Clinical Placements in NSW Health facilities require students to complete a

verification process to ensure the safety of themselves, other staff and the

patients in their care. Illawarra Shoalhaven Local Health District (ISLHD) staff

will be attending your university/college early in your course to undertake this

process.

Please refer to your Education Provider for details of the verification session

applicable to you.

In order to be prepared, you will need to start now to collect documentation

as follows (please note: some documents may take a number of weeks to

source):

Student ID card (issued by current Education Provider)

National Police Check (required for all tertiary students 18yrs and over)

Overseas Police check / Statutory Declaration (for Overseas students

ONLY)

Fully completed vaccination record card

Evidence of immunisation compliance (see next page)

Completed NSW Health Forms – Code of Conduct, PD2018_009

Attachment 6 & Attachment 7 (all included in this pack)

This Student Verification Pack contains:

A Checklist, which you should use, to ensure you have all your

documentation well ahead of your appointment

Information to assist you to collect required documentation

A letter to take to your local doctor explaining vaccination requirements

and evidence required for clinical placements

Copies of NSW Health Forms required to be completed ahead of your

appointment

(Please note: Attachment 7 may need to be completed again at a later

date, as it must be presented within 4 months of first clinical placement)

CHECKLIST FOR VERIFICATION APPOINTMENT

□ Original and photocopy of education provider-issued student photo ID card

□ Original and photocopy of current Police Check from Australia with no disclosable

outcomes, in exactly the same name as student photo ID card (not required for

secondary students, students under 18 years) and

□ Original and copy of overseas Police Check OR statutory declaration stating

no charges or convictions in country of origin for international students only (in

addition to Australian police check requirements above)

□ Have applied to Employment Screening and Review Unit (ESRU) if any disclosable

outcomes on police check

□ Original and copy of signed Code of Conduct for NSW Health

□ Attachment 6 Undertaking/Declaration form (check you have ticked the boxes

correctly) AND originals with copies of documented evidence of vaccinations and

serology records (printed blood test results) as follows:

□ dTPa (diphtheria, tetanus, pertussis) – at least one documented dose in

the last 10 years

□ MMR (measles, mumps, rubella) – 2 documented doses at least one month

apart OR positive serology for measles mumps and rubella OR born before

1966

□ Varicella – 2 documented doses at least one month apart OR 1 dose of

varicella received before age 14 OR positive serology for varicella

□ Hepatitis B – at least first dose documented for temporary compliance only

OR completed full age appropriate course AND positive serology taken at

least 4 weeks after last dose for full compliance (if all attempts fail to

obtain documented evidence of full age appropriate course of Hepatitis B

vaccination, then fully completed statutory declaration AND positive

serology result may be presented) Note: Hepatitis B compliance pathway

is attached

□ Attachment 7 – Tuberculosis risk form – A completed risk assessment form will

be required for this assessment. No further screening tests are required prior to

this session. Those students needing further screening will be advised of their

follow up plan and can be made temporarily compliant.

How to source an Australian National Police Check

Apply online at www.police.nsw.gov.au, present confirmation page and proof of

identification at police station and pay appropriate fee (currently $58.60).

Certificates are then mailed to your address. Newly purchased police checks first

presented after 14th January 2019 remain current for 5 years from issue date (subject

to additional requirements for aged care work or NDIS work).

OR

Apply for an online police check from any approved ACIC accredited broker (list

available from www.acic.gov.au). Please be aware that these checks need to be

verifiable online and many of these companies purge the data after 3 months (check

the fine print) meaning that you need to present the certificate within 3 months of

issue date. Once verified online by ISLHD staff, the check remains current for 5 years

from issue date.

OR

Current NSW Health employees can apply to have their police check status entered directly into ClinConnect by HETI by contacting [email protected]. These students may still be required to obtain a police check if attending placements providing Aged Care or NDIS work. OR Students with charges or convictions listed on their police check, or have charges or convictions since the issuing of their current police check are required to have a risk assessment performed by the Employment Screening and Review Unit (ESRU), HealthShare NSW. International Students: Please speak to your Education Provider about

sourcing additional police check requirements. Statutory Declaration forms

Documentation can be sourced from:

Your Blue Book or childhood vaccination book from your state or country (records

presented in a foreign language must be translated using the vaccine translation

website (http://www.immunize.org/vis/vis_english.asp) or using a local

translation service.

provided to you by your Education Provider. This card should be completed in

full and correctly by a GP or Immunisation Nurse. Please see ISLHD Doctors

Letter and example cards overleaf.

Dear Doctor,

The NSW Ministry of Health requires all students studying within Health disciplines to be immunisation

compliant before they are able to attend clinical placement during their course. The evidence that

must be provided is:

1. Adult dose of dTpa vaccine (NOT ADT) within the last 10 years.

2. 2 doses of MMR vaccine one month apart OR a positive serology (blood test) for Measles, Mumps and Rubella (an equivocal result is not acceptable and will require MMR vaccination) OR birth date before 1966 is considered protected (no vaccinations or blood tests required)

3. 2 doses of Varicella vaccine one month apart OR 1 dose of Varicella vaccine received

before the age 14 OR a positive serology (blood test).

Please note –MMR and Varicella vaccinations MUST be given on the same day OR 28 days apart. A history of childhood disease is not accepted. 4. Evidence of an age appropriate Hepatitis B vaccination course AND a positive serology

(blood test) collected a minimum of 28 days after the last Hepatitis B vaccine. Current age appropriate Hepatitis B vaccination course recommendations: An accelerated course is NOT accepted.

11-15 year old( School Based Program): 2 doses of hepatitis B vaccine (given 6 months apart)

Under 20 years of age: 3 doses of hepatitis B vaccine (given over 4-6 months minimum)

Over 20 years of age: 3 doses of an adult formulation Hepatitis B vaccine (given over 4-6 months minimum)

5. No TB screening tests are required prior to an initial assessment by ISLHD staff. Students who require further TB screening i.e. TB skin test (Mantoux) or IGRA (blood test), will be notified at their initial assessment. Whilst the IGRA can be arranged by a general practitioner, the TB skin test (Mantoux) can only be provided by the NSW Health Chest clinic in the student’s local area.

6. The influenza vaccination is highly recommended for all students during the influenza

season May-September. Each entry on this Adult Vaccination Card must be verified with the batch number (if available) of the vaccine, the date of vaccination, the signature of the vaccinator/ transcribing authorised immuniser and the practice stamp.
