Membership Payment MethodsThe following payment methods areavailable to members. A one-time,nonrefundable $10 enrollment fee isrequired for individual sales of the Pre-PaidLegal Services®, Inc., Family Plan in allstates and provinces.

1. Monthly / Annual Bank Draft—Initial payment of one monthThe membership may be paid monthly orannually by automatic bank draft transferfrom the member’s bank account. After theapplication and the first month’s or firstyear’s membership fees have been received,the member is notified of the deductionthrough his/her monthly bank statement.When writing a membership application, besure the Authorization to Honor Checks orElectronic Transfers section is signed, andthe monthly or annual amount filled in bythe member.

a) Membership fee: Fill in the correctmonthly or annual membership fee.b) Signature of account holder: The bankdraft authorization must be signed the sameway the account holder signs on his or herchecking account. We cannot accept theAssociate’s check for the membership fee.

A check must accompany the application.Payment for the first month’s or year’smembership fee and enrollment fee mustaccompany all bank draft business.Thereafter, the monthly membership fee willbe drawn automatically from the member'schecking account. Remember, you mustwrite separate checks for membership feesand Associate fees. Ohio residents mustmake membership checks payable to OAJ.

If the first month’s membership fee is paidthrough money order or cashier’s check:• Commissions will be paid as earned• You must include a voided check and

appropriate bank draft information

Cash will not be accepted.

Drafts may be made from a savings accountif clearly marked on the authorization.•Verify with the financial institution that

the savings account can be drafted.•A bank document verifying the savings

account number is necessary.

Remind the member that the bank accountis drafted on the same date as themember's effective date or the nextbusiness day thereafter.

Note: You must attach a bank document with

preprinted name and account # or averification of account letter on bankletterhead verifying the account holderand transit and account numbers.If bank documents are not included, wewill contact the member by mail toobtain the information. This mayhinder timely processing of the appli-cation and delay availability of benefitsto the member, and/or payment ofcommissions to the Associate. It mayalso result in failure to meet deadlinesfor Fast Start qualification, contestrequirements, or ED requirements.

Funds must be paid in Canadian Dollars.

Membership Payment Methods2. Credit CardA member may pay the first and subsequentmembership fees by Visa, MasterCard,American Express, or Discover. (In Canada,MasterCard and Visa only.) Use the creditcard portion at the bottom of the UniversalMembership Application to check the typeof card and write in the complete cardnumber, expiration date and the amount tobe charged.

Credit card payments may be monthly orannually. The member will continue to becharged when payment is due until awritten request from the member to stopcharges is received in the Corporate Offices.If an applicant wants to pay one time onlyby credit card, specify in writing on thecredit card authorization and the memberwill be billed for an annual payment nextyear.

3. Annual DirectThe member may choose to be billed annu-ally for the membership fee. The completedand signed application should be for-warded along with a personal check, moneyorder, or cashier’s check for the annualamount. The member will be sent a state-ment each year thereafter.

Quarterly payments are not accepted.Semiannual payments are accepted butwill be paid as-earned.

4. Payroll DeductionFor more information on group sales,contact Marketing Services. You must bequalified for groups sales before youapproach a group.

Upgrades, Add-ons & ReinstatementsHow are upgrades and add-onsdone?In order to upgrade or add on to an existingplan, members must fill out a new member-ship application. The bank draft or creditcard information must show the newmonthly bank draft amount. “Upgrade” or“Add-on” must be written across the top ofthe membership application.

Important:Member must authorize total newamount to be withdrawn in the bankdraft,not just the amount of the upgrade or add-on.

ExampleIf, for example, a member wishes to upgradeto the $25.00 plan or add the Legal Shieldbenefit to an existing family plan, a newmembership application must be filled outwith the new bank draft informationcompleted and the application must besubmitted to the Home Office.

NotePPLSI reserves the right to disqualify anyadd-on ($16.00 to $25.00, for example) fromcounting toward Executive Director Qualifi-

cation, Level promotions, or contests.Counters will not be given for upgrades oradd-ons written by the same Associate ororganization within 90 days of the originalapplication’s processing date.

