GENERAL INSTRUCTION TO TENDERERESSealed Tenders are invited from the contractors / firms who have executed electrical works ofsuccessfully.

1.0 Tenders documents are in two bids system i.e. “Technical Bid “and “Price Bid (online only)”.

1.1 Technical bid is to be submitted in a separate sealed envelope and super scribing theenvelope as “Technical Bid “ Technical bid shall be submitted with documents, asunder, falling which tender is liable for rejection.

i) Solvency certificate issued from any Nationalized Bank amounting to sum equivalent to20% of estimated cost (Minimum).

ii) Separate EPF Code No. Allotted by RPFC.

iii) PAN No. shall have to submit with Tech. bid.

iv) Past order copies of similar work carried out in GSECL power station or any otherpower station

v) Labor insurance copy with valid period.

vi) Service tax registration certificate.

vii) Experience certificate as per requirement in Schedule-I

viii) Details of equipments available with the tender as per Schedule-II.

The above documents shall be submitted in form of attested copies along with technicalbid. Also the successful contractor has to obtain valid labour license and GroupInsurance Policy of the insured labourers under W.C. Act. Immediately for sufficientlabours for the contract is entrusted and for extended period if any.

1.2 Bidders are requested to submit price – bid (Part-II) on-line only and not to submit theprice bid in physical form. This is mandatory. If price bid is submitted in physical form,same will not be opened and only on-line submitted price bid will be considered forevaluation.

1.3 It is mandatory for all the bidders to submit their technical bid documents by both formsviz. on – line (e-tendering) and physically in schedule time. Tender documents submittedin only physical form will not be accepted and same will not be considered.

2.0 Bidders shall complete the tender document in all respects and they are to be signed withCompany’s seal on all pages.

3.0 The bidders shall submit their offer without any deviations in general terms and conditions of thecontract, or in Technical specifications/Items. Tenders with such deviations, shall be rejected.Incomplete and conditional tenders shall not be considered.

4.0 GSECL reserves the right to split up the work covered in the scope of this contract amongstmore than one contractor.

6.0 Intending bidders shall submit tenders after studying all tender documents carefully and aftervisiting the site for satisfying themselves of actual site conditions, location and accessibility ofsite and nature and extent of the work involved etc.

Submission of tenders implies that bidders have obtained all necessary information and otherdata required for executing the work. No, claim for extra charges on account of anymisunderstanding or otherwise will be allowed.

7.0 Site cleaning and all approaches to the site shall be in the scope of bidders.

8.0 Bidders will not be allowed to give sub-contract of the works awarded to him for any reasonswhat so ever without permission of the Engineer-in-charge.

9.0 In the experience certificate, contractor has to furnish the details of similar works executed byhim along with the list of equipments, tools and tackles and manpower available with him alongwith the details of the same, which he intends to deploy on site of the work. The successfulcontractor has to deploy all such resources at site during course of work.

10.0 Tenders without EMD or with part payment of EMD will not be opened or considered. The detailsof EMD paid shall be submitted in letter giving details of payment of EMD i.e. demand draftNumber. The demand draft for EMD shall be put inside the cover containing technical bid & notinside the cover containing price bid. If this is done, the tender will not be opened or considered.Those who are exempted from payment of EMD shall submit a copy of certificate in separatecover along with the tender.

11.0 After opening of Technical bid no revised price from any bidders will be accepted.

12.0 Price quoted shall be firm, till overall completion of the work, under contract and there will not beany price escalation.

13.0 Date of opening of price bid will be intimated later on to those bidders who are qualified intechnical bid. Price bid of only qualifying bidders on the basis of evaluation of technical bid shallonly be opened. Price bid of bidders who are not technically qualified as per requirement ofspecial note for prequalification shall not be opened.

14.0 The decision of GSECL in the matter will be final and no any claim will be entertained in thisregard.

15.0 Tender shall remain open for acceptance, subject to the validity period of 180 days from the dateof opening of Tech.bid and during this period no tender shall be allowed to withdraw his tenderany such withdrawal during the period will entail forfeiture of the Earnest Money deposited withthe tender.

