Conference Participant Participation Waiver and Release Success Secrets from Spelman Sisters: A Girls Empowerment Conference

I hereby consent to the child’s participation in the Success Secrets from Spelman Sisters: A Girls Empowerment Conference. I voluntarily release the sponsor of this event, the Detroit Chapter of the National Alumnae Association of Spelman College (Detroit NAASC), from any and all liability, claims, actions, or rights of action, which are in any way related to my and my child’s participation in the conference activities. In case of emergency, I understand every reasonable effort will be made to contact the parents or guardians of minor registrants. However, if the parents or guardians cannot be reached within a reasonable time period under the circumstances, or if I, the below signed registrant am 18 years of age or older, I hereby give Detroit NAASC permission to act on my behalf in seeking and administering medical treatment in the event that such treatment is deemed necessary or advisable for the participants’ health, safety and welfare. I release Detroit NAASC from liability in acting on my behalf in this regard and rendering such medical treatment. Students and parents should also be aware that sensitive matters such as women’s health, human sexuality, religious beliefs, gender roles, politics and other issues may be components of conference discussions. Detroit NAASC and its agents are sensitive to these issues and are facilitating certain discussions in an effort to mentor and provide guidance to young girls as part of our mission. These discussions will be handled with consideration to all participants’ ages and confidentiality will be upheld as appropriate. I agree that Detroit NAASC have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Detroit NAASC’s mission. These uses include, but are not limited to illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional or educational materials in any medium now known or later developed, including the Internet.