How are reinstatements done?When an Associate reinstates a member-ship that has been cancelled over 90 days, anew writing Associate can be assigned. Ifyou were the original writing agent, nocounter will be issued, but you will receiveas-earned commissions. The new Associatewill receive a $40 reinstatement commissionand a counter ($30 if the member wasoriginally paying less than $10 a month) aswell as be able to earn on the membershipthrough future years.

When an Associate reinstates a member-ship that has not been cancelled over 90days, the original writing Associate willremain as the Associate and continue toearn on the membership.

If the membership has been cancelled forfive (5) years or longer, the new writingagent will be paid advance commissions onthe sale as a new membership.

Helpful RemindersPre-Paid Legal membership feesThe first month’s fee provides coverage forthe month paid, thereafter to be paid byautomatic bank draft each month. TheAssociate's check is not acceptable forpayment unless it is for an immediatefamily member* such as spouse, mother,father, brother, sister, grandparent, orgrandchild (commissions will be paid asearned). If a money order is submitted,commissions will be paid as earned.

Remember: Always include a Bank DraftAuthorization with bank information, orcredit card number and expiration date.

When bank draft is selected as the preferredpayment, you may call the bank with theaccount to be drafted for the bank’s transitnumber (always 9 digits in the U.S.) —OR—you can find the transit number at thebottom of the check.

Note: Many times the check number ap-pears in this string of numbers.DO NOT INCLUDE IT IN THE TRANSIT ORACCOUNT #.

*Attach a note to the application explainingrelationship of the member to the Associate.

Final check before mailing:Designate the desired plan and riders.Make sure the application is completelyfilled out (mail in the white copy).Make sure the authorization for bankdraft or credit card information on theapplication is filled out.Include a check for the fees plusenrollment fee. Also include a voidedcheck or a verification of account letteron bank letterhead verifying theaccount holder and transit and accountnumbers if monthly drafts are on adifferent account than the first’smonth’s payment.If you know the member is reinstatingor adding benefits, indicate either“REINSTATEMENT” or “ADD-ON”at the top of the application.Use the pre-addressed envelopessupplied to send business to theCorporate Offices.Do not use staples.

Reasons for Returned & Pending Business

If the membership application or AssociateAgreement you write is not processable, thebusiness will be returned to you . However,in some cases it will be held in the HomeOffice pending more information. TheCompany is obligated to promptly notify themember that his/her coverage is not inforce. You will receive a copy of any corre-spondence that must be returned to themember if the Home Office can identify youby name or number. Letters are sent with themembership application outlining require-ments for application completion. Pleasefollow the instructions carefully. The mostcommon reasons for returned or pendingbusiness are listed below.

Reasons for pending business

Money order, cashier’s check, or cashreceived for membership fee withoutvoided check and appropriate bankdraft information.

Questions 1, 2 & 3 on AssociateAgreement regarding License andFelonies not answered.

Make sure none of your business hasto be returned. A little extra attentionto details when writing membershipsand Associate Agreements will payoff in the long run!

Reasons for returned business

Bank information is incomplete orsignature of payor is missing.

Cannot accept the Associate’s checkfor membership except for immediatefamily members (spouse, mother,father, brother, sister, grandparent, orgrandchild).

No money was received with theAssociate Agreement.

Associate is not licensed in the state inwhich the member resides.

The state in which the membershipwas purchased is not open for busi-ness or for the plan requested.

Payroll deduction selected for anindividual sale (available only togroups).

Commercial Drivers Legal Planapplications do not include the state inwhich the membership was purchasedin addition to the member’s state ofresidence.

Miscellaneous.

Note: Please do not use Pre-Paid Legal

Services’ envelopes except for Pre-Paid Legal business. Because of thepreprinted bar code on the bottomof our envelopes, they are maileddirectly to our office regardless of theaddress on them. This applies evenif the bar code is marked through.
