Transition Team - Involvement

Involvement between 14-18yrs:
- Be ‘involved’ at yearly reviews
(where appropriate).
- Learn about your likes, dislikes,
needs, and aspirations.
- Hold ‘information sessions’ to
explain who we are and what we do.
- Assess (at 17yrs) if you will be
eligible for Adult Social Care
Services.
*Not lead professional at this point(But we will be working closely with Children's Services
where appropriate).

SEN Team:Fund the educational placements and transport
arrangements pre 19yrs.
Pathway Team (SEND):They identify post 19yrs educational options, and
secure the funding for these placements (from EFA
(formally YPLA)).
Continuing Health Care (CHC) Team:We have a duty to apply for CHC funding where
appropriate, and the CHC team support us to do this.
