Calling Card Service - The issuance of a card by the Company toa customer to enable him/her to bill telephone calls to his/heraccount.

Direct Dial Service - 1+ outbound long distance service.

Directory Assistance Services - Provides published and non­listed telephone numbers within Nebraska to requesting customersfrom the Company's territory.

Operator Services - Long distance service involving the servicesof a long distance operator.

Toll Free Service - The custom switched telecommunicationsservice that permits toll free inward 8XX number calling frompoints within the state of Nebraska to the subscribingcustomer's station.

1.1 Undertaking of the Company

The Company will provide access to Operator Services, aswell as, provide Directory Assistance, Direct Dial Service,Calling Card Service and Toll Free Service (8XX) betweenpoints within the state of Nebraska continuously (24 hoursper day, 7 days per week).

1.2 Use of Service

Neither customers nor their authorized users may use theservices furnished by the Company for any unlawful purpose.

1.3 Payment of Charges

Direct Dial Service, Calling Card Service, Toll FreeService (8XX) and Directory Assistance charges will bebilled in arrears and will appear on the customer's accountstatement. Prompt payment, as detailed on the statement,shall be expected.

1.4 Cancellation for Cause

The Company, by written notice to the customer, mayimmediately cancel the application for or discontinueservice to the customer without incurring liability for anyof the following reasons:

-Nonpayment of any sum due to the Company for servicemore than 10 days beyond the due date;

-Use of service in a fraudulent or suspectedfraudulent manner;

-A violation of or failure to comply with anyregulation governing the furnishing of service; or

-An order of the court or other government authorityhaving jurisdiction which prohibits the Company fromproviding service.

1.5 Credit and Deposit Requirements

A. The Company shall determine the credit standing of anapplicant for service using standard credit checkingmethods and by referring to information about theapplicant's prior telephone bill paying habits if theapplicant has had service before or by referring to tollcharges reasonably traced to the applicant but placed fromthe telephone of another if the applicant has not hadservice before. The Company may request that the customermake a cash deposit if the customer's credit historyindicates that he/she is a credit risk. The Company mayalso request a deposit from those customers with unknowncredit.

B. The amount of deposit required shall be at theCompany's discretion. An increase in the deposit amountmay be requested if increased usage or the customer'spayment record warrants it. The initial deposit shall bereceived in the Business Office before service will beestablished for the customer.

C. The Company shall pay interest on a deposit at theminimum rate established by the Commission. Interest shallaccrue following acceptance of the deposit, except that nointerest need be paid on deposits held less than thirty(30) days.

D. In the case of residential service, the deposit andaccrued interest shall be refunded upon request of thecustomer after twelve (12) consecutive months of payment bythe bill due date and refunded automatically after twenty­four months of consecutive payment by the bill due date.In the case of business service, the deposit shall berefunded after thirty-six (36) consecutive months ofpayment by the bill due date. Deposits may be refundedsooner at the Company's option.

E. Upon disconnection of service, the Company willpromptly and automatically refund the customer's depositplus accrued interest, or the balance, if any, in excess ofthe unpaid bills for service furnished by the Company.When the customer's deposit is applied to an unpaid bill,the Company shall render to the customer a statementshowing the bill then due and unpaid, the amount of thedeposit, together with the interest accrued thereon and theperiod covered thereby, and the balance due or remaining tothe credit of the depositor.

F. A transfer of service from one premises to anotherwithin the service area of the Company shall notnecessitate the requirement of a payment of a seconddeposit, unless a final bill has been issued at the firstaddress and the credit standing of the customer wouldotherwise require it.

G. In cases of a residential household that is divided dueto divorce or separation, the deposit will remain with theresidence where the service was being provided prior tosuch divorce or separation, unless other arrangements areagreed to by both parties.

H. The Company will keep a record of each cash deposituntil the deposit is refunded. The record will show:

1. The name and current billing address of eachdepositor;

2. The amount and date of the deposit;

3. Each transaction concerning the deposit.

I. Credit limits may be established by the Company fornew and existing customers based on credit history. In theevent the established credit limit is exceeded on anaccount, access to the Company's service will be restrictedwhere the facilities are available. Customers attemptingto access restricted services will be automatically routedto either a recorded announcement or a servicerepresentative for information regarding service restoral.

J. In the event a customer has not reached his/herinitially established credit limit, but has becomedelinquent in his/her payments, the Company may place arestriction on continued use of the Company's servicesuntil the customer is able to make satisfactoryarrangements with the Company. This restriction may be setlower than the customer's initial credit limit due tohis/her delinquent status. In the event that access isrestricted due to payment delinquencies, customersattempting to access the restricted services will beautomatically routed to either a recorded announcement or aservice representative for information regarding servicerestoral.

