and/or use

selected information of the research/project results to be sent to selected persons for lobbying purposes without using tables/arguments that will be reproduced in the scientific publication

• To the general public on the Internet : Publish only the executive summary, make a note « Not for citation – in the process of publication - for further information please contact the project leader » until the projects results are officially published.

• Framework project 2002-2003 The European Commission has changed its internal procedure to validate contracts which has increased efficiency and grant agreements are ready for signature almost on the starting dates of the porjects, which was not the case in the preceding years. As the grant agreement signed by the Commission has been received beginning of November, 12 project leaders have started to implement their project activities on time and without delay. The emphasis on this years projects lies on research to produce evidence for tobacco control policy development. It covers the ENSP network and co-ordination activities and gives support to 2 international conferences. It supports research to identify training gaps for dentists, the continuation of the Euroscip project on smoking cessation in pregnancy, a trade union project to build a network for the protection workers from passive smoking, the development of measurement tools for tobacco control activities and a report on how to tackle socioeconomic inequalitites insmoking. It gives continued support for the electronic network Globalink, the network of European Quitlines and the European network of smoke free hospitals.

• Framework project building procedure 2003-2006 Although the terms of reference for a call for proposal under the new public health programme are still not known, ENSP has developed working priorities for 2003 based on the available legal text and in consultation with other EU health networks. Based on these working priorities an ENSP call for expression of interest/call for proposals was initiated in July 2002 followed by a project building workshop which resulted in 7 nated potential grant applications. We are now expecting a call for proposals end of January 2003 to proceed with the project building and partner research. CONCLUSION - Strengths and weaknesses of the project Overall, the project work has been running smoothly. The work of the ENSP secretariat was slightly peturbed by the moving of office and the installation of a new computer equipment. The workload was also considerably higher for the 2 ½ persons staff in the first half of the project term, as there was no information officer between September 2001 and April 2002. ENSP has achieved higher visibility and recognition of its co-ordinating role on international level which was confirmed by WHO when inviting ENSP to represent European NGOs at the Ministrial conference in Warsaw in February 2002. The role of ENSP as a major player for European policy developed was recognised by Commissioner Byrne when encouraging in a personal letter to continue our advocacy efforts. Within the Framework Convention Alliance, ENSP played a key role to facilitate communication and to motivate European organisations to actively influence the framework convention negotiations. The installation of the new website with the possibility to put on documents within minutes of their publication and a “members only” section to facilitate communication even on sensitive issues, has been recognized by many partners as an essential asset to access to timely and correct information. On project building level, ENSP’s experience with EU grant applications has made an outstanding contribution to the understanding of EU administrative mechansims and was able to influence the Commission to streamline its grant agreement procedure to accelerate contract signature. The staff was entirely committed to the ENSP overall mission, receiving many signs of appreciation especially for the administrative support, for the new information service and for efforts to integrate accession countries and European public health networks into the tobacco control networking effort. The EU media campaign had perturbed slightly the relationship between the European Commission and the NGO community and had emphasised cultural differences between northern and southern European NGOs. Differences could however soon be solved due to a constructive dialogue between all concerned. As far as the management of the ENSP framework project initiative is concerned, one sub project and two project partners dropped out under the project 2001/2002. Also, after evaluation, the Commission rejected a subproject of the grant application 2002/2003. In this field of work, contacts with the Commission services were easy and collaboration with the project co-ordinators was excellent.