16.0 All the works shall be carried out as per specifications attached with the tender and relevantIndian Standards issued by the Bureau of Indian Standards. All materials procured should beconfirming to relevant Indian Standards issued by Bureau of Indian Standards, wherever notspecified in the item.

17.0 Tenders, which do not fulfill all or any of the conditions of the tender or incomplete in any respect,are liable to rejection.

18.0 GSECL reserves the right to reject any or all the tenders without assigning any reason thereof.

19. AGREEMENT :As per corporation’s rules, successful tenderer shall have to enter in to an agreement onstamped paper of appropriate value of Rs. 100/- with the Gujarat State Electricity CorporationLtd., in the prescribed form within one month. The cost of the stamp fee shall be borne bysuccessful tender. The tender with specifications, schedules, drawings, the contract booklet tobe signed by successful tenderer. The letter of acceptance, all above documents & subsequentcorrespondence shall be deemed to be a part of the contract agreement.

20. ARBITRATION:01 All questions, disputes, difference whatsoever which may at any time arise between the parties

to this contract in connection with the contract or any matter arising out of or in relation there to,shall be referred to the Gujarat Public Works Contracts Disputes Arbitration Tribunal” as per theprovision of the Gujarat Public Works Contract Disputes Arbitration Tribunal Act. 1992.

The reference to arbitration proceeding under this clause shall not :

(a) Affect the right of the E.I.C. to take possession of all or any tools, plants, materials & stores in orupon the work or site thereof or belonging to the contractor or procured by him and intended tobe used for the execution of the work or any part thereof.

(b) Preclude the E.I.C. from utilizing the materials purchased by the contractor in any work or fromremoving such materials to other place, during the period the work is stopped or suspended inpursuance of notice given to the contractor under General conditions.

(c) Entitle the contractor to stop the progress of the work of carrying out the additional or alteredwork in accordance with the provision of General conditions of the work where there are nospecifications.

(d) Preclude the GSECL from getting the work done by another agency.

02 Neither party is entitled to bring a claim to arbitration tribunal latest by 30 days after theexpiration of the defects liability period.

03 The provisions of the Arbitration and Conciliation Act 1996, Gujarat Public Works ContractDisputes Arbitration Tribunal Act-1992 and rules made there under shall apply to the Arbitrationproceeding under this clause.

Note:The clause no: 20” “Arbitration” of tender and contract for works booklet stands deleted andreplaced by new clause as above:-

21. All royalties, sales tax, toll tax, local tax, development charges, VAT tax, welfare cess and anyother taxes including works contract tax etc., in respect of this contract and also any statutoryvariation in future towards above mentioned taxes & any other taxes if levied in future bystatutory authority applicable to the this contract shall be payable by the contractor andGSECL will not entertain any claim whatsoever in this respect. The rates will be "Excluding theservice tax'. The reimbursement against “Service tax" will be paid to the contractor as per Govt.,rules & regulations. The proof of payment made by the contractor to the appropriate departmentshall be submitted to GSECL, failing which appropriate amount shall be withheld on gettinginformation/instruction from the concerned department. The contractor shall also produce thereceipt of payment of 1% welfare cess from the respective Govt., Department before submissionof final bill, otherwise final bill for the works will not be finalized”.

22. “OH&S Management System Requirement

It is to noted that GSECL, Ukai TPS is establishing and implementing Occupational Health &Safety Management System complying with OHSAS18001:2007.You as a part of the GSECL Ukai team and as defined by the scope of mentioned standard, all

contractors are required to comply with and participate to prevent occupational Health andsafety hazards and risks including observation of safety rules. Further you are also to complywith all applicable legal requirements while executing the work, in case the job is assigned toyou.”

SCHEDULE-A(Technical Bid)

Name of Work: Rate contract for Dismantling, lifting, shifting, overhauling and replacingof 6.6 KV HT motors of 2 x 120 MW Unit 1&2 during failure of motor inemergency as well as in AOH.

The terms & conditions applicable to this order are as under:

CONDITION FOR UNDERTAKING OF CONTRACT:

01. The work include removal of foundation bolts, lifting of motor from foundation and thenshifting to workshop/service bay at TPS, Ukai and vice-versa. The area of work cover HTmotor installed at Unit No.1&2.