1.6 Obligations of the Customer

The customer shall be responsible for the following:

-Establishing their identity in the course ofcommunication as often as necessary;

-Establishing the identity of the person(s) with whomconnection is made at the called customer line(s)

1.7 Credit Allowances for Interruption

Customers who re-establish·an intrastate Direct Dial orToll Free Service (8XX) call which has been involuntarilyinterrupted after the station has been reached may receivecredit for the interrupted call, provided the Company isnotified of the interruption. Interruptions due to thefailure of power, equipment, or systems not provided bythe Company will not receive credit allowances.

2.1 Description

Direct Dial Service is a 1+ outbound long distance servicefurnished by the Company between points within the state ofNebraska.

2.2 Terms and Conditions

A. Direct Dial Service is generally offered twenty-four(24) hours a day, seven (7) days a week.

B. Direct Dial Service is offered from originatinglocations within the state of Nebraska. Calls may beplaced to points within the state.

C. This service is provided for use by the customer andmay be used by others, when so authorized by the customer,providing that all such usage shall be subject to theprovisions of this catalog.

D. Call timing begins when two-way communications areestablished. Answer detection is based on standard answersupervision methods. For billing purposes the minimum callduration is one minute. Usage after the initial period isbilled in sixty (60) second increments.

E. Rates and Charges are listed in Section 8 of theCatalog.

F. From time to time, the Company may offer specialpromotions waiving the monthly rates or installationcharges for certain features, services, or packages.

3.1 Description

Calling Cards may be assigned by the Company to enableusers to bill intrastate telephone calls to their Companyaccount. This service may be accessed using the 8XX numberprinted on the card.

3.2 Terms and Conditions

A. Calling Card Service is generally offered twenty-four(24) hours per day, seven (7) days a week.

B. Because of potential unavailability of complete billinginformation to the Company, Calling Cards shall have a sixmonth back billing period.

C. Operator services are currently routed to otherproviders.

D. Rates and charges are listed in Section 8 of thiscatalog.

E. From time to time, the Company may offer specialpromotions waiving rates or charges for certain features,services, or packages.

4. OPERATOR SERVICES

4.1 Description

Operator services are currently routed to other providersfor the following types of calls and services:

Person to Person - Customer dialed "O-" call where theoperator completes the call to a designated person orextension.

Calling Card - Customer dialed "O-" call that will bebilled to the caller's calling card. The call can becompleted by the caller who enters his/her own creditcard number or by the operator who enters thecustomer's credit card number.

Collect - Customer dialed "O-" call where the operatorcompletes the call and arranges billing to the calledtelephone number.

4.2 Terms and Conditions

A. Operator services are generally offered twenty-four(24) hours per day, seven (7) days a week.

B. These services are provided for use by the customerand may be used by others, when so authorized by thecustomer, providing that all such usage shall besubject to the provisions of this catalog.

5. DIRECTORY ASSISTANCE SERVICE

5.1 Description

Directory Assistance Service, as made available by theCompany through this tariff, provides, upon customerinitiated request from the Company's territory, publishedand non-listed telephone numbers within the state ofNebraska.

5.2 Terms and Conditions

A. Directory Assistance charges specified in this tariffapply when a customer initiates a call to DirectoryAssistance from the Company's serving territory andrequests a telephone number within Nebraska.

B. Directory Assistance charges shall be billed to thecustomer on a per call basis. A maximum of two listingsmay be requested per call.

C. Rates and charges are listed in Section 8 of thiscatalog.

D. Directory Assistance charges are not applicable to:

1. Calls placed from residence telephones where a memberof the customer's household has been certified by aqualified authority as unable to use a directory or fromthe business telephone of a certified customer whereother assistance is not available. A qualified authorityis defined as including doctors of medicine,optometrists, registered nurses, therapists, professionalstaffs of hospitals, institutions and public welfareagencies. Certification of physical disabilitysufficiently severe to prevent reading or usingconventional reading materials may also be made byprofessional librarians or by any person whose competencein this area is acceptable to the Librarian of Congressof the United States. This exemption is limited to one­hundred (100) free calls a month. Any exempted customermaking more than 100 calls a month will be billed at theregular Directory Assistance rate. The DirectoryAssistance provided under this exemption is for personaluse only and may not be resold.

6.1 Description

Toll Free Service is a customer switched telecommunicationsservice that permits toll free inward 8XX number callingfrom stations within the state of Nebraska to thesubscribing customer's station.

6.2 Terms and Conditions

A. Toll Free Service(8XX) is generally offered twenty-four(24) hours a day, seven (7) days a week.

B. Toll Free Service consists of an 8XX telephone numberassociated with a customer's station that can be calledfrom originating locations within the state of Nebraska.

C. Calls are dialed and completed without the assistance ofa Company operator and do not include:
