Ability Centre — NDIS Planning Workbook

What is the NDIS? The National Disability Insurance Scheme (NDIS) is all about giving you choice and control over your disability services and supports.

It is designed to empower people with disability to work towards achieving their goals and to work out the types of supports they need.

The NDIS replaces other types of disability funding previously available across Australia and will be fully rolled out across Western Australia by 2020.

How do I access the NDIS?When you contact the NDIS (1800 800 110) they will determine your eligibility. The key criteria are that a person has a permanent disability that:

• reduces their ability to participate effectively in activities, perform tasks or actions unless they have support

• affects their capacity for social and economic participation

• is likely to require support under the NDIS for a lifetime

You must also be:

• living in an area where the NDIS is available and

• an Australian citizen, or

• the holder of a permanent visa, or

• hold a Protected Special Category Visa and

• under 65

How do I use the Ability Centre Planning Workbook? We want to help you get the best possible support accessing the NDIS.

This workbook will help you to think about what services and supports Ability Centre can provide you to live the life you want.

Why should I use this workbook?This workbook has been developed to help you think about what your life is like now, what’s important, the support you currently receive and the goals you have for your future.

These are all of the things that National Disability Insurance Scheme (NDIS) will want to discuss with you at your first meeting.

The NDIS planner will also talk to you about the informal, community and mainstream supports available to you and determine your eligibility for reasonable and necessary NDIS funded supports.

This workbook will also help Ability Centre to better understand how we can work with you to help achieve your goals.

The little things that make a differenceWhen you go through these questions sometimes it’s easy to miss the little things that make a huge difference in your life.For example, did you get new orthotics, did you or your family need a break and access an overnight stay at home or in another location? Please write or draw all of the things that you can think of over the last 12 months that have made your life easier and more enjoyable.

You're done!Now it’s time to have a break. Read through the workbook again tomorrow, to make sure you haven’t missed anything important.
