4. Click Download Certificate of Experience – Foundation Schools template for an international graduate

5. Click Download Certificate of Experience – Medical Schools template for a UK graduate that completed between 2007 and 28 May 2015

Fill in the form

Fill in the details on the form

Check the doctor’s details are correct

Check the spelling is correct

Note for Chrome users

If the form opens directly in your Chrome

browser the Submit Form function won’t work Save the file to your computer and open it in

Adobe

Submit form

Press the submit button in the top right hand corner

Note for users who don’t use Adobe If you use a third party program the submit

button might not appear. If this is the case go to the last page to find out what to do.

Type your email address and name

Tick the Remember me button if you want your details to populate automatically

Press Send

Pop up message?

If you receive this pop message press okay and go to your outbox in your mail browser

If you don’t the form should have sent and you will receive notification email

If the email goes to your outbox

If the email is in your outbox double open it

Press Send to send the COE

Notification email

Once it’s sent you should receive a notification email

Problems?

If you have any problems you using the submit button, you can email the form to us. It is important that you do the following: 1. Send each certificate in a separate email

(ie one certificate and one email) otherwise you’ll need to resubmit your certificates

2. Send COEs for international doctors to IMG@gmc-uk.org

3. Send COEs for UK doctors that completed F1 before June 2015 to UK@gmc-uk.org