02. Time limit of contract will be 11 Month or till completion of order value from the date ofcommencement. However if required GSECL has right to extend the contract for 6months for the same rate & terms and conditions.

03. No tools & tackles shell be given by GSECL, for the said works.Free of charge use of trolley chain pulley blocks shall be allowed. If trolley / chain pulleyblocks are not available at the location of works, it shall be arranged to shift to workinglocation by the contractors, however, shifting/fitting charges, from any place of TPSpremises to location of works as trolley/ chain pulley blocks, shall be given by thecorporation. If trolley / chain pulley blocks are not available at GSECL then same shouldbe arrange by contractor at his own cost and risk.

04. If available free use of cranes facility for lifting transportation of motor shall be providedby the corporation otherwise same should be arrange by contractor at his own cost andrisk.

05. Where rain hoods are provided for protection of motor, the same should be removed andplaced at convenient place so, as not to cause any inconveniency to other works andmay be refitted again after completion of work.

06. Motor should be removed without causing any damage to cable and cable box. Ifneeded motors should be removed in parts i.e. fan cover, cooler etc. separately. Anybolts nuts so loosened from the motor should screwed back on the motor body to avoidany misplacement of nuts and bolts.

07. Work of transportation must be undertaken with sufficient laborers and a soundsupervision to supervise the work irrespective of any representative of the corporation.Instruction of the corporation’s representative / Manager must be followed strictly.

08. Due to mal-function. Slip of failure of any equipment causing any injury either fatal ornonfatal to anybody either contractor personnel of others, GSECL will not be responsiblefor any liabilities whatsoever.

09. Motor overhauling work should be carried out in workshop/service bay with sufficientmanpower and complete care so that no part of the motor should be damaged.

10. Stator –rotor of the motor should be cleaned completely.

11. Complete overhauling should be carried out under supervision of technical/ manager ofGSECL. Instruction of manager should be followed strictly.

12. If bearing are required to be replace then bearing will be given by GSECL free of costand replaced bearing shall have to be credited to TPS store.

13. Petrol, Kerosene, cotton waste shall have to be used by contractor and for that no extracost will be given.

14. Any damaged either to motor or any other equipment of GSECL during the work. Will berecovered from the contractor such as full protection for safety of motor and otherequipment on plant during transport must be ensured by the contractor.

15. Quantities estimated in schedule – ‘B’ is only approximate and tentative. The paymentwill be based on actual work undertaken and completed irrespective of total value ofcontract.

16. Any labor laws in force from time to time will be applicable.

17. Welder along with gas for oil & pulley heating shall be arranged / given by the contractor

18. GSECL will not be responsible for damages/Injuries, fatal/non fatal accidents etc. tocontractor’s manpower/ tools & tackles etc.

19. For shifting of any motor free use of girder poles/sleepers for resting of motor will beallowed but after completion of work the poles/sleepers should be placed at its placeagain by contractor.

20. Grease for lubrication of motor will be provided by corporation free of cost.

21. Insulating varnish if necessary will be provided by corporation free of cost.

22. Air gap/shifting work is to be carried out only when required by GSECL.

23. Material for gasket will be arranged / provided by the contractor.

24. Seams will be arranged / provided by the contractor.

25. No adhesive for gasket will be given by corporation. Contractor has to got approved thequality of adhesive before using it, from engineer-in-charge.

26. Painting of motors and its accessories like fan cover; air coolers may be carried out inservice bay. Two coats of oil paints to be used for painting & paint must be “Asian paint”make OR any equivalent make. Before painting; motors, its fan covers, coolers shouldbe cleaned perfectly by iron brush, coarse paper up to satisfaction of EIC. Motors, fancovers, coolers should be painted on all sides. Inner sides of fan covers & coolersshould also be pained. Painting should be carried out by spray gun; not by brush. Partyhas to arrange the spray gun.

27. Rates quoted for painting inclusive the cost of paint / thinner along with labor.

28. Air gap setting of pedestal bearing of HT motors of Unit No 1&2 will have to be carriedout as detailed under. Air gap adjustment between stator and rotor by adjusting, shiftingthe stator. Air gap are to be adjusted on both side (i.e driving end and non-driving end)of motor to the satisfaction of engineer-in-charge. The decision of engineer-in-charge willbe final.

