On-site preventative health screenings, administered by Interactive Health, will be offered at 18 Chemours locations. Employees who can’t make it to an on-site screening can complete the screening at a LabCorp location (contact Interactive Health to schedule an off-site screening).

Interactive Health will send the results of your tobacco screening to your home mailing address within two weeks. The letter will include next steps, if necessary.

Your screening will reveal critical health data points, such as blood pressure, total cholesterol, HDL, LDL, triglycerides, and glucose levels.

Attend an on-site health screening* It’s quick, free, and confidential. Once you complete your on-site screening, your health coach will review your results, discuss your lifestyle habits, and recommend programs and resources to help you reach your personal health goals.

Go to a LabCorp locationThere are two ways to request a LabCorp voucher:

1. By phoneCall Interactive Health Member Services at (800) 840-6100 between 8:00 a.m. and 6:00 p.m. You must complete testing by September 30, 2019.

Interactive Health Member Services will ask you at the time of your request whether you would like your LabCorp voucher emailed to you or mailed to your home address. If you want your LabCorp voucher mailed to your home address, please submit your request as soon as possible to allow time for the LabCorp voucher to arrive.

2. Register for your “Test at a Lab” voucher online Register or log-in to myinteractivehealth.com using Sponsor Code SAPV4. Click the “Schedule Your Health Evaluation” box, and then select “At a Lab.” Upon completing your request, you may choose to download the LabCorp voucher immediately, or have it mailed to your home address. If you prefer to have your voucher mailed to your home, please submit your request as soon as possible to allow time for the LabCorp voucher to arrive.

Alternative options for health screeningIf you are medically unable to participate in the health screening, which is required for you to pay lower medical insurance premiums under this wellness program, you may qualify for an opportunity to pay lower premiums through an alternative course of action which must be completed by September 30, 2019. Please contact Interactive Health at least two (2) weeks prior to this date at (800) 840-6100 or email [email protected] to ask our health management team about a reasonable alternative standard.

November 29 is the last day to enroll in the tobacco cessation program.

Enroll by November 29 and complete six (6) 30-minute phone-based coaching sessions within three (3) months of enrolling in the program to be eligible for lower medical insurance premiums.

When you participate in the health screening, you will need to certify whether or not you have used tobacco products.

If you certify that you have not used tobacco products, you will get a tobacco verification test. If your results are negative, you are eligible to pay lower medical insurance premiums. If your results are positive, you can avoid paying higher premiums by completing a tobacco cessation program.

If you certify you are a tobacco user, you will not be given the tobacco test. You can still receive the benefit of lower healthcare premiums as long as you complete the required tobacco cessation program, as well as the Biometric Screening and the Health Risk Assessment.

If you are planning on quitting, we’re ready to help. Enroll in “Tobacco Free for Life” now by calling (800) 840-6100 and avoid paying higher medical insurance premiums. You must complete at least six (6) 30-minute phone-based coaching sessions within three (3) months of enrolling in the program to continue to pay lower medical insurance premiums.