29. Transportation of pedestal bearing motor is to be carried out with utmost care. Whiletransportation and unloading necessary wooden blocks are to be provided beneathstator and rotor so that load of rotor is on wooden blocks and not on stator and statorshould be sufficient high from ground level and rest on sleeper. Necessary carting ofwood blocks is to be carried out by contractor.

30. Overhauling of HT motor (without pedestal bearing ) is to be carried detailed as under :Removal of motor pulley, fan covers, fan cooler, greasing pipes, bearing etc. take out ofrotor from stator, clean all parts by air ( if necessary with petrol ) and cloth, replace thebearing if required, complete cleaning of cooler and its tubes. Cutting and fixing ofgaskets, applying varnish on stator and rotor winding after proper heating. Afteroverhauling, reassembling the motor, one hour no load test applying three phase 415volts supply to the motor to the satisfaction of Manager-in-charge. All the works are to becarried out as per direction of engineer-in-charge.

31.Overhauling of pedestal bearing HT motor is to be carried out as detailed.Removal of all covers, coolers, coolers fan, pedestal bearing, taking out of rotor fromstator if required, cleaning of stator/rotor, winding and all (If necessary with petrol),cleaning of pedestal and cleaning /checking of bearing, removal of pulley if required,complete cleaning of cooler and its tubes with air and brush, cutting and fixing ofgaskets, applying varnish on stator and rotor winding, after proper heating andreassembling. Air gap adjustment between stator and rotor by adjusting, shifting thestator as mentioned in sr no 28. Finally complete the motor box up work by providing allremoved cover/accessories.

32.You have to engage separate labors for this work i.e. different labors for different workorder and after completion of this work, labors may be discontinued from this job andgate pass issued, have to be surrendered.

33.The nature of work is such that if it is to be carried out in emergency and any odd hoursof day or Night therefore contractor is required to reside at Ukai and the work is requiredto be started within Two(02) hours of intimation otherwise penalty shall be imposed fordelay in starting of work.

34. During the AOH work to be carried out & completed within scheduled time period fromthe date of unit shutdown taken for AOH; otherwise penalty shall be imposed as perGSECL’s rules.Schedule Date is informed by office of SE (Elect).

35.CONTRACT AGREEMENT:Contract agreement shall be executed on appropriate (presently Rs. 100/-) stamp paperas per corporation’s rules. The cost of stamp paper shall be borne by the contractor

GENERAL TERMS & CONDITIONS OF CONTRACT:

LABOUR LAWS:01 Persons below the age of 18 years shall not be employed for the work.02 No female workers shall be employed in the night shift between 07.00 PM to 06.00 AM.03 Contractor shall maintain valid labour license under the Contractor (Regulation &

Abolition) Act 1972 for employing necessary manpower to be required by him. In theabsence of such license the contract shall be liable to be terminated without assigningany reasons thereof.

04 The contractor shall at his own expense comply with all the labour laws and keep theCorporation indemnified in respect thereof. Some of the major liabilities under variouslabour and industrial laws which the contractor shall have to comply with are as under.

05 Payment of contribution of employees (Contractor Labour) as well as employer’s(Contractor) contribution towards Provident Fund, Family pension Scheme, EmployeesDeposit Linked Insurance Scheme. Administrative charges, Inspection charges etc. atthe rates made applicable from time to time by Government of Gujarat / Government ofIndia or other Statutory Authorities.

06 Labours engaged by you shall be entitled for Earn Leave as per the provision laid downin Factories Act 1948. Contractor shall extend the facility of E.L. as per the provision laiddown in Factories Act and shall also have to maintain leave records in prescribed formunder Factories Act.

07 No labours are allowed to enter in Power Station premises who do not have identity cardprescribed under Factories Act.

08 Contractor shall have to comply with the Maternity benefit if he happens to engagefemale labours as per Maternity benefit Act.

09 Contractor shall have to obtain Insurance of the labourers engaged by him to carry outcontractual work before commencement of work under workmen’s compensation Act.

10 P.F. CODE NUMBER:The contractor shall have to submit the details of his own P.F. Code number along withoffer from appropriate authorities, without P.F. Code No. the tender is likely to bedisqualified.

11 PROVIDENT FUND & FAMILY PENSION SCHEME:The Contractor shall submit along with his bill (Month wise) a Statement regardingdeductions at the rate of 12% of the wages against employees Provident Fund & FamilyPension Scheme in respect of each concerned employee. (or at the rates madeapplicable by the Government from time to time) of the wages.

The contractor’s contribution and his workers contribution towards Provident Fund andFamily Pension Scheme shall be deposited by the contractor with Regional ProvidentFund Commissioner, Ahmedabad or to the authority prescribed under the Act along withthe other charges applicable. P.F. Chalan submits with R.A. Bill.

12 If any workman engaged by contractor meet with fatal or non-fatal accident while on dutythen contractor shall be liable to pay compensation to the legal heir or to the workman asthe case may be as per Workman’s Compensation Act.

13 EMPLOYEES DEPOSIT LINKED INSURANCE SCHEME:The contractor shall have to deposit ½% of the rate applicable from time to time of thewages in respect of employees who are a member of the Provident Fund against thecontribution towards Deposit Linked Insurance Scheme with regional Provident FundCommissioner, Ahmedabad or concerened PF office.

14 The Contractor should pay prevailing minimum wages to the labourers engaged by himas per the minimum wages Act in presence of the Corporation’s Officer of representative.

15 ADMINISTRATIVE CHARGES: Administrative charges for maintaining Provident Fundaccount shall be deposited by the contractor with Regional Provident FundCommissioner, Ahmedabad or concerened PF office at the rates applicable.

16 LABOUR LICENSE FOR LABOUR CONTRACT: The Contractor shall have to obtainthe copy of labour contract license under contract labour (Regulation & Abolition) Actfrom the appropriate authorities before commencement of work.

17 Contractor shall have to issue the appointment letter to their labour engaged during thecontract period each and every instance and should narrat clear cut terms andconditions for the liabilities of the concerned.

18 After completion of the contract work, the work completion certificate is required to beproduced before the DGM & IRO for the purpose of no due certificate and cancellation ofthe contract work.

19 The contractor shall have to maintain all the relevant records under labour laws asmentioned here in above and keep ready for verification by the Corporation’s authority aswell as the authority of the Government Officials as and when asked for the checking.

20 The contract shall have to produce/submit the original copy of wage register/muster rolland relevant records under labour laws for the contract work as and when demanded byCorporation for statutory compliance if arise in future.

21 The contractor shall deposit 15 Days salary at the rate of last drawn salary for everycomplete done year of service for each worker for the liabilities of gratuity.

GENERAL TERMS AND CONDITIONS01 The ‘contractor’ means the documents forming tender and the subsequent

agreement that may be entered in to.02 The tender should be submitted along with EMD without which no tender will be

accepted.03 Conditional tender will not be accepted unilateral withdrawal of condition after opening of

tender will be permitted.04 The general rules and regulations of the company for tender and contract for works will

apply to the extent the same are not modified herein. Any other rules and regulations,conditions etc. that are in force at present and that may be framed by the company fromtime to time in connection with contracts will be binding and acceptable to the contractor.

05 Sub-letting or tradable of contract without prior written approval of GSECL will be treatedas a breach of contract and it will be punishable by forfeiting the security deposit as wellas termination of contract.

06 Chief Engineer (Gen.) Ukai TPS reserve the right to reject any tender(s) withoutassigning any reasons thereof.

07 The successful Tenderer will have to pay total 5% of the order value as security depositis to be paid by D.D within a week time on receipt of final order.

08 The contractor shall hold the company indemnified against and in respect of any claimsarising out of injury to any persons (s), loss of life, workman’s compensation and anyother thieve party claims whatsoever. The contractor shall be responsible for anydamages to any property of the company due to negligence of the contractor on hisemployees. The company will be entitled to recover the damages from the contractorand it shall be final and binding to the contractor.

09 The contractor shall submit his sales tax registration number and same should be quotedon all his bills.

10 The successful tenderer/contractor will have to enter in to an agreement with thecompany and the cost of the stamped paper will be borne by the contractor. Thecontractor will also have to execute an indemnity bond on required value of stampedpaper as per company’s rules. The cost of the stamped paper will be borne by thecontractor.

11 All questions, disputes, OR difference, whatsoever which may at any time arise betweenthe parties to this contract in connection with the contract or any matter arising out of orin relation thereto, shall be referred to the Gujarat Public Works Contract DisputeArbitration Tribunals as per provision of the Gujarat Public Works Contract DisputeArbitration Tribunal Act, 1992.

12 The party shall strictly observe all the Labour laws and Factory acts in existence andamendments made from time to time.

13 I) Party shall have to strictly observe all safety rules as per Factory Act-1948 and whateveramendments made from time to time to avoid any charges of accidents. Contractorshall have to strictly follow the instruction of safety office.

II) Before starting the contract. It is advisable that party should have the documents forinsurance of workers.

14 The work shall be completed within stipulated time. If required, party will have to workround the clock to maintain the work schedule.

15 Payments shall be made as per actual work done only.16 The contractor shall have to take all necessary precautions to avoid fire during cutting

welding etc. in case of any smoldering fire, the same should be extinguished bycontractor immediately using their own labour tools water and buckets etc.

17 The party shall arrange is own adequate tools and tackles welding accessories drillingmachine, grinders, scaffolding/lighting materials including lighting transformers as per IS-1416 etc. required for the work included in Schedule-A.

18 Party shall have to arrange all consumables including required kerosene, rustolenepaints, grinding wheels etc. at his own cost.

19 An authorized are technically qualified representative of the contractor must be availableat site while the work is going on the address of the representative should be submittedto this office so that party can be contacted whenever, required in connection withexecution of the work

20 Scaffolding material, if available with GSECL, will be issued free of cost otherwisecontractor will have to arrange for the same at his own cost. After completion of workscaffolding is to be removed immediately.

21 After completion of work, working area and surrounding area should be made clean byremoving M.S. debris & all types of scrap etc. scrap should be credited to main storeonly & it should be dumped at place shown by EE (Store).

22 Electrical connection & consumption will be given free of cost at one mutually point.“GSECL will issue the LT power supply of max. 200Amp and is of 3 phase 3 wire systemonly. If neutral is required for control of their equipment; party should have toarrange the control transformer with necessary change of wiring in their panel. As 3phase 4 wire system is not available.”

23 Oxygen /Acetylene gas if available will be issued on chargeable basis from GSECL mainStore/otherwise party has to arrange on its own. The empty cylinders should be returnedto main Store with in stipulated time.

24 Party shall submit daily progress report for the work carried out by the.25 the contractor if desires can see the plant and site of work with permission of under

signed on any working day during working hours.26 Party has to carry out work at site only.27 “OH & S Management system requirement

It is to be noted that GSECL, Ukai TPS is establishing and implementing OccupationalHealth & Safety Management System complying with OHSAS 18001:2007.

You as a part of the GSECL Ukai team and as defined by the scope of mentionedstandard, all contractors are required to comply with and participate to prevent theoccupational health and safety hazards and risks including observation of safety rules.Further you are also to comply with all applicable legal requirements while executing thework, in case the job is assigned to you.”

UTPS-SAFETY-001: REF: 00: 15.12.2008: UKAI TPS“GENERAL SAFETY RULES / NORMS”

TO BE OBSERVED BY THE CONTRACTORS.

All the contractors working at Ukai Thermal Power Station shall have to strictly observe thefollowing Safety Rules. The Contractors shall be responsible for informing & observing these rules bytheir supervisors / employees / labours as well as the supervisors / employees/ labours oftheirsub-agencies / sub-Contractors engaged, if any. Prior to commencement of the work,Contractor shall have to submit a written assurance on their letterhead to the concernedSectional Head / Engineer-in-charge that they have thoroughly gone through these Rules,have educated their employees / employees of their sub contractor and will strictly observethe said Rules while execution of work under work contract awarded to them. They will have toindemnify the Corporation for any loss or damage / accident / injury to the Corporation‘sproperty / employee or employee of their own in default of non - observing these rules.

01 Persons to be employed for carrying out the work shall possess requiredqualification, be fully trained and conversant for works to be done. All persons shouldhave gate pass. Register consisting the full details (i.e. address, phone no, details ofnearest relative, etc.) of all persons is to be maintained. During the work execution,one trained & competent supervisor should always remain present at site.

02 The contractor shall take all the required safety measures prior to commencementof work on dangerous substances, machineries or area at which cautionary notice isdisplayed and obtain —Line Clear“ or —Work Permit“ through the concernedDepartment / Section.

03 Prior to carrying out welding, gas cutting, furnace heating or any other hot workjob, the contractor shall remove all the inflammable material lying at or nearby worksiteor cover it properly by suitable protective covering. Also, special care shall be takenbefore carrying out such job & see that all possible contributing factors to set fireshall be removed / vanished prior to commencement of the work. Advanceintimation shall be given to concerned section / fire section to commence the work in fireprone areas. They should also keep ready all the First Aid Fire Extinguishers /equipments & fire extinguishing media / material like sand / water buckets orother appropriate equipment at such place.

04 While carrying out work in confined space or inside vessel, the contractor shallobtain necessary —Confined Space / Vessel Entry Permit“ from concerneddepartment prior to commencement of the work. For lighting in such areas, only 24-volt(ISI certified & with proper guard) hand lamp shall be used. For taking care of thepersons working inside the confined space / vessel, a supervisor / person capableto keep continuous watch on person(s) working inside, assist them incase ofemergency or arrange to get immediate outside help, shall remain present at entrypoint and shall use full body safety belt without fail. While working inside sewage, trenchor in-depth, a person to warn outsiders / entrants / passers etc shall remain availablenear entry point or the entry point shall be cordoned by a barricaded tape with acautionary notice. After completion of the works, all the lids / covers / grills / gritsopened, shall be re-fixed / re-placed in the original position as it were prior tocommencement of the work and leave the work place in safe condition in allrespect, so as to prevent accident to fellow workers.

05 The contractor shall see that he / his persons do not block (by stacking material,spare parts, tools-tackles, equipments etc), any passages / walkways / gangways /aisles / staircases / ladders / lifts or any other approaches / roads leading toplants or its auxiliaries, on which there is a traffic movement or possible trafficmovements in case of emergency. Such passages are meant for safe escape in theevent of emergency. If it is utmost necessary to carry out work in such area with

blocking of passage, prior permission of Competent Authority or the Engineer-In-Charge shallbe obtained. To demarcate / declare the area as UNSAFE, cordon it using barricadingtape & display suitable caution notice or keep a person to restrict / divert the traffic onthis route through other safe passage.

06 Prior to use power / electrically operated hand tools / equipments / machines / gadgetslike welding machine, hand grinder, hand drill etc, ensure for its safe operation & useit only if it is found safe to use. Do not use defective, unsafe or improperly maintainedequipments. The electrical power supply required to run such equipments shall notbe taken directly at their own but shall be obtained through concerned ElectricalMaintenance Departments or their authorized persons or under their observations /guidance only. The Electrical Section shall provide temporary electrical connectionup to contractor‘s Mains Board on which it is compulsory to install mains switch, ELCB& fuses of adequate capacity. All such equipments shall invariably be earthedadequately to prevent electrical shock, sparking, short circuit etc. Power cord tobe used shall be of adequate capacity, without any joint & shall consist of earthwire also. Hence, it is necessary to use adequate capacity 3-wire power cord for single& 5-wire power cord for three phase power connections. The plugs, receptacles, pins,holders etc shall be of adequate capacity & safe to use. All electrical & mechanicalequipments / tools-tackles viz. welding machine, cutting machine, Grinder, Drill, ChainPulley Blocks, Hook chooks etc required to be used during work execution shall beof standard make & bear ISI certification mark on it. The consumables like weldingelectrodes, grinding wheels / discs etc which has specific prescribed life spanshall not be used in any case if its expiry date is over.

07 It is compulsory to use standard make Personal Protective Equipments (P.P.Es.) asper the job requirement. Do not work without use of required P.P.Es. Contractor isresponsible to provide standard make (ISI approved) Personal ProtectiveEquipments / Safety Gadgets suitable to give sufficient protection against hazardsinvolved in their work / job to their staff, as per the job requirement and insist / enforcetheir staff to put on the same while at works. The ongoing work is liable to bestopped at any time if the contractors‘ staff is found working without P.P.Es.Following is the list of various P.P.Es. to be used for various works / worksites

08 Before using lifting machines / tackles (like C.P.Bs., Hook chooks, winch, forklift,mobile crane, EOT crane etc) & its attachments (like D-shackles, slings, U-clamps,Eye bolts or any fixtures), it shall be checked and used only if found safe to use.Also, ensure that these are tested, examined & certified in form no 9 / 10 byCompetent Person as per the Factory act-1948 and its validity is not expired.Further, it shall be fixed properly and firmly prior to lifting the. weight.

09 Scaffoldings to be used for working at height shall be of adequate size &capacity. Obtain the work permit when working at height. While climbing onsuch scaffolding or working on any structure at height, use of full body safetybelt & Helmet is compulsory. It is also necessary to fasten chinstraps of thehelmet.

10 Contractor or their employee shall not interfere in day-today routine plant activities/ works except the work assigned to them, shall not loiter in the areas otherthan their work jurisdiction, as well as shall not temper / operate / touchthe machineries / equipments / auxiliaries with which they are notconcerned. Also, the contractor shall strictly instruct their staff not to sit ortake rest at / near / below running plants, auxiliaries, systems or anyplace which is risky, hazardous & prone to accident.

11 The cylinders containing poisonous / toxic or inflammable / explosivegas like Oxygen, Acetylene, LPG, Hydrogen, Ammonia, Chlorine, CO2 etcshall be handled safely taking due care. To handle / shift such cylinders aspecial trolley / cage meant for it must be used but in no case it should berolled. Domestic LPG cylinder shall not be used / permitted.

12 On completion of the work, cotton waste, spilled oil / grease, pieces of weldingrod & other waste material shall be removed from work site and the area shallbe left safe, neat & clean.

13 In case of any injury / accident while working, it shall immediately bereported to Safety Department through concerned Sectional Head / Engineer.The prescribed Form No. 21 may be obtained from concerned section or SafetyOfficer. For any incident occurred but have no injury to any persons shouldalso be informed to Safety Officer as —Near Miss Incident.“

14 In all risky jobs, before starting the work, contractor should obtainGeneral Safety Work Permit from concerned section well in advance.

15 In case of noticing smoke or fire during their work execution, they shall makeimmediate efforts to extinguish/control it and simultaneously inform

the Fire Brigade on phone No. 4444 or 4344 or 5555 or 02624-290049,shall shift the casualty to nearby hospital after rendering first aid in case ofaccident.

16 Over & above these, contractor shall have to follow all the safetyrequirements / rules & regulations / norms and legal provisions laid down invarious statutes, particularly the provisions of The Factories Act-1948 & The GujaratState Factories Rules-1963 (Amended up to date) shall be followed strictly.The contractor shall also obey the rules / regulations / instructions of the localCompetent Authority for safety requirements.

17 No women or young person shall be allowed to clean, lubricate or adjust any

part of a prime mover or of any transmission machinery while the primemover or transmission machinery is in motion. Examination or operation ofmotion machinery shall be made or carried out only by a specially trainedadult male worker wearing tight fitting clothing as per section 22 of factories act1948.

18 No women or young person shall be employed or permitted to work in Lead-compound area like battery room etc. as per schedule VI of GFR 1963.

19 All the relevant labour and industrial laws shall also be followed compulsorily.20 It is felt necessary to deploy safety officers / Safety supervisors by contractors /

Agencies involved in carrying out hazardous activities / operations inside TPS tohave better and constant supervision in terms of Health Safety and Environmentactivities.

21 In case, it comes to the notice of GSECL UTPS management that the safetyguidelines / safety rules / safety norms are not being followed then aminimum sum of Rupees One Thousand shall be penalized on thecontractor / Agency and for subsequent such violation, a severe penalty /action as deemed fit shall be imposed, which may please be noted.

22 The above rules shall be scrupulously followed and where required, they maycontact the Safety Officer for any ambiguity / further guidance in thisregard.

23 For performance evaluation of contractor, safety factors of work accident, fireincident & near miss accident will be considered. Steps can be taken toreview the job assignment up to cancellation for negligence.
